Term breech and long-term morbidity -- cesarean section versus vaginal breech delivery.

STUDY: Perinatal morbidity and mortality of term fetuses have been discussed extensively both for vaginal breech delivery and cesarean section. However, information regarding long-term morbidity and psychomotoric development of these children are scarce. DESIGN: Data of 154 children delivered after breech presentation at our institution between 1988 and 1994 were analyzed using a specific, standardized questionnaire (Enzephalopathiefragebogen, Meyer-Probst) with emphasis on psychomotoric development and skills. Hyperkinetic disorders, social adaptation, emotional instability, and intelligence were evaluated as subcategories and compared to perinatal data. RESULTS: pO(2) and base excess (BE) in the umbilical artery were lower in the vaginal group. pH, body weight and placental weight showed no difference between groups. Psychomotoric development and skills did not differ between children delivered vaginally or abdominally. Perinatal variables did not allow a prediction of long-term morbidity. CONCLUSION: Route of delivery has negligible influence on the measured values in the umbilical artery and no influence on long-term morbidity of fetuses presenting breech.

Steroid hormone receptors and long term survival in invasive ovarian cancer.

BACKGROUND: Steroid hormone receptors are important determinants of prognosis and predictive behavior in tumor tissues of several origins. Since their role in ovarian cancer is still controversial, we investigated the prevalence and prognostic impact of the estrogen (ER) and progesterone (PR) receptors and combinations (ER+PR+, ER+PR-, ER-PR+, and ER-PR-) in a comparably large number of patients with a long clinical follow-up. METHODS: The present analysis included 186 patients with invasive ovarian carcinomas treated at the Department of Obstetrics and Gynecology of the Justus-Liebig-University Giessen between 1982 and 1996, the follow-up lasting up to 15.8 years (median 2.4 yrs). The expression of ER and PR was assessed by immunohistochemistry using alkaline phosphatase antialkaline phosphatase in microwave pretreated, formalin fixed, and paraffin embedded specimens of the primary tumors and was evaluated semiquantitatively using a standardized immunoreactive scoring system. Receptor expression and combinations were compared to clinical, histologic and prognostic factors, the tumor proliferation, and the clinical outcome. RESULTS: Kaplan-Meier survival analyses supported the favorable prognostic value of PR and its level of expression in ovarian carcinomas. Especially the ER-PR+ combination, which accounted for 10.2% of all tumors, showed a significantly superior prognosis when compared with all other combinations (survivors 15 of 19 vs. 67 of 167, log rank P = 0.009) and was associated with early stage, low ascites quantity, and higher tumor differentiation. Five-year survival rates were 13/16 (81.3%) for ER-PR+ tumors versus 58/128 (45.3%) for all other steroid hormone receptor combinations. Residual analysis proved the results. CONCLUSIONS: The determination of steroid hormone receptor status offers additional prognostic information in ovarian carcinomas. Especially the ER-PR+ phenotype predicts a favorable tumor biology and long term survival, probably reflecting functional effects on tumor proliferation, differentiation, and cellular apoptosis.

[Oncologic mistletoe therapy: physicians' use and estimation of efficiency].

BACKGROUND AND OBJECTIVES: In the field of unorthodox therapies in oncology, mistletoe extracts represent the most important method in Germany. In spite of its use for decades, there is no sufficient evidence for its efficiency. Since physicians have been identified to be the main providers of unconventional cancer therapies, the question of what experiences they have made using mistletoe extracts and other methods. PERSONS AND METHODS: In a cross-sectional study, 202 physicians in their private practices answered a structured, pretested questionnaire on unconventional cancer therapies, including their attitudes towards them and their judgement on efficiency. Response rate 80.2%. RESULTS: 79.2% of the physicians reported to provide unconventional cancer therapies, especially older and more experienced doctors. Among these, the rate of physician with a special preference of mistletoe extracts was 44.4%. The average probability to achieve complete or partial remissions with mistletoe extracts, eventually in combination with other unconventional methods was estimated to be 6% and 15%, thus receiving slightly lower estimates than other methods. With respect to changes in quality of life, use of mistletoe extracts was judged to be inferior to other methods (pT-Test = 0.063; pT-Test = 0.059). Furthermore, mistletoe extracts were significantly less frequently used because the physician was convinced of its efficiency (p = 0.025). CONCLUSION: Clinical studies to prove possible benefits of mistletoe extracts are mandatory. It remains unclear why this method has become so popular in spite of providers moderate judgments on efficiency.

