Economic considerations of the use of new anesthetics: a comparison of propofol, sevoflurane, desflurane, and isoflurane.

UNLABELLED: Cost control in anesthesia is no longer an option; it is a necessity. New anesthetics have entered the market, but economic differences in comparison to standard anesthetic regimens are not exactly known. Eighty patients undergoing either subtotal thyroidectomy or laparoscopic cholecystectomy were randomly divided into four groups, with 20 patients in each group. Group 1 received propofol 1%/sufentanil, Group 2 received desflurane/sufentanil, Group 3 received sevoflurane/sufentanil, and Group 4 received isoflurane/sufentanil (standard anesthesia) for anesthesia. A fresh gas flow of 1.5-2 L/min and 60% N2O in oxygen was used for maintenance of anesthesia, and atracurium was given for muscle relaxation. Concentrations of volatile anesthetics, propofol, and sufentanil were varied according to the patient's perceived need. Isoflurane, desflurane, and sevoflurane consumption was measured by weighing the vaporizers with a precision weighing machine. Biometric data, time of surgery, and time of anesthesia were similar in the four groups. Times for extubation and stay in the postanesthesia care unit (PACU) were significantly longer in the isoflurane group. Use of sufentanil and atracurium did not differ among the groups. Propofol patients required fewer additional drugs in the PACU (e.g., antiemetics), and thus showed the lowest additional costs in the PACU. Total (intra- and postoperative) costs were significantly higher in the propofol group ($30.73 per patient; $0.24 per minute of anesthesia). The costs among the inhalational groups did not differ significantly (approximately $0.15 per minute of anesthesia). We conclude that in today's climate of cost savings, a comprehensive pharmacoeconomic approach is needed. Although propofol-based anesthesia was associated with the highest cost, it is doubtful whether the choice of anesthetic regimen will lower the costs of an anesthesia department. IMPLICATIONS: Cost analysis of anesthetic techniques is necessary in today's economic climate. Consumption of the new inhaled drugs sevoflurane and desflurane was measured in comparison to a standard anesthetic regimen using isoflurane and an IV technique using propofol. Propofol-based anesthesia was associated with the highest costs, whereas the costs of the new inhaled anesthetics sevoflurane and desflurane did not differ from those of a standard, isoflurane-based anesthesia regimen.

Detection of human papillomavirus type 16 DNA and of antibodies to human papillomavirus type 16 proteins in children.

We have measured markers of human papillomavirus type 16 (HPV 16) infection in children (1-10 years of age) who were hospitalized for reasons unrelated to papillomavirus infection. Genital and buccal swabs obtained from 79 children were tested for the presence of HPV 16 DNA by PCR. Low-level positivity was found in 34 donors, twice as often in oral than in genital swabs. There was no sex-specific difference, but there was a trend towards a higher positivity rate with young age. Serum antibodies (IgG) were measured by ELISA based on peptides derived from the HPV 16 early proteins E4 (one peptide), E6 (two peptides) or E7 (two peptides) in 75 children and by ELISA based on virus-like particles in 66 children. Low-positivity rates were found for E6 (5.1%), E7 (2.5%) or capsid proteins (1.5%), but 20.3% of the sera reacted with the E4-specific peptide. There was no correlation between sero-positivity and the detection of HPV 16 DNA. In those instances where HPV DNA positivity in young children represents true infection and not environmental contamination, we speculate that this infection is accompanied by low-level virus replication that does not induce a measurable antibody response. Reactivity to the E4 protein is likely due to cross-reacting antibodies directed either against E4 proteins of other HPV types or against unrelated antigens.

Electrochemical properties of platinum, glassy carbon, and pyrographite as stimulating electrodes.

Presently, platinum, platinum-iridium, and carbon (glossy and pyrographite) are the preferred materials to be used as stimulating electrodes. Electrochemical tests revealed higher thresholds with Pt-Ir, which possibly are a result of excessive connective tissue growth. A porous structure appears to be preferred especially if the electrode materials are smooth and activated glassy carbon. When comparing power consumption, glassy carbon was found to be a superior electrode material.

Threshold measurements using stimulating electrodes of different materials in the skeletal muscles of cats.

In the skeletal muscle of cats, semispherical stimulating electrodes having a radius of 1.2 mm deliver a mean threshold current of 0.15 mA, corresponding to 1.7 mA/cm2. These values are doubled within a period of four weeks after chronic implantation. In the case of non-polarizable electrodes the mean threshold voltage increases from 108 mV to 171 mV. The lowest individual values range between 50 mV and 60 mV at the time of implantation. The highest value is around 500 mV after four weeks. The high values have been observed with strongly polarizing ELGILOY electrodes. The chronic threshold energies vary between 3 x 10(-8) Ws for activated vitreous carbon electrodes and 17.5 x 10(-8) Ws for ELGILOY electrodes. There are two reasons for the low threshold energy of activated vitreous carbon electrodes. One reason is their low polarization, and the other is their better compatibility. Connective tissue layers with a thickness between 25 micron and 50 micron are found around well healed carbon electrodes.

[Determining the Bacillus subtilis biomass increase coefficient in weightlessness].

This paper presents the results of a microbiological experiment carried out by the Soviet and GDR scientists onboard Salyut-6. The experiment was performed using a Bacillus subtilis suspension in the Jena unit. The purpose of the experiment was to study the time-course variations of the cell biomass increase in zero-g. The cell culture development was measured with respect to the utilization rate of glucose or casein hydrolyzate in the nutrient medium and the rate of protein accumulation in cells. It has been shown that the rate of biomass increment in zero-g lags behind the 1 g level. It can be concluded that the decreased metabolic activity of bacterial cells in zero-g is associated with changes in the cell population distribution and physicochemical parameters of the nutrient medium.
