The effect of triathlon on urinary excretion of enzymes and proteins.

In order to investigate the effect of triathlon and renal function of normal subjects, we evaluated the excretion of urinary enzymes and proteins before and after triathlon. From 6 subjects samples were obtained 24 hours after the first urine collection. We performed quantification of urinary total protein, beta2-microglobulin (beta2-M), N-acetyl-beta-D-glucosaminidase activity (NAG), and concentration of urinary creatinine from each participant. There was a significant increase in urinary total protein excretion immediately after triathlon (p<0.01). The urinary beta2-M and NAG excretions after triathlon were higher than the initial values. Post-exercise proteinuria in one subject persisted until the next morning, whereas the increased excretion of urinary beta2-M and NAG returned to the pre-exercise level at least 24 hours after triathlon. It appears to be reasonable to presume that glomerular damage may persist in some subjects who do heavy exercise.

A case of primary giant calculus in female urethra.

Urinary calculus is rarely seen in the urethra and is usually encountered in men with urethral stricture or diverticulum. Primary urethral calculi are extremely rare in females. We describe a case of a giant urethral stone impacted in the urethra of a 103-year-old female.

Hemangioma of the bladder with extravesical extension.

Bladder hemangioma is a rare benign tumor occurring in any age group. We report the case of a 76-year-old woman having a bladder hemangioma. On cystoscopic examination no typical features of the bladder hemangioma were found. Biopsy of the lesion revealed a cavernous hemangioma. A partial cystectomy was performed for the control of macroscopic hematuria.

Comparison of the nephrotoxicity between ioversol and iohexol.

To evaluate the renal toxicity of intravenous administration of ioversol and iohexol, the urinary N-acetyl-beta-glucosaminidase (NAG) to creatinine ratio was measured before and after drip infusion pyelography. Thirty patients received intravenous ioversol and 29 patients received iohexol. The urinary NAG to creatinine ratio tended to increase after respective contrast media: 5% increase in the ioversol-treated, and 8% in the iohexol-treated patients. There were no significant differences in these values; the nephrotoxicity of ioversol appeared to be just as insignificant as that of iohexol.

Pathological changes of traumatic dislocated testis.

We present a 17-year-old patient with a right testicular dislocation. In this case, dislocated testis had been considered as hematoma and therefore remained untreated for 4 months. The patient underwent right orchiopexy 4 months after the injury. Intraoperative testicular biopsy revealed impaired spermatogenesis and existence of many alternative Sertoli cells, but no atrophy of seminiferous tubules. Six months after the operation, semen analysis of the patient revealed the deformity of most sperms. Eight months after the operation, rebiopsy of the right testis disclosed slight improvement in the spermatogenesis of the testis. Damage of the testis appears to be not severe in dislocation and may be recovered by repositioning.

Clinical and retrospective evaluation of Eviprostat: a non-hormonal and non-neuropharmacological agent for benign prostatic hyperplasia.

Eviprostat has been used as a non-hormonal and non-neuropharmacological treatment for benign prostatic hyperplasia (BPH) in Japan. We evaluated the clinical efficacy of Eviprostat in patients with symptomatic BPH and the anti-inflammatory effect of this drug was investigated by retrospective evaluation of TUR specimens. Clinically, Eviprostat subjectively relieved obstructive symptoms of BPH. Objective improvements were also demonstrated by ultrasonographic evaluation of prostatic volume and urinary flow rates. Histologically, preoperative administration of Eviprostat improved the degree of prostatic inflammation. This drug probably can have a place in the treatment of patients with mild or moderate symptoms of outflow obstruction.

Recurrence of urethral stricture after single internal urethrotomy.

Recurrence of urethral stricture after single internal urethrotomy was investigated in 66 patients with special reference to some factors which would influence restricturing. In the present study, recurrence rate was 16.7% (11 patients). Recurrences occurred within an average of 11.9 months. Factors that had no influence on restricturing were age, aetiology, site of the stricture and duration of indwelling catheterization. On the other hand, stricture length appeared to influence the outcome (chi 2, p < 0.001). Short stricture (< or = 10 mm) showed only 4.4% recurrence rate, while 42.9% in long stricture (> 10 mm). Internal urethrotomy appeared to be effective particularly when the stricture was short, while it seemed difficult to treat long strictures by single internal urethrotomy.

Bilateral ovarian metastasis from left renal cell carcinoma.

We report a case of left renal cell carcinoma with bilateral ovarian metastasis which was demonstrated 3 years after radical nephrectomy. The patient is alive without any evidence of disease 36 months after bilateral salpingo-oophorectomy.

Penile block via subpubic space for children who underwent superficial operation of the penis.