The attitudes of physicians and oncologists towards unconventional cancer therapies (UCT).

Physicians represent the main providers of unconventional cancer therapies (UCT) in Germany. However, little is known about providers' characteristics, as well as their attitudes towards UCT. 833 questionnaires on this topic answered by general practitioners and hospital physicians were analysed. Providers differed significantly from non-providers with respect to gender (male>female, i.e. more male providers), age (older>younger), amount of subjective knowledge about UCT, place of work (office>hospital>university clinic), greater wish for coverage of UCT costs, the belief in future positive trends concerning UCT, the recognition of patients' demand for UCT, the number of patients seen per month and medical specialty (GPs>oncologists and radiation oncologists). UCT were not considered to be highly effective, but estimations varied considerably. Further investigations in this area, better education about UCT, training in coping strategies with the fate of cancer patients, and reasonable complementary treatments appear to be of the utmost importance.

Wishes and expectations of pregnant women and their partners concerning delivery.

AIMS: The study investigated factors related to the expectations and wishes concerning delivery of expectant parents. METHODS: A self-administered questionnaire on the relevant aspects of delivery was developed, pre-tested, and administered to 545 expectant mothers (n = 336) and fathers (n = 209) attending open house informational events at 3 hospitals around Giessen, Germany. RESULTS: Response rate was 96.3%. Three major areas of interest were identified and converted into scales: "management and obstetrical equipment" (ME; alpha = 0.81), "medical standards" (MS; alpha = 0.82), and "hospital conveniences" (HC; alpha = 0.78). Preferences of participants were influenced by age, gender and parity, as well as by different levels of state- and trait-anxiety. Expectant fathers focused more on HC of the hospital whereas ME, as well as MS, were more important to pregnant women, especially older women from rural areas with high state and/or trait anxiety (p < or = 0.05). However, MS were also found to be important for younger, nulliparous women with at risk pregnancy. Parents of high-risk pregnancies did not emphasize their wishes for ME (p < or = 0.05). CONCLUSION: Fashionable obstetrical equipment of delivery rooms, high medical standards, the reputation of the hospital, and certain conveniences are important issues for expectant parents. However, their importance varies with the above mentioned factors.

Self-concept, body image, and use of unconventional therapies in patients with gynaecological malignancies in the state of complete remission and recurrence.

OBJECTIVE: Are there differences in self-concept and body image in patients with cancer recurrence in comparison to patients with complete remission? What impact has cancer recurrence on use, users and non-users of unconventional cancer therapies? PATIENTS AND METHODS: One hundred and nine patients with no evidence of disease after gynaecological cancer and sixty-one patients with recurrent disease were analysed for self-concept with the Frankfurter Selbstkonzeptskalen and body image with the Frankfurter Körperkonzeptskalen. Use and motivation for unconventional therapies was assessed with a questionnaire. RESULTS: With respect to frequency of use and expected benefits of unconventional therapies no differences were observed between the groups. However, cancer recurrence was found to induce considerable changes of self-concept and body image, some indicating even positive changes due to cancer recurrence. CONCLUSION: It may be beneficial to consider body therapy and psychotherapy as a mean to improve body image and self-esteem in cases with cancer recurrence.

Role of dexamethasone dosage in combination with 5-HT3 antagonists for prophylaxis of acute chemotherapy-induced nausea and vomiting.

