Tumorigenicity study of disodium glycyrrhizinate administered orally to mice.

Disodium glycyrrhizinate (DG) was administered at concentrations of 0.15 (maximum tolerated dose), 0.08, 0.04 or 0% in the drinking-water to groups of 50, 70, 60 and 60 male B6C3F1 mice, respectively. Female groups, each consisting of 50 mice, were given DG in the drinking-water at concentrations of 0.3 (maximum tolerated dose), 0.15, 0.08 or 0%. Treatment was continued for 96 wk and the experiment was terminated at wk 110. There was no difference between treated and control groups in tumour incidence, in the latent period before tumours appeared or in the distribution of different types of tumour. Thus the long-term oral administration of DG to mice did not yield any evidence of chronic toxicity or tumorigenicity.

Lymphocyte subsets in HLA-DR2-positive narcoleptic patients.

On the basis of our recent finding that all narcoleptic patients were HLA-DR2 positive, peripheral blood lymphocyte subsets were examined in 30 HLA-DR2 positive narcoleptic patients by using monoclonal antibodies and a flow cytometry. The percentages of OKIa1+ cells and OKM1+ cells increased significantly, while no quantitative changes were observed in the T cell subsets examined in the present study. No major immunological abnormalities which altered the T cell subpopulations quantitatively were apparent in narcolepsy.

T-cell subsets in patients with gastric cancer.

Peripheral blood T-cell subsets of 70 patients with untreated gastric cancer and 55 age- and sex-matched normal controls were studied by using monoclonal antibodies and flow cytometry. The number of T cells (Leu 4+ cells) was significantly decreased in stage IV patients. Statistically significant decreases in the number of Leu 3a+ cells and in the Leu 3a+/Leu 2a+ ratio were also found in stage IV patients. Stage I-III patients showed no marked imbalance in T-cell subsets. Quantitative changes in T-cell subsets in the advanced stage may be one of the factors responsible for the immunosuppression in advanced gastric cancer.

Human histocompatibility leukocyte antigen (HLA) haplotype frequencies estimated from the data on HLA class I, II, and III antigens in 111 Japanese narcoleptics.

In order to deduce the predominant haplotypes in Japanese narcoleptics, we have studied a total of 111 Japanese patients with narcolepsy and six multiple-case families for HLA class I and class II antigens, and for class III HLA-linked complement markers. In Japanese narcoleptics, the most frequent haplotypes were B35-DR2, B15-DR2, and B51-DR2. These haplotypes were rare in normal Japanese population. In contrast, the most frequent haplotype of HLA-DR2 in normal Japanese, A24-C blank-Bw52-C4A*2 B*Q0-BF *S-C2*C-DR2-DQw1, had a decreased frequency to one-third of the normal controls. Haplotypes B35-DR2, B15-DR2, and B51-DR2, which were more frequent among Japanese narcoleptics, were different from the haplotype found more frequently among Caucasoid narcoleptics, A3-Cw7-B7-DR2-DQw1. Haplotype analysis on six families showed that B35-DR2 and other rare haplotypes in normal Japanese were associated with narcolepsy. There were four cases without any signs of narcolepsy among 19 subjects with the disease susceptibility haplotypes. This finding suggests an incomplete penetrance of hypersomnia. Haplotype analysis of family members was also useful for the early detection of the high risk children to narcolepsy.

Clinical use of the measurement of airway resistance during spontaneous respiration.

Airway resistance was measured during spontaneous respiration in healthy smokers and patients with chronic obstructive pulmonary disease, and the relationship between the airway resistance and respiratory frequency was examined. No significant difference was observed between the airway resistance in smokers and nonsmokers. In the chronic obstructive pulmonary disease group, patients with pulmonary emphysema had highest airway resistance. The determination of airway resistance during spontaneous respiration appears to be a very useful, noninvasive method of pulmonary function testing.

[Surgical treatment of Borrmann type 4 (diffuse) gastric cancer].

A total of 435 cases with stage II-IV gastric cancer resected in our department was investigated. Cases with Borrmann 4-type cancer showed the poorest prognosis in all stages compared with other types of cancer. None of three drugs-PS-K, FT-207 and Levamisole-was specifically effective on Borrmann 4-type cancer in two randomized controlled trials. A study of in-vitro lymphocyte responsiveness to mitogens suggested the presence of a suppressive factor in the serum of the patient with poorly differentiated cancer. An immunofluorescent study using monoclonal antibodies against large and small intestinal mucin antigens showed strong positivity in poorly differentiated cancers.

[2 cases of solitary metastasis to the large intestine from gastric carcinoma].

