Quantitative assessment of the spatial distribution of low attenuation areas on X-ray CT using texture analysis in patients with chronic pulmonary emphysema.

An automated and quantitative assessment of the spatial distribution of low attenuation areas (LAA) on X-ray CT was performed using texture analysis in chronic pulmonary emphysema (CPE) patients. LAA was defined as those areas having a density less than the mean minus one SD of the control (-960 HU). The probability of change from non-LAA to LAA between a given pair of pixels with horizontal intervals of i pixels (RNi) was evaluated, because this reflects the interaction between LAA and non-LAA regions with different resolutions. The relationship between the percentage area of the LAA over the total area of the entire lung field (LAA%) was subsequently estimated. The RNi increased sharply as the i value increased from 1 to 5, but then almost became a plateau for i values larger than 5. This suggests that the fundamental structures in the LAA areas ranged from 1 x 1 to 5 x 5 pixels in size. RN1-LAA% and RN5-LAA% plots produced curves which were convex, with peak values at approximately 50 LAA% of 0.09 and 0.18, respectively. In the RN5/RN1-LAA% plot, the RN5/RN1 ratio remained constant at 2.0 regardless of the LAA%. A random process simulation was performed to determine the patterns of LAA proliferation if the spatial distribution of the LAA units was random. When the unit size was kept constant, the results of the simulation did not fit the empirical relationship between the LAA% and the three parameters (RN1, RN5 and RN5/RN1). The simulation provided the best-fitting curves when the unit size of the LAA increased in proportion with the LAA%, starting from a 1 x 1 pixel size increasing at a ratio 1 x 1/(5 LAA%). This suggested that the LAA units do not proliferate randomly in spatial orientation at a fixed unit size, but rather spread throughout the whole lung field in a congregated form whilst increasing their unit size. Thus, it may be concluded that healthy lung tissues near emphysematous lesions have a high probability of suffering from emphysema in the future. This may be due to a direct effect of the neighboring emphysematous lesion or due to a pathologic change in the larger bronchii which dominate both the healthy tissues and the emphysematous lesions.

[Optic nerve head and choroidal circulation measured by laser Doppler flowmetry in response to intravenous administration of noradrenaline].

We investigated the change of optic nerve head blood flow (ONHBF) and choroidal blood flow (CBF) in response to noradrenaline (NA) intravenously administered in anesthetized cats. NA (0.03, 0.1, 0.3 micrograms/kg/min) was injected 5 minutes continuously via the femoral vein. The relative blood flow and systemic blood pressure were continuously measured before and after NA administration with fundus camera-based laser Doppler flowmetry. Systemic blood pressure increased by 6%, 42% and 48% according to the dose. The changes in ONHBF after NA administration were -1.4 +/- 3.5% (mean +/- standard deviation) with 0.03 micrograms/kg/min, 2.4 +/- 6.9% with 0.1 micrograms/kg/min, -2.2 +/- 4.4% with 0.3 micrograms/kg/min, respectively. It did not change significantly despite doses of NA. CBF did not change significantly with 0.03 micrograms/kg/min. CBF increased significantly by 62.7 +/- 65.4% with 0.1 micrograms/kg/min, and 80.4 +/- 73.1% with 0.3 micrograms/kg/min. These results suggest that ONHBF is autoregulated despite the change of perfusion pressure by NA administration. This response is different from that of CBF.

Altered imprinting of the H19 and insulin-like growth factor II genes in testicular tumors.

PURPOSE: H19 and insulin-like growth factor II (IGF2) genes are imprinted in normal human tissues. Recent reports have described loss of imprinting (LOI) for these genes in some malignancies. We therefore examined the allele-specific expression of the H19 and IGF2 genes in testicular tumors. MATERIALS AND METHODS: Restriction fragment length polymorphisms were examined in 17 testicular germ cell tumors and 2 testicular malignant lymphomas. RESULTS: All 4 informative testicular germ cell tumors showed LOI for the H19 gene. Five of 9 informative germ cell tumors showed LOI for the IGF2 gene, while testicular lymphomas did not show biallelic expression of the H19 or IGF2 gene. CONCLUSION: Loss of imprinting for the H19 and IGF2 genes in testicular seminomatous germ cell tumors may be involved in oncogenesis.

[A successful surgical repair of total anomalous pulmonary venous connection associated with Williams syndrome].