Penile nerve block via the subpubic space was performed to relief the postoperative pain of children who underwent superficial surgery of the penis. Thirty-one male children were subdivided into two groups. After dorsal split procedure had been carried out under general anesthesia, 16 patients underwent subpubic penile nerve block (group 1), while 15 patients had no additional anesthetic procedure (group 2). Time to the first postoperative pain in group 1 was statistically longer than that in group 2 (p < 0.05). Most patients in group 1 did not require postoperative analgesics during the first 24 h after surgery. This technique appeared to be effective in relieving the postoperative pain of children who underwent superficial operation of the penis.

[Clinical evaluation of the combination of carumonam and fosfomycin in the treatment of complicated urinary tract infection].

Carumonam (CRMN), the first monobactam antibiotic in Japan, has excellent activity against gram-negative bacteria and is useful in the treatment of urinary tract infections. However, it may be insufficient in the treatment of complicated urinary tract infections because of the increase in isolation of gram-positive bacteria, and it may be necessary to co-administer antibiotics active against gram-positive organisms to achieve a broader spectrum of coverage in connection with severe infections. The combination of CRMN and fosfomycin (FOM) was evaluated for its effectiveness and safety at the Department of Urology, Yamagata University Hospital and 7 affiliated hospitals. Clinical efficacy was assessed on 64 patients with complicated urinary tract infection according to the Criteria for Clinical Evaluation of Antimicrobial Agents in UTI (3rd. ed.) recommended by the Japan UTI Committee. Clinical efficacy was evaluated as excellent in 16, moderate in 32, poor in 16, with an overall clinical effectiveness rate of 75.0%, which is superior compared with CRMN alone. Of the total of 92 bacterial strains isolated, 66 (71.7%) were eradicated. Subjective adverse reaction was seen in 1 patient (1.4%), as nausea and anorexia. Slight increases in serum GOT and GPT ware recorded in 5 patients (7.1%). These findings disappeared after the termination of administration without treatment. The combination of CRMN and FOM might therefore be useful in the treatment of complicated urinary tract infections.

[Renocolic fistula caused by xanthogranulomatous pyelonephritis: a case report].

Renocolic fistulas caused by typical xanthogranulomatous pyelonephritis are rare. We present a case of renocolic fistula caused by xanthogranulomatous pyelonephritis. A 58-year-old woman was admitted to the Department of Urology, Sakata City Hospital, with the chief complaint of pain in the left flank region. The plain film showed no calculi. An intravenous pyelogram showed left non-functioning kidney. A left retrograde pyelogram demonstrated a communication between the left renal calyx and the descending colon. A computed tomography done after the retrograde pyelography revealed contrast medium in the descending colon and a heterogenous left renal mass and thickness of Gerota's fascia. On the basis of the radiologic studies a diagnosis of renocolic fistula was made. The patient underwent a nephrectomy with closure of the fistula of the colon. The pelvicaliceal system of the resected kidney was distended and contained yellow fatty like tissue. Histologically xanthogranulomatous pyelonephritis was confirmed. The patient made an uneventful recovery.

[Clinical study on long acting amoxicillin in urinary tract infection].

Amoxicillin (AMPC), a widely used oral penicillin exhibites a strong bactericidal activity and a high level of safety. In the present study we administered L-AMPC, long acting AMPC granule, to 18 cases with urinary tract infections and obtained the following clinical results. 1. In all cases with acute simple cystitis, the effect was excellent. 2. Of 12 cases with chronic complicated UTI, the effect was excellent in 1 case, good in 4 cases and no effect was noted in 7 cases. The efficacy rate was 41.7%. 3. No side effects nor abnormal values in the laboratory examinations were noted. 4. Sixteen of 18 cases reported that this preparation was very agreeable to take compared with other powder type drugs, capsules and tablets. From these results we can confidently state that L-AMPC is a highly useful drug combining the excellent properties of AMPC, the convenience of b.i.d. dosage which improves patients compliance and satisfactory efficacy.

[Clinical experience with gentamicin in complicated urinary tract infections (author's transl)].

Gentamicin (GM) was intramuscularly administrated to 25 patients with complicated urinary tract infections in doses of 20-120 mg a day for 4-16 days. The results obtained are as follows: 1. The clinical results of GM were excellent in 4, good in 3, fair in 1, poor in 3 and indeterminate in 14 patients out of 25 patients. The effective percentage was 63.6% (7/11). 2. The clinical results of GM against 20 isolates were excellent in 4, good in 3, fair in 1, poor in 3 and indeterminate in 9 isolates. The effective percentage was 63.6% (7/11). 3. In laboratory examinations, BUN elevation was noted in 1 patient, GOT elevation in 2 patients and GPT elevation in 1 patient during GM therapy. No other abnormal findings were observed in this clinical study.
