Congenital mesenteric arterio-portal fistula: report of a case.

A male patient with an arterio-portal fistula resulting from a mesenteric arteriovenous malformation, who developed portal hypertension and liver cirrhosis, is presented herein. The malformation was considered to be congenital in origin and its location made any ablative surgical procedure impossible. Such alternative treatments as ligation of the afferent arteries, followed by transarterial embolization were therefore given, but both were unsuccessful. We also present a review of the literatures of mesenteric arteriovenous fistula. Radical surgical approach for this rare entity is proposed. The case reported here as related to mesenteric arteriovenous communications of congenital origin is the seventh such case published, and the first which was ever found to be located in the trunk of the superior mesenteric artery.

[Clinical studies on T-1982 (cefbuperazone) in the field of obstetrics and gynecology].

Clinical studies on T-1982 (cefbuperazone) were performed in 20 patients of obstetrics and gynecology, and the following results were obtained. Clinical effect: The effectiveness was 100% (excellent in 5 cases, good in 4 cases) in intrauterine infection, 75% in adnexitis (excellent in 1 case, good in 2 cases, poor in 1 case), 100% in pelveoperitonitis (excellent in 2 cases, good in 2 cases), 0% in parametritis (poor in 1 case) and 100% in Bartholin's abscess (good in 2 cases), with the overall effectiveness rate of 90% (excellent in 8 cases, good in 10 cases, poor in 2 cases). Bacteriological effect: A total of 17 strains was isolated from 12 out of 20 cases. Fourteen out of 17 strains were aerobic organisms (7 of E. coli, 4 of Klebsiella, 2 of S. faecalis and 1 of S. epidermidis) and 3 out of 17 strains were anaerobes (each 1 strain of B. fragilis, P. prevotii and P. anaerobius) and all of them were eliminated. No side effects due to this drug were noted.

Etiology of QT prolongation and T wave changes in chronic alcoholism.

Etiology of QT prolongation and T wave high voltage was studied in 90 chronic alcoholics in relation to history of alcoholism, blood chemical values, heart rate and QRS voltage with a technic of multiple regression analysis. Incidences of QT prolongation (22%), T wave high voltage in lead V2 (9%), hypopotassemia (23%), hypocalcemia (26%) and hypomagnesemia (28%) were high, despite these examinations were done after relatively long abstention period (35 days on average). Sinus tachycardia (19%) and QRS high voltage (SV1 + RV5 exceeding 4 mV, 41%) were also frequent. Unexpectedly, QT interval did not correlate to serum electrolytes, including calcium. Major factors associated with QT prolongation were sinus tachycardia, longer abstention period and larger amount of daily alcohol consumption. Although the reason of each association was not quite clear, alcoholic myocardial damage may be a cause of QT prolongation. Voltage of T wave in lead V2 was sensitive to serum potassium level, but the observed tendency of hypopotassemia acted to the direction against high voltage of T wave. The sole factor positively associated with high voltage of TV2 was high QRS voltage, which may be a manifestation of left ventricular hypertrophy.

Long-term clinical effect of calcium inhibitors in hypertrophic cardiomyopathy compared to the effect of beta-blocking agents. A preliminary report with special reference to the beneficial effect of nifedipine on angina pectoris.

Long-term clinical effects of beta-blockers (propranolol in most cases) and calcium inhibitors (nifedipine in most cases) were studied in 16 patients with hypertrophic cardiomyopathy. On overall subjective symptoms, beta-blockers were effective in 50% of symptomatic patients, while calcium inhibitors were effective in only 33%. On angina pectoris, however, calcium inhibitors were superior to beta-blockers in our patients. Blood pressure decreased with each drug, and the decrease was significant with nifedipine. Otherwise there was no change in physical findings with either drug. Long-term (more than 6 months) use of beta-blockers resulted in an increase in cardiothoracic ratio on chest X-ray, a decrease in left ventricular ejection fraction on echocardiogram and more pronounced ST-T change on electrocardiogram. Prolonged use of nifedipine resulted in a slight decrease in cardiothoracic ratio, but no systematic change on echocardiogram and on electrocardiogram.

Intracardiac pressures in pregnant patients with mitral stenosis.