The surgical correction was performed successfully in a 14-year-old boy with total anomalous pulmonary venous connection (type Ia) associated with Williams syndrome. The diagnosis was made at the age of 3 and he had been well until the age of 13 when sudden chordal rupture caused severe mitral regurgitation and supraventricular tachyarrhythmia. The operational procedure includes common pulmonary vein and left atrium anastomosis through Gersony-Malm approach, chordal reconstruction of mitral valve with 4-0 Core Tex sutures. His postoperative course has been well without the sign of venous obstruction. As far as we know, this is the first documentation of a case of Williams syndrome associated with total anomalous pulmonary venous connection.

[Renal cell carcinoma in a horseshoe kidney: a case report].

A 69-year-old man presented for further examination for left renal mass. Computed tomography and magnetic resonance imaging revealed a heterogenous mass on the left side of a horseshoe kidney accompanied with renal vein thrombus. Aortography showed a hypervascular mass in the left kidney and aberrant arteries to the isthmus originating from the aorta. A left nephrectomy with the division of the isthmus was performed through an abdominal transperitoneal approach. Histological evaluation revealed grade 2, stage T3b, renal cell carcinoma. The patient was free of tumor, but died of intracerebral hemorrhage 5 months after the operation.

[CPE chemotherapy for tongue carcinoma--clinical effects and side effects].

UNLABELLED: Recent advances in chemotherapy have markedly improved the treatment results for oral cancer. Among many chemotherapeutic regimens, the usefulness of multiple combination chemotherapy with cisplatin as the primary drug has been frequently reported. During the past 6-year period, we have performed combination chemotherapy with cisplatin as the primary drug, peplomycin, and etoposide (CPE chemotherapy) as one of the chemotherapeutic regimens for oral cancer. The subjects were 11 patients (7 males and 4 females) with tongue cancer treated by CPE chemotherapy as neoadjuvant chemotherapy at our department between March, 1990 and April, 1995. RESULTS: PR in 8 (73%), and NC in 3 (27%). No patient showed CR and PD. The side effects observed were reversible findings such as transient myelosuppression, nausea-vomiting, and alopecia. No patient showed severe or persistent suppression of hematopoietic function.

[Effects of amrinone on oxygen demand-supply relationship after cardiopulmonary bypass in patient undergoing coronary bypass surgery].

We evaluated the effects of amrinone on oxygen demand-supply relationship after cardiopulmonary bypass (CPB) in patient undergoing coronary bypass surgery comparing with the patients not receiving amrinone. In amrinone treated patients bolus dose of amrinone 1.5 mg.kg-1 was administrated into the reservoir of CPB and followed by continuous infusion at a rate of 5 micrograms.kg-1.min-1. Hemodynamics and blood gas (arterial and mixed venous) measurements were carried out immediately after CPB, 1 hour after CPB and after the chest closure. After the chest closure, cardiac index and oxygen supply in amrinone treated group were significantly higher and systemic vascular resistance was significantly lower than in non-treated group. Left ventricular stroke work index, rate pressure product and oxygen consumption showed no significant difference between the two groups. These data indicate that the amrinone treated patients preserved better oxygen demand-supply relationship after CPB compared with the non-treated patients undergoing coronary bypass surgery.

[Renal cell carcinoma associated with renal artery aneurysm].

We report the 5th case of renal cell carcinoma associated with renal artery aneurysm in the Japanese literature. A 57-year-old woman was admitted to our hospital for further examination of a right renal mass. Computerized tomography and magnetic resonance imaging revealed a 6 x 5 cm tumor at the upper pole of the right kidney and an aneurysm of the right renal artery. An angiography showed duplicated renal arteries and a saccular aneurysm 2.1 x 2.5 cm occurring at the first bifurcation of one renal artery. A transperitoneal radical nephrectomy was performed.

[Clinical results of transurethral electrovaporization of prostate (TVP)].

Transurethral electrovaporization of prostate (TVP) is a new minimally invasive procedure to treat enlargement of prostate. From April 1996 to the end of October, TVP was carried out in 37 cases (mean age, 71.0) with symptoms of urethral obstruction. Standard transurethral resection equipment was utilized. This included a Stortz 26F resectoscope with a Conmed SABRE 2400 electrical current generator. A Stortz 3 mm Spike Electrode was used as the vaporization electrode. The average setting was 250 W for cutting and 80 to 100 W for coagulation. Efficacy parameters evaluated included International Prostate Symptom Score, peak uroflow and postvoid residual volume. By the end of January 1997, 28 cases could be followed up for more than three months after surgery and evaluated. Postvoid residual volume decreased from 68 to 10 and 17 ml at 1 and 3 months. Peak uroflow increased from 8.5 to 15.1 and 17.9 ml/sec at 1 and 3 months. All 28 cases were categorized as effectively treated cases at 1 and 3 months postoperatively. TVP was found to have several advantages, particularly minimal bleeding and the low incidence of postoperative morbidity. The technique is simple and symptoms improve at an early stage following surgery. We intend to continue longer follow up with larger numbers of patients.