Dexamethasone (20 mg) or its equivalent in combination with 5-HT3 antagonists appears to be the gold-standard dose for antiemetic prophylaxis. Additional to concerns about the use of corticosteroids with respect to enhanced tumour growth or impaired killing of the tumour cells, there is evidence that high-dosage dexamethasone impairs the control of delayed nausea and emesis, whereas lower doses appear more beneficial. To come closer to the most adequate dose, we started a prospective, single-blind, randomized trial investigating additional dosage of 8 or 20 mg dexamethasone to tropisetron (Navoban), a 5-HT3 receptor antagonist, in cis-platinum-containing chemotherapy. After an interim analysis of 121 courses of chemotherapy in 69 patients, we have been unable to detect major differences between both treatment alternatives. High-dose dexamethasone (20 mg) had no advantage over medium-dose dexamethasone with respect to objective and subjective parameters of acute and delayed nausea and vomiting. In relation to concerns about the use of corticosteroids in non-haematological cancer chemotherapy, we suggest that 8 mg or its equivalent should be used in combination with 5-HT3 antagonists until further research proves otherwise.

Epicutaneous breast forms. A new system promises to improve body image after mastectomy.

Mastectomies will remain a treatment alternative for breast cancer in spite of efforts to perform more breast-conserving treatment. Restoration of body symmetry may then be an important issue, which can be achieved by surgical breast reconstruction or with an epicutaneous breast prosthesis. A new improved system has recently been developed, which is self-adhesive to the thorax wall. In this study we investigated the advantages and disadvantages of the new system. The body image of 55 patients after unilateral mastectomy was assessed before they entered the study and after approximately 6 months of experience with the system. The "Frankfurter Körperkonzeptskalen" plus additional questions concerning problems after mastectomy and the handling of the new breast form were used for the assessment. About 50% of the patients had problems with the adhesiveness of the breast form, which remains a problem to be solved. Patients who suffered badly from mastectomy, have a smooth and plane mastectomy scar, and who were not suffering from hot flushes, which can impair the adhesiveness of the prosthesis, were more likely to profit from the new self-supporting breast forms. Significant differences were observed in the scales "self-acceptance of the body", acceptance of the body by others" for the subsets of patients mentioned above. The differences were less pronounced in patients who were already familiar with another type of epicutaneous breast form, which is attached to an adhesive plate on the thorax wall by nylon touch and close fasteners. Most (90.7%) of the patients would recommend this new type of breast prosthesis for other patients with mastectomy. The concept of self-supporting breast forms is an improvement with respect to social and psychological rehabilitation. According to the manufacturer, the problem with adhesion has meanwhile been solved.

Invasion of cytotrophoblastic JEG-3 cells is stimulated by hCG in vitro.

Trophoblast invasion into the uterine wall is controlled by many factors. Previously, a human chorionic gonadotropin (hCG) receptor has been found to be expressed on invasive trophoblast as well as on choriocarcinoma cells implying a possible role for the hormone in trophoblast invasion. Therefore, this study examined the role of hCG in the invasion of trophoblastic (JEG-3) cells. Increasing hCG concentrations were applied in a trophoblast invasion model, JEG-3, through matrigel-coated filters. The proliferation was quantified by WST-1 cleavage assay. Cell migration was studied by examining the number of cells that had passed the uncoated porous (8-microm pore size) filters. After staining, filters were examined microscopically for cells on the underside of the membrane. A quantitative protease assay was also performed. Flow cytometric analysis of alpha5 and alpha6 integrin subunits, which are essential for interactions between cells and extracellular matrix, was performed. hCG increased significantly (P<0.01) the in vitro invasion of trophoblastic JEG-3 cells in a dose-dependent manner. Migration was also increased by hCG (P<0.01). However, cell proliferation remained unchanged. The second messenger analogue dibutyryl cAMP (db cAMP) and the cAMP elevating factor (forskolin) mimicked the effects of hCG by stimulating a dose-dependent increase of trophoblastic cell UEG-3) invasion. The collagenolytic activity of trophoblastic cells (EG-3) was increased by hCG stimulation. No changes were shown in the expression of alpha5 and alpha6 integrin subunits on JEG-3 cells. In vitro hCG is a regulatory factor of invasion and migration in trophoblastic JEG-3 cells, whereas proliferation is not influenced. The endogenous production of hCG by the trophoblast in vivo implies an autocrine control of invasion processes by hCG.