Localized metastatic foci of the colon from gastric cancer have been reported in only a few cases. Tow cases of gastric cancer were reported. One relapsed in the form of a localized lesion at the sigmoid colon (Case 1) and the other at the rectum and the transverse colon (Case 2) after curative resection of gastric cancer. From the results of clinical examinations obtained from these cases, it may be reasonable to postulate that peritoneal dissemination and/or lymphatic invasion of cancer was the main cause of solitary metastasis to the large intestine.

[A case of minute carcinoma associated with an atypical epithelial lesion of the stomach and histopathological analysis].

A patient with minute carcinoma associated with an atypical epithelial lesion (ATP) of the stomach is reported. The resected stomach from this 81-year-old male exhibited a 1.3 X 1.0 cm elevated ATP; microscopic examination revealed a minute (0.4 mm) carcinoma. The lesion was differentiated tubular adenocarcinoma and was suggestive of one-gland carcinoma. It consisted of one faveola in the upper half and multibranched , compound alveolar glands in the lower half, structurally mimicking the initial stage of intestinal metaplasia. The lesion existed in small metaplastic foci; the surface foveolar epithelia of the lesion showed no cancerous changes. These findings suggested that this minute carcinoma occurred in the initial stage of intestinal metaplasia.

Clinico-pathological studies on ovarian metastasis from gastric cancer.

Thirty autopsied female patients with gastric cancer were investigated clinico-pathologically, regarding ovarian metastases. According to the histological pattern of the ovarian metastases, there were five types; no-metastasis in five, capsular type (cancer involvement confined to the capsule) in three, hilar type (carcinoma is evident in the hilus but not in the cortex or medulla) in four, multifocal type (multifocal growth of cancer in the cortex and/or medulla) in eight and diffuse type (diffuse proliferation of cancer) in ten. Although marked peritoneal dissemination was noted in 27, the capsule of the ovaries was usually intact. These results suggest that in the development of ovarian metastasis, cancer cells first reach the hilus, then spread into the medulla or cortex multifocally, and later grow diffusely. While cancer cells reach and cover the capsule of the ovary in many instances, they seldom break and invade it. All seven patients, except one unknown patient, with no-metastasis and with the capsular type were postmenopausal, while one hilar, two multifocal and five diffuse type women were premenopausal. This indicates an "affinity" of ovarian tissue for cancer cells, in younger patients.

[Management of rectal and ureteral obstruction following gastrointestinal cancer].

During the period from June 1973 to August 1982, thirteen patients with gastric cancer and seven patients with colorectal cancer received surgical treatment due to the obstruction of the rectum or ureter. Of thirteen gastric cancer patients, 11 underwent the formation of the artificial anus for rectal obstruction. Nephrostomy was performed in 4 gastric cancer patients with ureteral obstruction. Seven patients with colorectal cancer underwent the formation of the artificial anus for rectal obstruction. In the recurrent cancer of the gastrointestinal tract, obstruction of the rectum or ureter mainly occurred due to peritoneal invasion or tumor formation. During laparotomy, OK-432, a Streptococcal preparation, was administered with a large dose of 100 K. E. to the patients with advanced or recurrent cancer foci. The prognosis for those patients was poor, and the main reason for poor survival was the advancement of disease in the cases of rectal or ureteral obstruction.

Angiotensin-converting enzyme in diseases of the liver.

Serum angiotensin-converting enzyme activity was measured in various diseases of the liver. Activity increased in progressive order in patients with chronic persistent hepatitis, chronic aggressive hepatitis, and liver cirrhosis. Activity was increased also in patients with acute hepatitis. On the other hand, patients with fatty liver had normal angiotensin-converting enzyme activity and patients with extrahepatic obstructive jaundice showed subnormal activity. Although the mechanism for these enzymatic changes in diseases of the liver remains to be elucidated, serum angiotensin-converting enzyme determination may be useful in the diagnosis of diseases of the liver under certain conditions.

Comparative studies on cytological and histological evaluations of disseminating peritoneal metastasis in gastric cancer.

Macroscopic disseminating peritoneal metastasis of gastric cancer (P) was investigated in relation to its compatibility with microscopic findings (p) in 52 (P1 19, P2 21, P3 12) cases among 240 gastrectomized cases by cytological, histological, clinical and postmortem findings. In total, numbers of the evaluations for yes (P is p), probable (P is probably p), possible (P is possibly p), probably not (P is probably not p) and no (P is not p) were 28 (53.8%), 8 (15.4%), 14 (26.9%), 2 (3.8%) and 0, respectively, These results indicate that macroscopic disseminating peritoneal metastasis is compatible with microscopic findings, in most cases. Although histology is the most contributory factor for the evaluation of peritoneal dissemination, cytology of lavaged saline from the Douglas pouch also is important.
