MRI visualization of the aortic nipple.

The aortic nipple, a small "pseudotumor" adjacent to the aortic arch, is the left superior intercostal vein. In our series of 40 patients it was demonstrated in six; all had a right upper thoracic mass and four had a superior vena caval syndrome. Coronal magnetic resonance images (MRI) of the thorax were superior to all other imaging methods in demonstrating the nipple. It is concluded that in patients with masses in the right upper chest, coronal MR chest examinations are valuable in demonstrating an aortic nipple, a sign of impending present superior vena caval or innominate vein obstruction.

[Follow-up of combined radiotherapy and chemotherapy of advanced bladder cancer using magnetic resonance tomography. Initial results].

A combination of cis-platinum and radiotherapy was applied in five patients with advanced bladder carcinomas. The patients were examined by nuclear magnetic resonance imaging prior to and following therapy. Corresponding to cell-kinetic considerations regarding tissues with slow cell turnover, a maximum therapy effect is shown about nine months after the end of local therapy. Smaller control intervals are recommended in case of bladder carcinomas with lymph node metastases. Due to the possibility of multiple slice imaging, the nuclear magnetic resonance technique is particularly suited for the monitoring of bladder carcinomas. Sagittal and coronary slices show a better image of tumoral extension at the roof and floor of the bladder than axial computed tomography. Lymph node metastases are represented relatively late by both methods.

[Visualization of the left superior intercostal vein by MR tomography].

A retrospective study of 56 MRT chest examinations showed the left intercostal vein in its horizontal trajectory in 3 patients with left mediastinal masses compressing the superior vena cava. In 2 patients the posterior trajectory of the horizontal segment was clearly visualised. In the third case the anterior part, namely the insertion of the left superior intercostal vein into the left brachiocephalic trunk, was demonstrated.

[Magnetic resonance tomography in injuries of the cervical spine].

Twenty patients who had suffered spinal trauma were examined by magnetic resonance tomography. Fifteen patients with first degree trauma in Erdmann's classification showed no abnormality. Magnetic resonance tomography of the cervical spine appears to be a suitable method for investigating patients with whiplash injuries. It is indicated following severe flexion injuries with subluxations and neurological symptoms, since it is the only method that can demonstrate the spinal cord directly and completely and show the extent of cord compression. For patients with thoracic trauma and rapidly developing neurological symptoms, magnetic resonance tomography is ideal for showing post-traumatic syringomyelia. Magnetic resonance tomography following whiplash injuries is recommended if plain films of the cervical spine show any abnormalities, as well as for the investigation of acute or sub-acute neurological abnormalities. The various findings are discussed.

[Inflammatory changes in shoulder arthrography].

Arthrography of the shoulder joint in so-called periarthritis humeroscapularis often shows nodular contrast defects, irregular defects in the bursa and contrast-filling of irregular lymphatics; these are characteristic signs of a synovitis. Arthrography can provide an exact diagnosis, not only in suspected post-traumatic rotator cuff, tears, but also in other painful conditions that limit movement. It is superior to CT in demonstrating inflammatory changes.

[Combined low-dose radiation and adriblastin therapy of lung metastases].

Two patients with lung metastases are presented who were submitted to a combined low-dose radiotherapy and adriamycin monotherapy with good palliative results. The mode of action as well as the experimental examination results are discussed. The ineffectiveness of single low-dose chemotherapy is demonstrated by the non-response of lung metastases beyond the irradiation fields (heart block, diaphragm). Both patients were resistant to chemotherapy.

[Gorham-Stout syndrome--progressive osteolysis with viscerocranial involvement].

Two rather young patients with spontaneous massive mandible osteolysis are presented, i.e. a woman suffering from a rhinogenous liquor fistula and recurrent meningitides, and a young man. The authors discuss etiology, pathologic anatomy, clinical and radiological course, as well as the combined surgical and radiotherapeutical treatment. The pathologic process shall be stopped by the combination of both therapy methods. A radiation dose of 30 to 50 Gy is recommended.

[Comparative CT study of the anterior cruciate ligament in direct projection and using sagittal reconstruction].

The authors present an indirect method for the visualization of the anterior crucial ligament, using the method of sagittal reconstruction. Positioning in the so-called tailor's seat is often impossible in freshly injured persons. The abdominal positioning described in this article, with external rotation of the leg, is tolerated by almost all patients. Sagittal reconstruction yields adequate information in fresh lesions and if there is an obstruction to bending. However, this method is diagnostically not as infallible as the direct visualization. The authors discuss whether additional reconstructions on different planes and with pixels of different thickness (so-called thick pixel reconstruction) may be able to contribute to improved diagnostic results in future. In addition, the authors compare their own examination results with normal and pathological findings published by other authors.

[Treatment results in radio-iodine therapy of the autonomous adenoma of the thyroid gland (author's transl)].

In order to control the results of radio-iodine therapy, the autonomous adenomas treated in our clinic from 1961 to 1975 with radioactive iodine on 105 patients have been subjected to follow-up examinations and compared with the results obtained by other authors, especially Swoboda et al. (23), who treated 185 patients with corresponding follow-ups. In both groups, efforts were aimed at achieving successful treatment via high single-dose radio-iodine therapy. The article discusses the indication, method, results and posttherapeutic care involved. Both the examined groups demonstrate good treatment results (in 95 or 98%, respectively, of our cases and of the cases treated by Swoboda et al., the autonomous adenoma was eliminated). This proves that high single-dose therapy with radioactive iodine (I131) is a successful method of treating an autonomous adenoma, thus representing a therapeutic alternative to surgery.

[The Baker's cyst - a diagnostic problem? (author's transl)].

Precise definition of Baker's cyst has been prevented by variety of synonyms. Following anatomical description, Baker's determination, and investigations of myself one should differentiate between the rupture of capsule, bursa semimembranous-gastrocnemia, and Baker's cyst because they are clearly independent from the pathologic-anatomical point of view. Clinical importance of Baker's cyst in connection with further diseases of the knee joint and therapeutical possibilities are discussed.
