[Congenital absence of the right main pulmonary artery--a case report].

A 32-year-old man was admitted to hospital for investigation of bloody sputum. Chest roentgenogram showed a decrease in the volume of the right lung, with mediastinal shift to the right. Chest CT examination revealed narrowing of the right main pulmonary artery. Bronchofiberscopic examination was normal. Pulmonary arteriogram demonstrated complete absence of the right pulmonary artery. Cardiac catheterization showed no concomitant cardiac malformation. Therefore, this case was diagnosed as congenital absence of the right main pulmonary artery.

[A case of pulmonary arteriovenous fistula treated by subsegmentectomy with CUSA].

A 35-year-old female who had no symptom was admitted to our hospital because of an abnormal shadow in the middle area of the right lung in a chest X-ray film. The chest tomography, CT and the angiography revealed a pulmonary arteriovenous fistula. It located in the right upper lobe, and was successfully resected by subsegmentectomy of S3b using CUSA. The results indicated that the CUSA was useful in the surgical treatment of the pulmonary arteriovenous fistula which had many vascular anastomoses.

Lipomas of the peripheral lung--a case report and review of the literature.

Bronchial lipomas are uncommon benign neoplasms of the lung and the number of these tumours is only 109 cases in the international literature (108 cases reported in English and one in German) up to date. Moreover, most of the tumours occur in the proximal portions of lobar or segmental bronchi: the references in the literature to peripheral lipoma of the lung make up only 6 cases. Peripheral lipomas of the lung are thought to originate from fatty tissue in the wall of peripheral bronchi. We present a further case of peripheral lipoma and analyze the reported 7 cases.

Effects of the yeast extract components pyrroloquinoline quinone and aspartic acid on vitamin B12 production in Klebsiella pneumoniae IFO 13541.

The production of vitamin B12 from carbohydrates, peptone, casamino acid, etc., by intestinal bacteria was investigated. Klebsiella pneumoniae IFO 13541 was the most efficient strain for vitamin B12 production, which depended exclusively on the concentration of yeast extract added to the medium. A concentrated solution of yeast extract (1 ml) was chromatographed on a Sephadex G-25 column (1 x 180 cm) and eluted with H2O (eighty fractions of 3 ml each were collected). It was found that fractions in which bacterial growth was most prevalent also exhibited the highest amount of vitamin production. The effectiveness of yeast extract was shown by the participation of pyrroloquinoline quinone and aspartic acid in the growth stimulation and in the vitamin B12 production in this strain.

[An immunoradiometric assay of the serum TPA from patients with primary lung cancer and a comparison with the conventional double antibody method].

The serum level of tissue polypeptide antigen (TPA) has been measured by a new immunoradiometric assay (IRMA) method that has been recently developed using a specific antibody, and compared with the results obtained by the conventional double antibody method, using a polyclonal antibody. Both methods gave essentially the same results, i.e. the serum TPA level in patients with primary lung cancer was higher when compared with non-malignant pulmonary diseases and healthy individuals (p less than 0.01). However the positive rate and average values of TPA with patients of non-malignant diseases were found to be lower by the new IRMA method than by the conventional method. This may be due to the improvement in specificity and accuracy of this newer method of assay, as they also statistically decrease within the cut off level (110 U/L) estimated in non-malignant diseases and healthy individuals. Therefore, it has been concluded that the IRMA method is more suitable than the conventional method for diagnosis of primary lung cancer.

[The management of chest injuries].

The mortality from chest injuries is so high due to severe physiologic imbalance that an immediate and accurate diagnosis of the injured organ and prompt treatment can salvage the patient from the strategy. This study comprises 1329 injured cases including 145 patients with crushed chests. The cause of injury was traffic accident in 537 cases (40.5%), fall or degradation in 332 cases (25%). There was a correlation between the cause of injury and age, as that traffic accident was a major one in young aged and fall in elders. Treatment against crushed patients included 150 surgical operations, 206 plaster bandages, 56 drainage of thoracic, peritoneal and cranial cavities. Thoracotomies performed in patients with flail chest (2), lung contusion (4), rupture of the bronchi and diaphragm (each 1) and for evacuation of clotted hemothorax in 3 patients. The prognosis of all these patients was good. Lastly we conclude that since the prognosis of injured patients depends on how fast the patient can be carried to the hospital and how quickly the physician or surgeon can evaluate the trauma and institute a prompt treatment, the education of the primary staff is the most important.

