Monoclonal antibody analysis of keratin expression in carcinomas of sweat glands.

Characteristics of keratins of five carcinomas of sweat gland origin were immunohistochemically investigated with several antikeratin monoclonal antibodies with differing specificities. Specimens were obtained from two cases of mucinous carcinoma of the skin, two cases of classic type of eccrine adenocarcinoma, and a case of eccrine porocarcinoma. The tumor cells of mucinous carcinoma expressed only simple epithelial keratins. In a case of eccrine adenocarcinoma, simple epithelial keratin 19 was diffusely expressed. The expression of the other simple epithelial keratins was confined to the luminal cells, whereas the remaining tumor cells further expressed stratified epithelial keratins. Eccrine porocarcinoma and a second case of eccrine adenocarcinoma did not express simple epithelial keratins, although stratified epithelial keratins were diffusely expressed. These data suggest that carcinomas of sweat glands express various combinations of simple and stratified epithelial keratins. Development of additional data along these lines may help to further define their classification.

Histochemistry in psoriasis. 5'-Nucleotidase in psoriatic parakeratotic horny layer.

The 5'-nucleotidase activity in psoriatic and normal human epidermis was studied in comparison to acid phosphatase activity. The optimum pH in normal human epidermis was about 5.0 at room temperature. The activity of both enzymes was found to be high in the transitional zone. Acid phosphatase (non-specific) activity was strongly positive in the psoriatic parakeratotic horny layers whereas 5'-nucleotidase activity in that area was completely absent. The results suggest that the enzyme which degrades nucleoside-5'-phosphate to nucleoside and inorganic phosphate is not acid phosphatase but 5'-nucleotidase. Nuclear preservation in psoriatic hyperkeratosis was attributed to absence or inactivation of specific enzymes of nuclear degradation, such as 5'-nucleotidase, rather than acid phosphatase.

Lichenoid dermatitis due to chlorpromazine phototoxicity.

A 75-year-old woman had lichenoid and bullous lesions in the sun-exposed areas. She had a history of cerebral embolism and received chlorpromazine (CPZ) until consultation at our clinic. Patch tests with CPZ were negative, but photopatch tests were positive at relatively high concentrations of CPZ and ultraviolet light radiation sites of 310, 340 and 370 nm. Rapid improvement of the lesions occurred upon discontinuation of CPZ and the avoidance of exposure to sunlight. From these results, the patient was considered to show lichenoid eruptions due to CPZ phototoxicity rather than photoallergy.

Isolated epidermolytic acanthoma. A histological study.

A solitary scrotal lesion of isolated epidermolytic acanthoma was studied by optical microscopy. The lesion displayed typical granular degeneration. The tumor parenchyma comprised several foci extending deep in the cutis to the continuation of the invaginated epithelium. Serial sections showed the parenchyma was free from acrosyringium and epidermis proper. The findings with hematoxylin-eosin stain and Fontana-Masson stain indicated that the tumor had originated from portion B of the hair follicle.

Lipid droplet of sebaceous carcinoma. Electron microscopic study utilizing glycol methacrylate-glutaraldehyde-urea procedure.

The ultrastructure of lipid droplets of sebaceous carcinoma (Meibomian gland carcinoma) was studied by conventional methods and a lipid-retention procedure. Two different structures were clearly differentiated by the latter technique. The peripheral part of the lipid droplet was composed of whirled lamellar structures (myelin-like structures) while the interior appeared to be continuously homogenous. The lamellar structures were presumed to have come from smooth-surfaced endoplasmic reticulum, and to transform to the amorphous material-triglyceride-during maturation of the lipid droplets. A true limiting membrane or membranous shell around the lipid droplet was not deceted.

Juvenile melanoma--a case report and histogenetic investigation.

A case of juvenile melanoma which appeared to be completely intradermal was studied by optical and electron microscopy. The histological dopa reaction was negative and melanin granules could not be detected with Fontana-Masson stain, but detailed electron microscopic examination revealed melanosomes very similar to those found in intradermal nevus cells. The tumor was therefore thought to be composed exclusively of melanogenic nevus cells, and participation of schwannian nevoblasts could be excluded. This view would support the concept of the unitary origin of such nevus cells.

An electron microscopic study of sebaceous epithelioma. A case report with two new observations on lipid droplet formation.

A lesion presenting a typical histopathological picture of sebaceous epithelioma comprised three kinds of cells; immature, transitional, and mature cells corresponding to the components of the normal sebaceous gland. Melanocytes and melanosomes were absent. These findings clearly distinguished the tumor from basal cell epithelioma, and it was concluded that sebaceous epithelioma represents a primary neoplasm of the sebaceous gland. New observations on lipid droplet formation are made with regard to the glycogen-containing cytolysome - tentatively designated a Liposome - and membrane structures around the lipid droplet found in mature cells.

HLA antigens and susceptibility to atopic dermatitis.

The possible association between peculiar HLA antigens ant atopic dermatitis associated allergic rhinitis and/or bronchial asthma was investigated. (1) Atopic dermatitis (no deviation in HLA), complicating allergic rhinitis (HLA-BW40), and bronchial asthma (HLA-B12) are associated by different HLA antigens. (2) The diseases are associated through the same antigen (HLA-BW15), if the concept of disease resistant antigen is accepted. (3) No association is found between HLA antigens and serum IgE levels in patients.

Syringocystadenoma papilliferum: light and electron microscopic studies.

Light microscopic and electron microscopic studies of a lesion displaying a typical histopathological picture of syringocystadenoma papilliferum (S.P.) are reported. Light microscopic serial sections showed that the tumor parenchyma extended to the epithelium just above the intrafollicular sebaceous duct. The tumor thus presumably develops from the epithelial region corresponding to the intrafollicular duct of the apocrine sweat gland. Electron microscopy revealed a non-keratinized intracytoplasmic cavity and intercellular canaliculi apparently formed as a continuation of this cavity. Keratinized cells were absent from the glandular and duct epithelium and even from the superficial epithelial portion of acanthotic areas. Neither myoepithelial cells nor secretory granules were identified in areas showing tubular, glandular, or sinusoidal structures. It was concluded that the tumor differentiates towards both the intrafollicular and intradermal duct of the embryonic apocrine sweat gland apparatus.

Electron microscopic study of nevic corpuscle.

An intradermal nevus, in which a number of nevic corpuscles were clearly observed, was studied, using the electron microscope. Nevic corpuscles contained laminated cells consisting of flattened cytoplasmic processes stretching across the corpuscles to form a complicated labyrinth. The perikaryon of these cells contained premelonosome-like dense bodies and other organelles including mitochondria, rough-surfaced endoplasmic reticulum, free ribosomes, and Golgi apparatus. Neither axons nor dendrites were found in these areas. From these observations, it was concluded that the nevic corpuscles were composed exclusively of nevus cells and could be clearly distinguished from Meissner corpuscles. This view would support the idea of a unitary origin of nevus cells. In addition, an isolated cilium found in a laminated cell is briefly discribed.

HLA antigens in atopic diseases.

The immunogenetical relationship among atopic diseases was investigated from the point of human major histocompatibility antigen. The relationship between IgE level and HLA antigen was also discussed. (1) Atopic dermatitis had no high frequency in HLA antigens. HLA-B12 in bronchial asthma and HLA-BW40 in allergic rhinitis were frequently found in the population study. (2) No HLA-BW15 was observed in these three diseases. The term called 'disease-resistant antigen' was proposed in this paper. (3) We considered that IgE level is not controled by HLA antigen.
