The reproducibility of computerized boundary analysis for measuring optic disc pallor in the normal optic disc.

The optic discs of eight normal subjects were photographed on three separate visits under widely varying conditions. Each photograph was scanned by a television camera, converted to digital form, and then analyzed using computer image analysis techniques. The computer was programmed to delineate the boundary around the optic disc and the area of pallor. The percentage of disc pallor was then computed across the entire disc and for each disc quadrant. The reproducibility of the procedure was determined by evaluating several photographic parameters: camera flash intensity, film type, magnification, and centering of the disc in the photograph. The focus adjustment on the television scanner was also varied. Under carefully controlled conditions, results showed that the reproducibility of measuring the total percent of disc pallor was 2%. Results for reproducibility are also given for less than optimal conditions and for manual planimetry on the same set of photographs.

[Transformation of ovarian yolk sac tumor to immature teratoma following chemotherapy].

The patient, a 28-year-old woman, in her ninth week of pregnancy, was operated on for stage Ia, mixed germ cell tumor (grade 3 immature teratoma + yolk sac tumor) of AFP decreased to the normal level. Eight months later, an intrapelvic mass and raised AFP were found. The extirpated recurrent tumor in the pouch of Douglas was a grade 2 immature teratoma with no yolk sac element. FAM chemotherapy was given again, and the patient is alive and well after taking oral UFT. As in testicular germ cell tumors, ovarian germ cell tumors can be converted to a more differentiated tumor following chemotherapy.

[Phase II study of recombinant human leukocyte A interferon on urogenital cancer patients].

A phase II study on recombinant human leukocyte A interferon (rIFN-alpha A) was carried out in 30 patients with urogenital cancers. Each patient received rIFN-alpha A by i.m. injection every day for at least 4 weeks. The initial daily dose was 3 X 10(6) U, being escalated at intervals of 3 days or more up to 50 X 10(6) U. The results are summarized as follows: In aged patients, the daily dose appropriate for everyday i.m. injection was considered to be 9 X 10(6) U or below, judging from the adverse reactions observed. According to Koyama and Saito's response criteria, partial response (PR) and minor response (MR) were obtained, respectively, in 3 and 1 out of 12 patients with renal cell carcinoma, while PR was seen in 1 out of 9 with urothelial cancer. No response was observed in patients with testicular cancer and in those with prostatic cancer. Various kinds of adverse reactions were recognized and each patient showed one reaction or more. Fever, fatigue, leukopenia, anemia, thrombocytopenia and elevation of GOT and GPT were observed relatively frequently. Among these, fatigue and thrombocytopenia were regarded as dose limiting factors.

Demonstration of p35 in hairy cell leukemia of Japanese patients.

Using SDS polyacrylamide gel electrophoresis (SDS-PAGE), several coworkers isolated a unique membrane protein that they named p35 from hairy cells of white patients with hairy cell leukemia. In the current study using a similar technique, p35 was identified in hairy cells from 2 of 4 Japanese patients with hairy cell leukemia. Further, p35 was immunoprecipitated from the leukemic membrane proteins of one of these two Japanese patients with an anti-hairy cell serum prepared against hairy cells of a typical white patient. Because both Japanese and white patients share the unique membrane antigen, it is proposed that the Japanese have the hairy cell leukemia as whites despite some differences in laboratory features.

[Primary carcinoid tumor of the uterine cervix associated with positive serotonin reaction].

A 46-year-old female with primary cervical carcinoid is reported. She was well with no recurrence for more than 5 years after surgery and radiation therapy. Pathologically, the tumor was composed of solid nests including rosette-like structures and tubular formations. Multiple argyrophil granules and a few argentaffin granules were revealed histochemically and serotonin was demonstrated by formalin-induced fluorescence.

[A case of so-called plasma cell granuloma of the stomach--demonstration of monoclonal immunoglobulin (IgM, K) in the plasma cells].

A patient with a surgically removed plasma cell granuloma of the stomach is reported. The patient was a 54-year-old male, the lesion was IIc-like on the anterior wall of the antrum. Histologically conspicuous features were marked thickening of the submucosa by fibrosis, hyperplastic lymph follicles and massive infiltration of mature plasma cells. However, immunohistochemical study using the PAP technique demonstrated the monoclonal nature of the infiltrating plasma cells which were positive for IgM, k. This is the fourth case of plasma cell granuloma reported in the literature, however, our findings also suggested that this might be an early stage of plasmacytoma.