Invasion of cytotrophoblastic (JEG-3) cells is up-regulated by interleukin-15 in vitro.

PROBLEM: Trophoblast invasion into the uterus is controlled by many factors. Some cytokines (interleukin [IL]-1, IL-6, and IL-10) have been shown previously to play an important role in placentation. The human placenta is an important source of IL-15, although the cellular source of IL-15 in the placenta has not yet been specified. IL-15 influences cell adhesion and migration by redistributing adhesion molecules in lymphocytes and has been shown to have effects on endothelial cells and in some human tumors. METHOD OF STUDY: To study the role of IL-15 in trophoblast invasion, we investigated the effect of IL-15 (concentrations, 1-10 ng/ml) in a trophoblast invasion model (JEG-3 with matrigel-coated filters). Cell invasion was assessed using matrigel-coated filters and was expressed as the quotient of invading cells in comparison with the number of cells that had passed the control membrane. Cell migration was studied by examining the number of cells that had passed the filters without matrigel. Cell proliferation was quantified by a tetrazolium salt WST-1 cleavage assay. Matrix metalloproteinase (MMP)-1, MMP-2, and MMP-9 activities were measured by specific enzyme assays. RESULTS: IL-15 significantly (P < 0.05) increased the in vitro invasion of cytotrophoblastic (JEG-3) cells in a dose-dependent manner. There was a fourfold increase in the invasion at a concentration of 10 ng/ml of IL-15. Migration also was increased by a factor of 2.3 (P < 0.05). Cell proliferation, however, remained unchanged. The collagenolytic activity of cytotrophoblastic (JEG-3) cells was increased by IL-15 stimulation. A significant increase in MMP-1 concentration occurred after the incubation of JEG-3 cells with IL-15. No changes appeared in MMP-2, MMP-9, and tissue inhibitor of metalloproteinase-1 concentrations. CONCLUSIONS: Trophoblast invasion and migration, but not proliferation, are enhanced by IL-15. Our results suggest a role for IL-15 in the modulation of MMP-1 secretion by JEG-3 cells. Furthermore, we speculate, that IL-15 might be related to the changes of cell adhesion molecule phenotype during the process of invasion.

[Expression of cell adhesion molecules in the extravillous trophoblast in placentas of preterm pregnancies and in placentas at term].

Invasion of the human trophoblast is regulated by cell adhesion molecules (CAM) such as integrins and members of the immunoglobulin superfamily, which also play an important role in a number of immunological reactions. Abnormal trophoblast invasion of the uterus and its arterial system has been related to preterm delivery. We examined the differences of CAM expression in the extravillous trophoblast of preterm (n = 18) and term pregnancies (n = 21). Placenta and decidua frozen sections were examined by double-staining immunohistochemistry using antibodies against the immunoglobulin superfamily (ICAM-1, ICAM-2, ICAM-3, VCAM-1), integrins (alpha 2 beta 1, alpha 3 beta 1, alpha 4 beta 1, alpha 5 beta 1, alpha 6 beta 1) and cytokeratin. The percentage of immunopositive extravillous trophoblast cells and the intensity of immunoreactivity for the mentioned CAM antibodies was assessed. The expression of alpha 3 beta 1, alpha 5 beta 1 and VCAM-1 (p < 0.05) in the extravillous trophoblast of preterm placentas was lower than in normal placentas, whereas the expression of alpha 6 beta 1 in the extravillous trophoblast of preterm placentas was higher than at term (p < 0.05). No differences were observed for alpha 2 beta 1, alpha 4 beta 1, ICAM-1, ICAM-2 and ICAM-3. Our results show that there is a different expression of cell adhesion molecules in the extravillous trophoblast of placentas in preterm delivery. These differences in CAM might be associated with abnormal immunological and cell-cell interactions between mother and developing fetus and thus cause preterm labor and delivery.

