Hairy cell leukemia. Enzyme histochemical characterization, with special reference to splenic stromal changes.

Seven spleens and two peripheral blood specimens from eight patients with hairy cell leukemia were examined with enzyme cytochemical and histochemical methods. Hairy cells consistently exhibited acid phosphatase and tartrate-resistant acid phosphatase. However, nonspecific esterases characteristic of monocytes and histiocytes were consistently absent or very weak. beta-glucuronidase and cytoplasmic membrane-bound ATPase were positive in four cases, suggesting a possible relationship to the B-lymphocytic series. Fundamental splenic changes were accumulation of hairy cells and benign macrophages within the pulp cords, with resulting extreme expansion of the cords. Abnormally well developed ellipsoids were identified around the sheathed arteries within the cords. Sinuses, specifically delineated with the NASDA reaction, were atrophic and often destroyed. No cytogeneologic relationship was found between sinus endothelial cells and hairy cells. The pulp cords are the primary site of involvement of the spleen in hairy cell leukemia. A simultaneous proliferation of neoplastic cells, histiocytes and reticulum fibers accounts for the splenomegaly and clinical hypersplenism characteristic of the disease.

Splenic pseudosinuses and hepatic angiomatous lesions. Distinctive features of hairy cell leukemia.

In 14 cases of hairy cell leukemia unique vascular lesions not previously reported were observed in surgically resected spleens and hepatic biopsies. In all 14 spleens there were variably prominent distended spaces filled with erythrocytes and resembling dilated sinuses. These structures appeared to be lined by hairy cells,and studies utilizing histochemical and enzymatic methods confirmed that they were in fact pseudosinuses lacking the endothelial cells and ring fibers of normal splenic sinuses. These changes were not present in normal spleens or in spleens involved by other types of chronic leukemia, and appear to be qualitatively specific for hairy cell leukemia. In three of five hepatic biopsies similar angiomatous lesions were present in multifocal clusters resembling hemangiomas. Their pathogenesis may be related to possible adherence of hairy cells to reticulum fibers in involved tissues.

Alkaline phosphatase-positive malignant lymphoma. A subtype of B-cell lymphomas.

Alkaline phosphatase (ALP) activity was evaluated histochemically and cytochemically in concert with immunologic technics in 60 cases of non-Hodgkin's lymphomas and lymphocytic leukemias. Surface membranes of neoplastic cells were positive for ALP only in certain B-cell malignancies: 3 of 6 lymphocytic lymphomas of intermediate differentiation, 4 of 13 nodular lymphomas, and 1 of 7 Burkett's lymphomas. All other B-cell tumors, including chronic lymphocytic leukemia, well-differentiated lymphocytic lymphoma, and diffuse "histiocytic" lymphoma, were ALP-negative. The neoplastic cells of Sezary syndrome and lymphoblastic lymphoma were also consistently negative for ALP. In control lymph nodes ALP-positive lymphocytes were present only in primary follicles and in mantle zones of secondary follicles. ALP-positive lymphomas appear to be neoplastic counterparts of these normal lymphocytes, not only cytochemically, but also with respect to their morphologic and immunologic characteristics. Furthermore, histochemical inhibition tests suggested that the ALP activity demonstrated may reflect a newly recognized, unique isoenzyme.

[Clinical studies with minocycline intravenous instillation. Chemotherapy in patients with renal impairment (author's transl)].

In the present studies, we had an opportunity to review the effects of minocycline intravenous instillation for the treatment of moderate disorder of renal functions. No accumulation of minocycline was observed in the repeated administration of this drug instilled intravenously up to 7 days in the patients with moderate disorder of the renal functions. Also, no particular abnormality was observed in the tests of the liver, kidney and hematology. For patients with moderate Ccr 30 approximately 60 ml/min, as entered into the present study, the usual dosage of minocycline intravenous instillation for less than 7 days would not cause adverse reaction on renal functions.

Non-endemic Burkitt's lymphoma. A B-cell tumor related to germinal centers.

To investigate the nature of non-endemic Burkitt's lymphoma, we examined neoplastic cells from eight American patients for receptors for sheep erythrocytes (E), complement (EAC), and Fc fragment of lgG (igGEA), and for surface immunoglobulins (Slg) and hydrolytic enzymes. In addition, we reviewed 47 biopsies and 17 autopsies from American patients to ascertain patterns of involvement by tumor in lymph nodes, spleens and Peyer's patches. Neoplastic cells in all cases studies bore monoclonal surface immunoglobulins of the igM class. Receptors for EAC and igGEA were identified on a minority of the cells. Little or no hydrolytic enzyme activity was demonstrable. These results indicate that, like Burkitt's lymphoma in Africans, this histologically identical tumor in American patients consists of B lymphocytes. In 10 biopsies and two autopsies, germinal centers were selectively involved by tumor, suggesting that these neoplastic cells may be related to some B lymphocytes of normal germinal centers.

Ocular sarcoidosis: evaluation of intraocular findings.

Intraocular lesions occurred in 55 of 70 sarcoidosis patients (78.6%) and their intraocular findings were biomicroscopically analyzed. Intraocular lesions were observed bilaterally in 54 of 55 patients (98.0%) and these various lesions occurred simultaneously or within a short period in almost all tissues of the eyeball. From these characteristics a new attempt was made to correlate the presence of sarcoidosis with intraocular findings. A gross positive correlation was revealed between the number of active intraocular lesions and the presence of sarcoidosis. Four to six active, bilateral intraocular lesions were found only in sarcoidosis. Although the correlation was not statistically tested, it should be clinically very useful in managing patients with suspicious intraocular lesions. Among the causes of visual disturbances secondary glaucoma was the most frequent. In 41 (37.2%) eyes various degrees of intraocular hypertension were observed. Impairment of aqueous outflow due to nodular infiltration of trabeculae was the fundamental mechanism in the rise of intraocular pressure. It should be emphasized that this peculiar trabecular finding can be identified only by a trained ophthalmologist and is very frequent in and characteristic of ocular sarcoidosis.

Enzyme histochemical investigations of human malignant lymphomas.

26 cases of malignant lymphomas and 23 other lymphoreticular conditions were investigated enzyme histochemically. Each type of malignant lymphoma revealed a different enzyme histochemical pattern characteristic of its type. These features are not only applicable to differential diagnosis but also suggest clues to the understanding of histogenesis and nature of malignant lymphomas.
