Origin of descending afferents to the rostral part of dorsal cap of inferior olive which transfers contralateral optic activities to the flocculus. A horseradish peroxidase study.

Neurons in the ventromedial tegmental area that project to the rostral part of the dorsal cap of the ipsilateral inferior olive were labeled by the technique of retrograde transport of horseradish peroxidase (HRP). Following iontophoresis of HRP into the rostral part of the dorsal cap a cluster of labeled cells are found ipsilaterally in the area ventromedial to the red nucleus as well as in the area between the substantia nigra and the medial lemniscus. Following HRP injections into the medial accessory olive just beneath the dorsal cap, labeled cells are found exclusively contralaterally in the deep layers of the superior colliculus and in the adjacent reticular formation. These findings clearly show that neuronal groups in the ventromedial tegmental area are involved in the pathway which from the contralateral eye via the rostral half of the dorsal cap conveys impulses to the flocculus, recorded as climbing fiber activation.

A case of tuberous sclerosis with calcification of angiomatous change in the white matter.

A 5-year-old Japanese boy had had infantile spasms at 5 months of age, and had intractable seizures, adenoma sebaceum and prominent tramtrack-like calcification in the left occipital pole at five years. A neurosurgical operation was performed for the resection of the left occipital focus. Pathological examination showed patchy glial fiber nets on the cortical surface, scattered dysgenetic giant cells both in the cortex and white matter, and calcified angiomatosis in the white matter. These findings correspond to those of tuberous sclerosis and calcified angiomatosis in the white matter.

Coronary collateral circulation as an important factor to modify the ischemic injury of the myocardium in coronary ligated dogs.

Effect of coronary ligation on epicardial and intramyocardial ST-segment voltage was investigated in 121 dogs. After ligation of the anterior descending coronary artery, marked ST-elevation, more than 2 mV, was detected in 102 dogs (Group-I) in epicardial lead of the area which was nourished by the ligated coronary artery. In 7 dogs (Group-II), coronary ligation produced negligible changes in ST-segment voltage in epicardial lead, while significant ST-elevation was observed in the leads of middle and inner layers of the myocardium. Systemic hemo-dynamic parameters showed no statistically significant difference between the 2 groups. Coronary collateral indices, however, revealed the significant difference between them. Systolic peripheral coronary pressure was 29.3 +/- 3.1 mmHg in Group-I and 59.2 +/- 10.6 mmHg in Group-II (P less than 0.05). Retrograde flow was also greater in Group-II (14.8 +/- 4.4 ml/min) than in Group-I (1.3 +/- 0.5 ml/min) (p less than 0.01). Diversion of retrograde flow produced a marked ST-elevation in Group-II. Restoration of retrograde flow to the myocardium immediately decreased the ST-segment voltage. These results suggest that spontaneously developed collaterals have an important function for the prevention of the occurrence of ischemic injury after coronary ligation in dogs.

Computed tomography in tuberous sclerosis--with special reference to relation between clinical manifestations and CT findings.

Concerning the clinical signs and symptoms, we noticed skin white macula in 87%, adenoma sebaceum in 53%, mental retardation in 75%, retinal phacoma in 54% and seizure in 92%. Numbers of nodules ranged from zero to 11, and showed no correlation with aging. Half of the cases showed slight ventricular dilatation, one quarter showed moderate dilatation and the remainder were normal. There was no definite correspondence between size of the ventricle and number of nodules. There were relatively many cases with normal mental state in the group with normal ventricles. In adult cases a large ventricle does not always mean poor prognosis in mental state. Almost all of the cases with cortical atrophy had mental retardation. In the group with moderate dilatation infantile spasms were the most frequent type of seizure. On the contrary, grand mal was most frequent in the normal group. As for the position of nodules there was no difference between the normal and dilated groups. Nodules were predominantly seen in the lateral aspect of the body, trigone of the lateral ventricle and adjacent to the foramen of Monro. Asymmetrical lateral ventricles were noticed in 18%. Incidence of laterality increased as dilatation proceeded. It was larger on the left side in 9 out of 11 cases. We have no reasonable explanation of this tendency. There was one case with a tumor which was verified by enhanced CT. There were five cases with brain anomalies.

Effects of graded coronary constriction on regional oxygen and carbon dioxide tensions in outer and inner layers of the canine myocardium.

This study was undertaken to investigate the effects of graded coronary constriction on regional gas tensions of the myocardium. In 12 open chest dogs, tissue carbon dioxide (PtCO2) and oxygen (PtO2) tensions were measured simultaneously in outer and inner layers of the myocardium using a mass spectrometer. In normal condition, higher PtO2 and lower PtCO2 were observed in outer layer than in inner layer. With application of coronary constriction, increase in PtCO2 and decrease in PtO2 were observed in both layers of the myocardium, but the response to the ischemic stimuli by applying coronary constriction in inner layer was different from that in outer layer. Severe coronary constriction, more than 90% in its diameter, was necessary to produce significant changes in both gas tensions in both layers of the myocardium. Decrease in PtO2 was found in the condition of less severe coronary constriction and to be greater in inner layer than in outer layer of the myocardium. In terms of the changes in PtCO2, inner layer was also more susceptible to the ischemic stimuli than outer layer. The greater and earlier elevation of PtCO2 in inner layer than in outer layer is regarded as one of the possible mechanisms of the reduction of myocardial contraction in the early stage of myocardial ischemia.

Transcatheter embolization of autologous clot in the management of bleeding associated with fractures of the pelvis.

Extraperitoneal hemorrhage, associated with a fracture of the pelvis, is a major cause of death in pedestrian accidents. Transfusion alone may be unsatisfactory. Direct control of bleeding may be required. Surgically, this may be technically difficult or inadequate. Transcatheter embolization of autologous clot was used to control hemorrhage in three patients with such a fracture. If laparotomy is required immediately, arteriography of the pelvic area may be done postoperatively, If laparotomy is not performed, arteriography may define pelvic bleeding sites. Transcatheter embolization of autologous clot controls hemorrhage from branches of the hypogastric artery.

Influence of monoamine oxidase inhibitor on the induction of persistent estrus by androgen in the rat.

Effect of monoamine oxidase inhibitor (MAOI) on the induction of persistent estrus by testosterone propionate (TP) was studied in Wistar strain female rats. Daily treatment with MAOI for the 1st 10 days of life accelerated the occurrence of persistent estrus in rats given TP at 8 days of age. The onset of persistent estrus occurred at 57.3+/2.0 days of age in these animals compared to 92.6+/5.5 days in saline-treated controls. The contribution of hypothalamic monoamines to the induction of persistent estrus by TP treatment in neonatal rats will be discussed.
