[Evaluation of urgent treatment for impacted bile duct stones].

One hundred and forty-four urgent treatments for acute obstructive suppurative cholangitis (AOSC), acute obstructive cholangitis (AOC), or/and acute pancreatitis caused by impacted bile duct stones were performed for eight years from 1984 to 1991. The breakdown of these treatments are as follows. [table: see text] It was not easy to diagnose every severe case of AOSC. As a result, however, endoscopy was very effective both in diagnosis and treatment. Concerning patients with thinner bile duct, endoscopic drainage was useful than percutaneous drainage. We prefer ENBD to EST followed by basket extraction of bile duct stones in an emergency state. ENBD is a rather easy technique even for beginners of ERCP, and is less invasive. The life-saving effect of ENBD is not inferior to that of EST.

[Halothane or enflurane treatment in life-threatening asthma].

We performed inhalation anesthetic therapy in an attempt to produce improvement in cause of life-threatening asthma, which were standard pharmacological therapy. We analysed the results obtained in 6 cases given inhalation anesthetic therapy (4 cases were treated with halothane and 2 cases with enflurane). The following observations were made: 1) The criteria for starting inhalation anesthetic therapy were persistent hypoxycemia or hypercapnia, persistently high inspiratory intra-airway pressure, clinical exhaustion and bronchial toilet with bronchofiberscope. 2) We treated the patients with halothane concentrations of between 1.0 and 2.0% and enflurane concentrations of between 1.0 to 4.2%. 3) No major complications were observed in inhalation anesthetic therapy.

[A case of left main bronchial rupture due to bronchocath tube].

A 68-year-old woman underwent a right upper lobectomy for lung cancer. After resection, we noticed the left main bronchial rupture due to bronchocath tube (polyvinyl chloride double lumen tube). The lesion of the rupture was repaired by interrupted sutures with 4-0 prolene. Subsequent course of the patient was uneventful. Tracheobronchial rupture is rare complication of intubation with polyvinyl chloride double lumen tube. There are 6 cases of this complication last five years in Japan. The cause and prevention of this complications are described. It is important to use an adequate tube size, to prevent malposition of the tube and overinflation of the bronchial balloon.

[A case of localized pleural mesothelioma].

A case of localized pleural mesothelioma is reported. A 49-year-old man was operated for coin lesion detected on routine chest X-ray. A tumor arose from the visceral pleura of right lower lobe and was pedunculated. Size of the tumor was 4 x 3 x 3 cm. Cut surface of the tumor was solid and yellowish white. Pathological diagnosis was fibrous type of the pleural mesothelioma. The patient is well and has no sign of recurrence for three years. There are 123 cases of localized pleural mesothelioma including our case in Japan. Five in 78 cases which was diagnosed to be benign histologically at operation, had recurrence later. It is important to follow up strictly for long term.

Laryngostrobovideography using a flexible laryngofiberscope performed in conjunction with the phonatory examination.

The phonatory examination was performed while monitoring vocal fold vibration by laryngostrobovideography. Vocal fold vibration was video-taped by a laryngostroboscope and flexible laryngofiberscope inserted through the nasal cavity. Simultaneously, the phonatory examination was conducted with a phonation analyzer. The data were entered into a personal microcomputer via an A/D converter and analyzed to obtain the parameters of sound pitch, sound intensity and mean expiratory air flow volume, which were superimposed on the color video monitor screen.

Hyogo ear bank experience with allograft tympanoplasty--review of tympanoplasties on 68 ears.

Allograft tympanoplasties on 68 ears done between October, 1980 and December, 1986 were analyzed from the viewpoint of graft take rate and hearing improvement, and also the results were compared between draining and nondraining ears as well as among three stabilizing methods of the transplanted tympanic membrane (Type I: allograft tympanic membrane (ATM) covered with reflected epithelium; Type II: entirely covered ATM with fascia; and Type III: ATM reinforced with thin crescent fascia) and four groups of allograft materials (Group I: ATM with attached malleus and incus en bloc; Group II: ATM with attached malleus; Group III: ATM attached malleus and incus columella; and Group IV: two stage surgery with Group III material). Among the three types of stabilization in draining ears, the anatomic success rate of the tympanic membrane transplant was 27% in Type I, 56% in Type II and 90% in Type III, while in nondraining ears it was 81%, 88%, and 100%, respectively. The success rate for hearing improvement in draining ears was 40% in Group A, 44% in Group B, 33% in Group C, and in nondraining ears, it was 82%, 100%, and 82%, respectively. Our results show that allograft tympanoplasty can be successful for a well-controlled middle ear with good utility of the areolar fascia in stabilization.

Study of temperature factors in cubic crystals by high-voltage electron diffraction.