[Violations of chemotherapy protocols for ovarian cancer--reasons and consequences].

In times when colony-stimulating factors were not available, delays of treatment or dose reductions were necessary, to assure that chemotherapy could be safely administered. In a retrospective analysis the effects of chemotherapy protocol violations on the survival of patients with ovarian cancer was evaluated. The serial courses of leukocyte counts were often the determinants for a protocol adequate chemotherapy in contrary to the thrombocytes but the serial platelet counts had no influence on protocol violations. Only time-related protocol violations have been found in 7.6% of the cases. There seems to be no apparent influence on patients' survival. However, accomplishment of treatment schedules, which may be regarded as a reaction towards unsatisfactory tumour response, at the initial visit alone or in combination with time-related protocol violations as well as tumour stage, course of tumour markers (CA 125) had a strong impact on survival while higher dosage levels produces only a trend towards improved survival. The use of growth factors will probably reduce the percentage of protocol violations caused by neutropenia, but it is questionable if it will reduce mortality due to tumour progression.

Changes in self-concept and body image during alopecia induced cancer chemotherapy.

Alopecia as a result of cancer chemotherapy has been reported to cause changes to the self-concept and body image. In a prospective longitudinal study, self-concept and body image were analysed in 29 patients after histological confirmation of gynaecological malignancy, mainly ovarian cancer, who were assigned to receive a complete-alopecia-inducing PEC combination chemotherapy (cisplatin 50 mg/m2, epirubicin 60 mg/m2, and cyclophosphamide 500 mg/m2 in 1 day every 28 days). The analysis was performed before the commencement of treatment and repeated when alopoecia was complete and after completion of therapy when patients had already experienced regrowth of hair, using the Frankfurt self-concept scales (FSKN) and Frankfurt body concept scales (FKKS). Significant differences were observed in the various evaluation scales FSAP (general ability to solve problems), FSSW (general self-esteem), SGKB (state of health), and SKEF (physical fitness). For all scales the results worsened during chemotherapy but did not return to normal or improve when patients experienced regrowth of hair. It was found that 73.3% of the patients did not feel as self-confident as before treatment and that for 46.6% alopecia was the most traumatic side effect of chemotherapy. Since there is no chemotherapeutic regimen or any other effective treatment that can prevent alopecia, either of the following conclusions can be drawn: the observed differences may not be related exclusively to alopecia, but also associated with coping processes initiated by chemotherapy and perhaps enhanced by alopecia; or the changes persist even after the discontinuation of chemotherapy. Regrowth of hair and other adaptive processes do not normalize or improve the impaired body image and self-concept.

Serum CA 125 levels and survival in advanced ovarian cancer.

We made a retrospective analysis of 85 patients with elevated serum CA 125 after surgery for ovarian cancer. Absolute CA 125 serum levels were a poor guide to prognosis. However, the ratio between the serum CA 125 after the first, second, or third course of treatment and the postoperative value was an excellent guide to prognosis. These were also independent and stable in the Cox Regression analysis.

Expression of cell adhesion molecules in the extravillous trophoblast is altered in IUGR.