The influences of pregnancy on the pulmonary circulation in mitral stenosis (MS), were studied. 1) Twenty-two cardiac patients underwent serial right heart catheterization with the use of flow-directed catheter without X-ray fluoroscopy during the course of pregnancy. 2)In 23 cases of MS, in whom cardiac catheterization had been performed before pregnancy, clinical courses during the pregnancy were studied. In the 26-32 weeks' gestation: in MS (Group I), an elevation of peak systolic pulmonary arterial pressure (s-PAP) (mean + 14.9 mmHg, P less than 0.001) was seen; in contrast, in atrial septal defect (Group II), s-PAP remained constant. Right atrial pressure rose in the 26-32 weeks, gestation in both the groups. Infusion of 200 ml of dextran in 12-16 weeks, gestation induced an elevation of s-PAP in Group I, especially in those who fell into heart failure in the succeeding courses of pregnancy; but not in Group II. In MS, in 12 cases, whose s-PAP before the pregnancy was below 35 mmHg, no heart failure occurred during the pregnancy; in 6 of 8 cases with s-PAP between 35 and 50 mmHg, mild heart failure occurred; and in 2 cases with s-PAP over 50 mmHg, severe heart failure occurred during the pregnancy. In conclusion, in MS, PAP rises during pregnancy, and S-PAP before pregnancy and the reaction to dextran infusion can give suggestions for allowing pregnancy.

Human cartilage proteoglycans isolated from normally ossifying and congenitally malformed leg bones.

Proteoglycans were extracted with 4 M guanidine HCl solution containing protease inhibitors from various zones of human epiphyseal cartilages of the normally ossifying fibula and cartilaginous rudiment of the tibia of a 12-month-old boy with congenital absence of the tibia, when the knee disarticulation was performed. All the proteoglycan preparations from the epiphyseal cartilages were separated with a sucrose density gradient centrifugation into two components: a heavy, major component and a light one. The molecular size and the proportion of isomeric chondroitin sulfates of polysaccharides of the heavy component differed from those of the light one. The relative amounts of isomeric chondroitin sulfates in the polysacharide moieties of the components also varied among these zones. The glycosaminoglycan content in the rudimentary tibia was equal to that of the epiphyseal cartilage of the fibula. However, proteoglycan preparations showed neither the normal sedimentation profile with two peaks nor the zonal differences as to the proportion of isomeric chondroitin sulfates. These results suggest that the alterations in proteoglycan metabolism might be involved in the pathogenetic mechanisms producing the congenital limb defect.

Variable prognosis in congestive cardiomyopathy. Role of left ventricular function, alcoholism, and pulmonary thrombosis.

Prognosis of 36 patients with congestive cardiomyopathy was studied in relation to various clinical factors. Half life of the survival curve after overt heart failure was about 7 years. Although left ventricular function was a major determinant of clinical course in congestive cardiomyopathy in general, its relation to prognosis was variable according to the type of cardiac involvement. In peripartal cardiomyopathy and in a type of cardiomyopathy named subacute cardiomyopathy with pulmonary thrombosis in this paper, factor(s) other than left ventricular function, possibly including pulmonary thrombosis, may be operative as more important determinant of extremely poor prognosis in these subtypes. Alcoholic cardiomyopathy was also unique in its favorable prognosis in association with reversible cardiomegaly following abstention from alcohol.

Scanning electron microscopy of ciliary zones of the ciliate protozoa in the large intestine of the horse.

The surface structure of the ciliary zone in 13 species of ciliates found in the large intestine of the horse was observed by scanning electron microscopy. In Holophryoides ovalis many fine depressions considered to be a result of phagocytosis or pinocytosis in the naked cytostome were noticed. In Blepharocorys spp. a distinct section was present between the portion with cilia and that without cilia. It was not present, however, in some species of the family Buetschliidae, such as Bundleia postciliata and Didesmis spp. The species of Entodiniomorphida had a lip around the ciliary zone with cilia forming synciliary tufts. In Spirodinium equi and Tetratoxum unifasciculatum the ciliary zone revolved counter-clockwise in an en face view. Some differences in the surface structure of the ciliary zone between the entodiniomorphid and spirotrich ciliates are discussed.

Ultrastructure of cartilage in heritable disorders of connective tissue.

Ultrastruct of cartilage were examined in Marfan syndrome, Menkes kinky hair syndrome, achondroplasia, asphyxiating thoracic dysplasia, mild diastrophic dysplasia and mucopolysaccharidoses I and III. Ruthenium red staining revealed decrease of proteoglycans in cases with Marfan syndrome and kinky hair syndrome, and increase in cases with osteochondrodysplasia and mucopolysaccharidosis III. This morphologic tendency coincided with the result obtained by biochemical analysis of glycosaminoglycan contents in cartilage matrix from cases with Marfan syndrome (decreased content) and asphyxiating thoracic dysplasia (increased content). It was postulated that proteoglycan content in cartilage matrix might be related to excessive or reduced skeletal growth in Marfan syndrome or osteochondrodysplasia.
