[Meconium ileus equivalent].

Meconium ileus equivalent (MIE) can be defined as a clinical manifestation in cystic fibrosis (CF) patients caused by acute intestinal obstruction by putty-like faecal material in the cecum or terminal ileum. A broader definition includes a more chronic condition in CF patients with abdominal pain and a coecal mass which may eventually pass spontaneously. The condition occurs only in CF patients with exocrine pancreatic insufficiency (EPI). It has not been seen in other CF patients nor in non-CF patients with EPI. The frequency of these symptoms has been reported as 2.4%-25%. Pathophysiologically, MIE is probably caused by a combination of EPI, increased intestinal transit time, and abnormal intestinal mucus. The treatment should primarily be non-operative. Specific treatment with N-acetylcysteine, administrated orally and/or as an enema is recommended. Enemas with the water soluble contrast medium, meglucamine diatrizoate (Gastrografin), provide an alternative form for treatment and can also serve diagnostic purposes. It is important that the physician is familiar with this disease entity and the appropriate treatment with the above mentioned drugs. Non-operative treatment is often effective, and dangerous complications following surgery can thus be avoided.

Nasal calcitonin for treatment of established osteoporosis.

Thirty-seven women with established osteoporosis completed a one-year double-blind, placebo-controlled study with the primary aim of examining the effect of nasal salmon calcitonin (200 IU daily) on bone and calcium metabolism. All the women received a daily calcium supplement of 500 mg. For comparison we also report data from an age-matched group of healthy women who did not receive calcium supplementation. The bone mineral measured in the forearm (single photon absorptiometry) and spine (dual photon absorptiometry) showed a similar pattern during treatment. The calcitonin group (n = 17) did not lose bone mineral in comparison with the placebo (n = 20) and the control groups (n = 19) (P less than 0.01). The biochemical estimates of both bone resorption and bone formation decreased highly significantly in the calcitonin group (P less than 0.001) and were unchanged in the control group, whereas the placebo (calcium) group showed intermediate values. Neither subjective nor objective side-effects occurred in any of the groups. We conclude that nasal calcitonin is a realistic treatment of established osteoporosis.

Lack of seasonal variation in bone mass and biochemical estimates of bone turnover.

Three previous studies have indicated a seasonal variation in bone mineral content, with values during the summer being 1.7% to 7.5% higher than during the winter. We have examined the seasonal influence on both bone mass, biochemical estimates of bone turnover and vitamin D metabolites in 86 healthy women, aged 29-53 years. All participants were followed up for 2 years with examinations every 6 weeks or 3 months. Bone mineral content in the proximal and distal part of the forearm (single photon absorptiometry) did not reveal any significant seasonal variation, whereas bone mineral density of the lumbar spine (dual photon absorptiometry) indicated that the highest values occurred in winter. None of the biochemical parameters showed any statistically significant cyclical changes. Serum concentrations of 25-hydroxyvitamin D and 24,25-dihydroxyvitamin D3 showed a highly significant seasonal variation, whereas the serum 1,25-dihydroxyvitamin D concentration was virtually unchanged. We conclude that seasonal variation in bone mineral content and bone turnover should not be taken into account when interpreting data from longitudinal studies of healthy pre- and postmenopausal women on a sufficient vitamin D nutriture.

The oral contraceptive user.

The use of oral contraceptives (OC) has been correlated to several diseases, primarily thromboembolic conditions. Whether these correlations represent a causal relationship or a mere statistical correlation based on other variables, depends on whether users of OC differ in other respects from women in fertile age not using OC. We interviewed 519 Danish women, 15-45 years old and selected at random, concerning present and past use of OC, age, occupation, years of schooling, marital status, income, smoking habits, urbanization, and their opinion on the postulated thrombotic risk among users of OC. The data were analysed by methods for multivariate contingency tables. Opinions on the thrombotic risk, age, smoking habits and, to a lesser degree, regarding income, were directly correlated to the oral contraceptive use. There was no direct interaction between the use of OC and the other variables. Users of OC were younger and smoked more than the non-users and were of the opinion that the use of OC implied only minor thrombotic risk, or none at all.

ATP-induced release of vasopressin from isolated bovine neurohypophyseal secretory granules. Dependency on chloride and effects of analogues of ATP.

Secretory granules isolated from bovine neurohypophyses released vasopressin in the presence of a buffered medium containing ATP, Mg2+ and KCl. Substitution of K+ in the medium with Na+ or choline did not affect the release. Substitution of Cl- with either sucrose, sulphate or acetate strongly reduced the release. Analogues of ATP, substituted at the beta-gamma anhydride bond with methylene or imido groups caused a smaller release which was not related to a very small breakdown of analogues that occurred. It is suggested that at least part of the ATP induced release is due to a physicochemical action.

Hyperthermic tumour-cell devitalization in vivo.

A review of the morphologic, biochemical and clinical effects of hyperthermia on malignant cells indicates the presence of two principally different heat-induced alterations. (1) A 'destructive' lysosomal dependent cytoplasmic reaction dominates the tumour-cell devitalization in vivo, probably influenced by the characteristic tumour cell environment. (2) 'Repressive' nuclear abnormalities may be observed, but seem to be secondary in the in vivo reaction. However, under certain conditions (combined treatment modalities) this nuclear effect may be of importance.

Histologic and histochemical reactions in a mouse mammary carcinoma following exposure to combined heat-roentgen irradiation.

In mouse experiments a high curative effect on implanted tumours was obtained by successive application of low local heat and roentgen doses. Histologic and histochemical examinations of tumours so treated revealed a special reaction in the tissues. A total and rapid destruction of all hypoxic tumour tissue was observed and (in successful cases) a total blockade of all mitotic activity in euoxic tumour tissue was followed by a long-drawn necrobiotic decay of the existent tumour cells. The disturbances are electively bound to the tumour cells, only some normal tissues may secondarily be injured. A rapid and intensive lysosomal activity in all the tumour cells may probably be of importance in the reaction.

Radiation sensitizing effect of heat.

The radiation sensitizing effect of heat is analysed. In an isologous mouse-tumour system a successive local exposition to heat and roentgen irradiation with doses of both components of a size expected separately to give no or a few cures only an evident rise in the curative results was obtained. The presence of a synergism of the factors applied is suggested. The effect obtained depends on the dose of both factors. It is independent of the sequence of the application of the two components and is not reduced by an interval of 24 h between the exposures.