The critical voltages for systematic reflections and splits of Kikuchi lines were measured using a high-voltage electron microscope to investigate the atomic temperature factors in cubic crystals. The split of the Kikuchi line at the intersection with the forbidden 222 Kikuchi line as well as the critical voltage of the 333 reflection for Si and Ge decreased steeply with temperature. The temperature dependence showed that the anharmonic contribution to the atomic-temperature factor for Si and Ge is extremely weak in the temperature range 300 approximately 1078 K. On the contrary, the B factors obtained from the measured critical voltages for Al, Cu, and Fe varied nonlinearly with temperature, suggesting the importance of the anharmonic effect in the vibration of atoms. The observed temperature dependence of the critical voltages for the metals were compared with calculations based on harmonic, quasi-harmonic, and anharmonic approximations. The quasi-harmonic approximation that takes into account the thermal expansion modification reproduces well the observed values for Fe but not those for Al and Cu. The effect of intrinsic anharmonic vibration should be considered for reproducing the results for Al and Cu. Fitting the measured critical voltages with the calculated ones, we estimated the values for coefficients of the isolated atom potentials. The results are in good agreement with those obtained by neutron and X-ray diffraction.

[Clinical studies on steroid-dependent intractable asthma. Comparison between early and late onset of asthma].

The various components making for severe intractable asthma were clinically and allergo-immunologically studied in 90 patients with bronchial asthma, by comparison between early onset and late onset groups. 1. In the early onset asthma group, cases with low serum IgE levels showed a stronger tendency toward severe intractable asthma. 2. Late onset asthma cases with negative skin tests and negative specific IgE antibodies to house dust tended more often to be severe intractable cases. 3. There was no correlation between sensitization by specific antigens (house dust and Candida), especially Candida, and a tendency toward severe intractable asthma. 4. Severe intractable asthma might be caused by bronchospasm in cases under 30 years of age, by bronchospasm plus hypersecretion in cases between 31 and 40 years of age, and by bronchospasm plus bronchiolar obstruction in cases over 40 years of age.

Small intestine transplantation: a logical solution for short bowel syndrome?

The purpose of this study is to determine whether small intestine transplantation could be considered as an alternative treatment in infants and children with short bowel syndrome. The potential nutritional consequence of orthotopic small intestine transplantation was evaluated in a rat model. Young Lewis strain rats (weighing 250 to 275 g) were used. Lewis rats with resection of 90% of the small intestine were studied as short bowel group (group I, n = 5). In the transplant group rats, 90% of the original small intestine was transplanted orthotopically using microvascular techniques (group II, n = 5). During the study period of 8 weeks, group II gained weight at rates equal to that of normal age matched rats (+30% of the preoperative weight), whereas rats with short bowel (group I) lost 10% of their weight. Two weeks following transplantation, serum albumin levels were maintained in the normal range in group II. However, group I rats showed decreased albumin levels. Serum cholesterol levels showed no significant difference between the two groups. Maltose absorption was evaluated as a functional test of small intestinal graft absorption (1.0 mg/g body weight of maltose was orally administered, and serum glucose levels were measured). The glucose level at 45 minutes was significantly blunted in group I in comparison with group II. The data from this study suggested that small intestine transplantation can produce adequate nutritional support to sustain growth and development in this rat model. It would be anticipated that small intestine transplantation in patients with short bowel syndrome would also benefit nutritionally.

Isolation and characterization of rotavirus from feral pigeon in mammalian cell cultures.

Avian rotaviruses were isolated from feral pigeon faeces treated with trypsin using roller tube cultures of mammalian cells. Two pigeon strains, designated as strains PO-8 and PO-13, produced a marked cytopathic effect (CPE), small intracytoplasmic inclusion bodies and high titres of infectious particles in infected MA-104 and MDBK cell lines without cell adaptation and roller drum apparatus. The pigeon rotaviruses shared a common group specific antigen with the Lincoln strain of bovine rotavirus by indirect immunofluorescence, but differed from both the Lincoln strain and the Wa strain of human rotavirus in neutralization tests. The RNA segment profile of this virus on polyacrylamide gel electrophoresis differed from that of group A mammalian rotaviruses. The results of a serological survey suggested that antibody to pigeon rotaviruses was widespread in avian species in Japan.

Eosinophilic leucocytes and arylsulfatase activity in bronchoalveolar lavage fluid of patients with bronchial asthma.

The arylsulfatase activity and histamine concentration of bronchoalveolar lavage fluid (BALF) were examined in patients with bronchial asthma in relation to the eosinophil count and asthma type (atopic and non-atopic). The BALF arylsulfatase activity and histamine concentration were significantly higher in atopic asthmatics than in non-atopic asthmatics. In atopic asthmatics, the activity of arylsulfatase was significantly increased in patients with a higher eosinophil count (10% or more). However, the BALF histamine concentration did not correlate with the eosinophil count. In non-atopic asthmatics, there was no significant correlation between arylsulfatase activity and the eosinophil count. The results show that arylsulfatase participates in IgE-mediated allergic reactions.

[The Su-polysaccharide skin test in lung cancer].

Skin testing with intradermally injected Su-Polysaccharide (extracted from Su strain Streptococcus bacteria) was performed in 41 cases of lung cancer. Su-polysaccharide skin test results were correlated, to some extent, with the patient's age, clinical stage and performance status and showed a similar trend to the simultaneously performed PPD skin test. These results suggested the potential usefulness of Su-Polysaccharide skin test results as one of the parameters of immunological status of patients with lung cancer. It was also demonstrated that skin reaction to Su-Polysaccharide was increased specifically after OK-432 immunotherapy and was well correlated with the prognosis of the disease. The Su-Polysaccharide skin test was thus considered to be a useful parameter for monitoring the immune response to OK-432 immunotherapy in lung cancer and also one of the parameters of prognostic value.