Asialo GM1-positive cells in mouse cochlea.

The presence of ganglio-N-tetraosylceramide (asialo GM1)-positive cells in mouse cochlea was tested for using an immunohistochemical method with an anti-asialo GM1 antibody which reacts with natural killer cells and macrophages in mouse. Only a few asialo GM1-positive cells were present on the surface of the scala tympani of the basal turn near the collecting venules. Transmission electron microscopic study showed the internal structure of these cells to be that of macrophages. These findings suggest that asialo GM1-positive cells function as resident type macrophages against invasion by pathogens of the mouse cochlea.

[Liver metastases from breast cancer 14 years after radical mastectomy; successful treatment with hepatic arterial infusion chemotherapy and systemic endocrine therapy--a case report].

The patient was a 73-year-old woman who was admitted to our hospital for epigastric discomfort and body weight loss. She had undergone radical right mastectomy in March 1982. Ultrasonography and computed tomography revealed multiple tumors in the liver. Three of the tumor markers (CA-125, CA 19-9, and CA 15-3) were positive. US-guided fine needle aspiration biopsy of the liver tumor showed small atypical cells with solid cell nests. Immunohistochemical tests revealed estrogen receptor was positive. We diagnosed the patient as recurrent breast cancer metastatic to liver, 14 years postoperatively. The liver tumors were successfully controlled with the combination of intra-arterial infusion chemotherapy (5-fluorouracil, epirubicin, and mitomycin C) along with systemic endocrine therapy (medroxyprogesterone acetate and fadrozole hydrochloride hydrate).

Role of the sarcolemmal sodium pump in nitroprusside-induced vasodilation of the pulmonary artery.

To elucidate the mechanism of nitrovasodilator-induced pulmonary vasodilation, we examined the role of sodium pump and K+ channels in the relaxant responses of canine pulmonary arterial rings to sodium nitroprusside (SNP) under isometric conditions in vitro. Pretreatment with the sodium pump inhibitor ouabain attenuated the SNP-induced vasodilation of KCI-contracted tissues, so that the maximal relaxation decreased from 90 +/- 7 to 62 +/- 6% (P < 0.01), and the negative logarithm of SNP concentration required to produce a half-maximal effect (pD2) decreased from 5.9 +/- 0.4 to 5.1 +/- 0.4 (P < 0.01). This effect was not altered by mechanical removal of the endothelium. In contrast, pretreatment with K+ channel blockers including iberiotoxin, apamin and glibenclamide did not change the relaxant responses to SNP. Incubation of endothelium-denuded rings with SNP increased ouabain-sensitive 86Rb uptake in a dose-dependent manner, an effect that was inhibited by KT 5823, a cGMP-dependent protein kinase inhibitor. These results suggest that activation of sarcolemmal sodium pump may be involved in the nitrovasodilator-induced cGMP-mediated pulmonary vasodilation, whereas K+ channels may play a less important role in this action.

[A case of locally advanced squamous cell carcinoma in bladder diverticulum successfully treated by bladder-preserving therapy].

A 62-year-old man visited our hospital with asymptomatic gross hematuria. On cystoscopy, two diverticula were identified at the dome of the urinary bladder and one of them was packed with a tumor. Biopsy showed squamous cell carcinoma (SCC), G1, but malignancy was not detected in the rest of the vesical mucosa. We selected bladder-preserving therapy. Pathological diagnosis after partial cystectomy was SCC, G1 > G2, pT3b. Postoperatively, 3 courses of adjuvant chemotherapy with methotrexate, bleomycin and cisplatin were performed. The patient has remained free of recurrence for 4.5 years.

Antineoplastic Combined Chemotherapy Protocols

In vivo testing of a magnetically suspended centrifugal pump designed for long-term use.

