Metacarpal fracture fixation with absorbable polyglycolide rods and stainless steel K wires: a biomechanical comparison.

Self-reinforced polyglycolide (SR-PGA) rods have been used widely for fracture fixation as an alternative to metallic implants. However, because of its insufficient strength and strength retention, the use of SR-PGA has been restricted to fractures or osteotomies of low stress bearing cancellous bone. To investigate its usefulness in fixing the fractures of cortical bone, the mechanical strength of a human metacarpal fracture fixed with SR-PGA rods was evaluated. The surgical constructs were immersed in buffered saline solution and maintained at body temperature (37 degrees C). The mean initial bending stiffness of the specimens fixed with the SR-PGA rods was 61% of that of the specimens fixed with Kirschner wires. The bending stiffness of the specimens fixed with the SR-PGA rods decreased gradually, and most of the specimens lost their mechanical stability when immersed for 4 weeks. When SR-PGA rods are used to fix the fracture of human cortical bone, such as diaphyseal fracture of the metacarpal bone, it should be protected by additional means.

Progressive cystic degeneration of the spinal cord following spinal cord injury.

From a group of 520 spinal cord injury patients treated at Rancho Los Amigos Hospital, two cases of progressive myelopathy secondary to cystic degeneration of the spinal cord have been identified. The cyst may dissect proximately to produce progressive neurologic deficit. Surgical treatment with shunting can allow stabilization and improvement with return of newly lost function.

CT evaluation of hip trauma.

Selection of treatment for fracture dislocations about the hip is based on the stability of the joint space, congruity of fracture fragments, the presence or absence of intraarticular fragments, and the overall condition of the patient. However, the condition of the acetabulum, femoral head, and joint space is frequently difficult to evaluate on conventional radiographs of the pelvis. To provide the orthopedist with more information on which to base treatment, 13 patients had computed tomography (CT) after accidents resulting in trauma to the hip and pelvis. The CT scan was believed to have significantly influenced the treatment in four of the patients and provided useful information in several others. CT was found to be helpful especially for evaluating presence or absence of intraarticular osseous loose fragments after reduction of the dislocated hip for evaluating congruity of the joint space after fractures of the femoral head and acetabulum. CT also provided useful information concerning the adjacent soft tissues and associated pelvic fractures.

Considering the needs of carers: a survey of their views on services.

This paper describes a survey undertaken in Derbyshire. A total of 244 carers responded to a postal questionnaire enquiring about the services they needed, how they rated the services used, how they coped generally, and whether they would need long-term day, respite or residential care. The responses suggested that carers were keen to express their views. Most wanted advice on benefits, and satisfactorily rated the services they received. Those who found it hardest to cope were caring for elderly parents or children, and had other family or work commitments. The authors conclude with a plea for consultative research to play an integral role in service planning and evaluation.

Unity is strength in joint research project.

The advent of the purchaser/provider split produced a number of challenges for a coordinated approach to supporting research. This article describes a collaboration between North Derbyshire Health Commission and Chesterfield and North Derbyshire Royal Hospital NHS Trust in bringing together purchaser and provider. A research interest group spanning purchaser and provider organisations has been responsible for a number of initiatives including training days, a research database and a resource pack for would-be researchers which aim to raise the profile of research across North Derbyshire.
