[Adriamycin, cardiotoxicity--a case report].

In 1976, a 61-year-old female was diagnosed as malignant cystosarcoma phyllodes, and she was admitted to the hospital for chemotherapy. She was started receiving the combination chemotherapy consisting of adriamycin. 60 mg/3-week, mitomycin C, 1 mg/week, 5-Fluorouracil, 175 mg/week, and citosine arabinoside, 10 mg/week. The initial ECG revealed incomplete right bundle branch block pattern. After the first 60 mg administration of adriamycin, the ECG showed non-specific ST-T changes in addition to I.R.B.B.B. pattern. At the second 60 mg administration of adriamycin, the ECG returned to normal. After the administration of adriamycin, the ECG showed the same ST-T changes again which were seen at the first adriamycin treatment. The same phenomenon was observed at the third adriamycin treatment. In this case, there was a reproducible coincidence between the administration of adriamycin and the occurrence of non-specific changes on ECG; the fore a possibility of adriamycin cardiotoxicity was strongly suggested.

ECG and VCG changes in experimental hemiblock and bifascicular block.

ECG and VCG changes in QRS configuration before and after left anterior subdivision block (LASB), isolated and combined with right bundle branch block (LASB combined with RBBB), were studied in canine hearts. In another series left posterior subdivision block (LPSB), isolated and combined with right bundle branch block (LPSB combined with RBBB), were also performed. With LASB, the A QRS axis deviated superiorly to the left and the QRS duration showed no significant prolongation. Additional RBBB to the existing LASB, moreover, rotated the A QRS axis counterclockwise, and prolonged the QRS duration significantly. With LPSB, the axis was deviated inferiorly to the right and the QRS duration showed no significant prolongation. Additional RBBB to the existing LPSB rotated the A QRS axis clockwise. The QRS duration showed significant prolongation. Vectorcardiographically, the initial part of the QRS loop did not show any essential changes with either LASB or LPSB. The main and terminal parts of the QRS loop, however, were deviated superiorly and to the left with LASB and inferiorly and to the right with LPSB. No changes were observed in the direction of rotation of QRS loop with either subdivision block. From these results it appears that the changes in the terminal forces are more of importance than the initial forces in the diagnosis of hemiblock.

Experimental studies on the antiarrhythmic action of a lidocaine analog.

The electrophysiologic properties of a lidocaine analog (Kö 1173) was experimentally studied in 41 mongrel dogs. The threshold for occurrence of repetitive ventricular extrasystoles was significantly raised by the administration of 2, 4, and 8 mg/kg of the drug, while atrioventricular and intraventricular conduction times were not affected. After ligation of the anterior descending branch of the left coronary artery, the threshold for occurrence of repetitive ventricular extrasystoles was significantly lowered before the administration of Kö 1173 but it returned to control values after the administration 1 mg/kg of the drug. The strength-interval curve shifted profressively to the right when the dose was increased from 2 to 4 and 8 mg/kg. These results indicate that Kö 1173 prevents the decrease in threshold for occurrence of repetitive ventricular extrasystoles in acute coronary insufficiency at a dose which does not affect the conduction system.

Oxygen uptake and its relation to physical activity and other coronary risk factors in asymptomatic middle-aged Japanese.

PURPOSE: Low physical activity is considered to be an important risk factor for atherosclerotic coronary artery disease. However, few data are reported on the Japanese general population. The authors have studied whether oxygen uptake in physical fitness evaluation is a quantitative index for physical activity and whether or not it has a relation to coronary risk factors. METHODS: Five hundred thirteen asymptomatic Japanese (40-64 years of age, 282 males and 231 females) were tested on a cycle ergometer for measurement of peak oxygen uptake (peak VO2) and oxygen uptake at anaerobic threshold (VO2AT). Physical activity was estimated by pedometer score. Data for oxygen uptake were adjusted by age or by age and body mass index (BMI), then its relationship to the following risk factors was investigated: physical activity, BMI, blood pressure, total cholesterol, HDL and LDL cholesterol, fasting blood glucose, and triglycerides. RESULTS: Subjects in the highest peak VO2 quartile walked significantly more than those in the lowest quartile in both males and females. Those in the highest quartile showed lower BMI, lower blood pressure, lower triglyceride, and higher HDL cholesterol. The same relationship was observed for VO2AT. CONCLUSIONS: Higher fitness level determined by peak VO2 or VO2AT is related to higher physical activity and lower coronary risk factors in the asymptomatic middle-aged Japanese. These data provide support for exercise prescription in the primary prevention of coronary heart disease in Japan.