The life of currently-available centrifugal pumps is limited to no more than three days. As a magnetically suspended centrifugal pump (MSCP) contains no shaft or seal, it could be expected to have a longer life expectancy. The MSCP was evaluated in a chronic animal model using eight adult sheep. Left ventricular assist with the MSCP was instituted between the left atrium and the descending aorta. The flow rates ranged from 2.5 to 6.0 L/min. The duration of the experiments ranged from 14 to 60 days. No mechanical failure occurred. The plasma free hemoglobin levels remained within an acceptable range (3-19 mg/dL). No reduction in the counts of red blood cells or platelets was observed. Thrombus formation within the MSCP was recognized in one pump. The main reason for termination was thromboembolism derived from the circuits. Three types of regulation methods (constant rotational speed, constant motor current, and controlled motor current) were also investigated. Regulation by a constant motor current mode altered the pressure-flow (P-Q) characteristics, and thereby, a steadier pump flow was obtained compared with regulation in the constant rotational speed mode. Moreover, the controlled motor current mode can change the P-Q relationship. These results demonstrate that the MSCP is a promising device for long-term use.

[A case of a successfully treated subclavian aneurysm complicated by a brachiocephalic aneurysm, and the left vertebral artery occlusion].

The patient was a 67-year-old male. Preoperative DSA study revealed a right subclavian aneurysm, a brachiocephalic aneurysm, stenosis of the left common carotid artery and the left subclavian artery, and occlusion of the left vertebral artery. An intraoperative occlusion test of the right vertebral artery showed a decline of the brainstem auditory evoked responses (BAER) seven minutes after the start of the procedure. Cardiopulmonary bypass was established by femoral arterial and bicaval cannulation. Under selective cerebral perfusion by cannulation of both common caroid arteries, a partial aortic arch replacement was performed using a graft patch with 3 branches. These three branches were connected to the right subclavian artery and to both common carotid arteries, respectively. Under deep hypothermia the right vertebral artery was reconstructed with the saphenous vein graft. Proximal anastomosis between the saphenous vein graft and the right caroid graft was done perfusion from a branch of the saphenous vein graft in order to shorten ischemic time of basilar arterial region. The patient was weaned from bypass without problems. After weaning the BAER test was again performed and found to be normal. The postoperative course was uneventful. Postoperative DSA examination showed good blood flow in all the reconstructed vessels. This study suggests that deep hypothermia and shortening of ischemic time of the basilar arterial region are effective in preventing ischemic injury of the brain stem and the cerebellum.

[A case report of one-stage repair for transposition of the great arteries with intact ventricular septum and coarctation of the aorta in neonate].

We report a neonatal case of one-stage repair for transposition of the great arteries with intact ventricular septum (TGA with IVS) and coarctation of the aorta (CoA). The surgery was performed at 27 days of age when the patient, a male, weighed 3408 g. Preoperative cardiac catheterization and angiography confirmed the diagnosis of TGA with IVS, CoA, atrial septal defect, patent ductus arteriosus, persistent left superior vena cava, left sided juxta-positioning of appendages and dextrocardia. The procedure was simultaneous subclavian flap aortoplasty through a left thoracotomy and an arterial switch operation through a median sternotomy. The patient's postoperative course was uneventful and he has grown well. Postoperative cardiac catheterization revealed mild pulmonary stenosis (pressure gradient of 19 mmHg between the right ventricle and the pulmonary artery) and mild aortic arch kinking from the arterial switch maneuver. One-stage repair, the combination of subclavian flap aortoplasty and arterial switch operation, provides a good clinical result in this complex malformation.

Fas/APO-1-mediated apoptosis of human renal cell carcinoma.

We have investigated the Fas-mediated apoptosis of cultured human renal cell carcinoma OUR-10. OUR-10 cells express Fas antigen on their cell surface and showed apoptotic cell death by stimulation with anti-Fas agonistic antibody. Cell proliferation of OUR-10 was reduced to 62% by the addition of anti-Fas antibody. The cytotoxic effect of anti-Fas antibody on OUR-10 was augmented by the pretreatment of these cells with interferon-gamma, not with interferon-alpha or with interleukin-2. Flow cytometry demonstrated that preculture of Our-10 cells with interferon-gamma increased the cell surface Fas antigen. Immunohistochemical analysis of resected tissues from the patients with renal cancer revealed stronger expression of Fas antigen on the surface of cancer cells than that of normal cells. These data suggest that Fas may be one of the possible targets for a novel approach to human kidney cancer, and the efficiency of this Fas-mediated treatment for kidney cancer could be augmented by the treatment with interferon-gamma.
