Apparent Michaelis-Menten kinetic parameters of phenytoin in pediatric patients.

In a retrospective study of 104 pediatric patients (0.5-16 yr) with seizures, the effects of concurrently administered anticonvulsants on Michaelis-Menten kinetic parameters of phenytoin were investigated. Individual kinetic parameters (Km and Vmax) for phenytoin were estimated from at least two reliable steady-state plasma concentration and dose data. Km tended to increase and to become more variable as the number of coadministered anticonvulsants increased. Significant difference was found in a group of patients taking three or more coadministered anticonvulsants. On the contrary, Vmax was not affected by those drugs in the higher age groups. Additive inhibitory rather than inductive effects of commonly used anticonvulsants appeared likely to cause an increase in Km. These observations suggest that dosage regimen adjustment of phenytoin based on the averaged Vmax in a given pediatric age group rather than the Km is recommended for a particular patient with inadequate therapeutic control of seizure or toxicity of this drug.

Micro-determination of total phthalate esters in biological samples by gas-liquid chromatography.

A method was investigated in which all of the phthalate esters in biological samples were determined as phthalic acid by gas-liquid chromatography. The method is based on the separation of phthalate esters from the sample with n-hexane, saponification of the esters with an alkaline ethanolic solution to give phthalic acid, purification of the acid by extraction with diethyl ether and column chromatography using silica gel, and conversion of the acid into bis(2,2,2-trifluoroethyl) phthalate with a 2,2,2-trifluoroethanol solution containing boron trifluoride. The derivative obtained is highly sensitive to an electron-capture detector, giving a sensitivity of 0.1 pg. Biological samples fortified with di(2-ethylhexyl) phthalate at levels of 5-100 ppb were analyzed, with recoveries of 70-100%.

Identification and quantification of 6 beta-hydroxydexamethasone as a major urinary metabolite of dexamethasone in man.

Identification of 6 beta-hydroxydexamethasone as a major urinary metabolite of dexamethasone in man has been accomplished by nuclear magnetic resonance spectroscopy and gas chromatography-mass spectrometry. Mass fragmentographic measurements revealed that more than 30% of the intravenously or orally administered dexamethasone dose was excreted in the 24-h urine as 6 beta-hydroxydexamethasone, while only a small fraction of the dose was excreted as unchanged dexamethasone and its glucuronic acid conjugate.

Walker-Warburg syndrome in a Japanese patient.

We report the first Japanese female patient with Walker-Warburg syndrome. She had generalized muscle hypotonia with hydrocephalus due to Dandy-Walker malformation and bilateral microphthalmia with opaque corneas. She had severe motor and mental retardation. Muscle histology reflected advanced changes of muscular dystrophy. We discuss the relationship between Fukuyama congenital muscular dystrophy and Walker-Warburg syndrome, both of which fall within a spectrum of developmental abnormalities with a common cause. In Fukuyama congenital muscular dystrophy, ocular abnormalities are less severe.