[The effect of epidural injection with morphine on the post-thoracotomy respiratory function].

Patients undergoing thoracotomy experience severe post-operative pain and marked respiratory impairment, which causes pulmonary atelectasis and pneumonia. The effects of epidural injection on postoperative pain and respiratory function were examined in this study. The group undergoing epidural injection of 3 mg morphine (at the end of operation, 09oo and 21oo for the next 3 days) included 37 patients, while the control group involved 16. The number of required analgesics on the operating day and next three days were compared between the two groups. And postoperative vital capacity (VC), forced expiratory volume in the first second (FEV1), maximum mean flow (MMF) were compared with preoperative value. Patients receiving epidural morphine required significantly less analgesics throughout the postoperative periods (p less than 0.01). The morphine injected group had significantly better value in VC and FEV1 in the first two postoperative day (p less than 0.01), while significance were seen only in the first postoperative day in MMF (p less than 0.01). It seems that epidural morphine is highly effective in alleviating pain and improving respiratory function in post-thoracotomy patients. These effects help the expectoration of sputum especially in senile patients. As the side-effects of epidural morphine, urinary retention, nausea, vomiting and itching were seen in few patients. No serious side effect such as hypotension or ventilatory depression were seen.

Primary squamous cell carcinoma of the stomach: a case report with a review of Japanese and Western literature.

A case of a 75 year-old male with primary squamous cell carcinoma of the stomach is reported. It is extremely rare to see squamous cell carcinoma developing in the stomach, without being accompanied by a component of adenocarcinoma. Up to the present, 18 Japanese and 62 Western cases of this type of carcinoma have been reported in the literature. The origin of this malignancy has not been well elucidated yet and thus, several plausible hypotheses have been proposed. In this presented case, the tumor consisted of only squamous cell carcinoma and the focus of squamous metaplasia was not found histologically in the adjacent mucosa. Therefore, it may be considered that the carcinoma arises from misplaced squamous cell nests of the stomach.

Combined therapy with FK-506 and cyclosporine for canine lung allotransplantation: immunosuppressive effects and blood trough levels.

The therapeutic and adverse effects of FK-506 and cyclosporine used in combination were examined in 35 adult mongrel dogs with allotransplantation of the left lung. The experimental animals were allocated to six groups: group A, optimal dose of FK-506 (n = 5, FK-506 0.1 mg/kg/day, intramuscularly); group B, optimal dose of cyclosporine, (n = 5, cyclosporine 20 mg/kg/day, by mouth); group C (n = 5, FK-506 0.03 mg/kg/day, intramuscularly); group D (n = 5, cyclosporine 6 mg/kg/day, by mouth); group E (n = 5, FK-506 0.03 mg/kg/day+cyclosporine 6 mg/kg/day); and group F (n = 10, FK-506 0.05 mg/kg/day+cyclosporine 10 mg/kg/day). The animals were killed on postoperative day 28 or when extreme deterioration had developed. In groups C and D, the grafts underwent moderate to severe rejection. The treatment in group E resulted in insufficient immunosuppression, although additional effect was noted. The rejection was suppressed at least as strongly in group F as in groups A and B. One death occurred in each of groups A and F from hematemesis. The blood concentrations of FK-506 and cyclosporine in group E were not higher than in groups C and D. The whole blood concentrations of FK-506 deviated less, and they were superior to the serum concentrations as a monitor. The combination of FK-506 and cyclosporine at one half of their optimal doses suppressed the rejection almost completely and did not increase the adverse effects.