PROBLEM: The invasion of trophoblast cells into the uterine wall and its arterial system is essential for the normal development of pregnancy. Cell adhesion molecules (CAM), such as the immunoglobulin superfamily and integrins, play a crucial role in a number of immunological reactions and in the invasion of the human trophoblast. Intrauterine growth restriction (IUGR) has been associated with abnormal trophoblast invasion. Therefore, the expression of CAM in the extravillous trophoblast of pregnancies complicated by IUGR might be different from normal pregnancies. METHOD OF STUDY: Normal (n = 21) and IUGR (n = 19) placentas were collected and stored at -70 degrees C. Immunohistochemistry (avidin-biotin complex peroxidase-doublestaining) of frozen tissue sections was performed using antibodies specific for the immunoglobulin superfamily vascular adhesion molecule-1 (VCAM-1; CD 106), intercellular adhesion molecule (ICAM-1) (CD 54), ICAM-2 (CD 102), ICAM-3 (CD 50), the integrins alpha 2 beta 1, alpha 3 beta 1, alpha 4 beta 1, alpha 5 beta 1, alpha 6 beta 1 and cytokeratin. The percentage of immunopositive extravillous trophoblast cells (EVT) and the intensity of the immunoreactivity for the various CAM and integrin antibodies was assessed. RESULTS: In IUGR placentas, there was less expression of VCAM-1 (CD 106), alpha 2 beta 1, alpha 3 beta 1, and alpha 5 beta 1 (P < 0.05) in the extravillous trophoblast than in normal pregnancies. Finally we observed for the first time that ICAM-3 was expressed on EVT and that its expression was markedly up-regulated in the EVT or IUGR placentas. No differences were found for ICAM-1 (CD 54), ICAM-2 (CD 102), alpha 4 beta 1 and alpha 6 beta 1. CONCLUSION: Our data show that there are significant differences in the expression of cell adhesion molecules of the extravillous trophoblast from IUGR and normal pregnancies. These differences might reflect changes in the immunological reactions and cell-cell interactions between mother and the developing fetus which could interfere with fetal growth.

Unconventional cancer therapy--survey of patients with gynaecological malignancy.

Although the benefits remain unproven, a considerable number of cancer patients use unconventional therapy, in many cases without the knowledge of the oncologist. At the oncological out-patient department of the women's clinic of the Justus-Liebig-University Giessen we conducted a survey of the various unconventional methods used. 38.8% (80/206) of the patients used unconventional therapies, mainly mistletoe extracts (50%), trace minerals (46%), megavitamins (39%), and enzymes (22%). The perceived etiology of cancer determined the choice for the various methods (p = 0.00074). Users of unconventional methods suffered significantly more from conventional therapy, had less faith in their doctors, and felt more nervous and emotionally unstable after the diagnosis of "cancer".

[Epicutaneous breast prosthesis after breast ablation].

INTRODUCTION: Restoration of body symmetry is a very important aspect after mastectomy to help patients coping with daily life. There are basically two possibilities for patients who do not wish breast reconstruction: either using breast forms that are suspended in a brassière or a new system in which the breast form is attached on an adhesive strip to the thorax wall. With burr crushers the breast form is attached to the strip. Brassières are not necessarily needed. The influence of improved prosthetics on the patients' self-concept and well-being was investigated. MATERIALS AND METHOD: 67 patients in postoperative care at the Universitätsfrauenklinik Giessen were randomized in control group using customary breast forms and the study group changing to self adhesive breast forms. Using the "Frankfurter Selbstkonzeptionsskalen" (Frankfurt Self-Concept Scales), plus additional questions concerning problems after mastectomy, self-concept was assessed before study and after 3 months. RESULTS: Significant differences with regard to the scale "Social Contact and Relations" (FSKU) were seen. Most patients were better satisfied with epicutaneous self-adhesive breast forms. Weight and movement behaviour of the self-adhesive breast form was considered natural and almost like a part of the body by 92%. 80% considered adhesive strips to be well tolerable. Thoughts about operative breast reconstruction became less important for 62%. 78% were happy to be able to wear more fashionable clothing. CONCLUSION: The new concept of self-adhesive breast forms is an improvement with respect to social and psychological rehabilitation.
