Evidence based criteria for the histopathological diagnosis of toxoplasmic lymphadenopathy.

AIMS: To formulate evidence based histopathological criteria for the diagnosis of acquired toxoplasmic lymphadenitis, in an area of high tuberculosis prevalence. METHODS: Multiple histopathological parameters were assessed in a consecutive sample of biopsies from 68 patients presenting with lymphadenopathy with a duration of less than six months. Serum IgM enzyme linked immunosorbent assay was used as the standard reference test for the diagnosis of toxoplasmic lymphadenitis. The sensitivity, specificity, and likelihood ratios of various histological parameters were estimated. RESULTS: The presence of microgranulomas (p < 0.0001), paracortical widening (p = 0.006), paracortical hyperplasia (p = 0.02), monocytoid B cells in sinuses (p = 0.007), lower than grade 2 macrogranuloma (p = 0.002), and the absence of giant cells (p = 0.05) were found to discriminate between IgM positive cases and IgM negative controls. Using a composite criterion-(1) presence of microgranulomas, (2) lower than grade 2 macrogranuloma, (3) absence of giant cells, and (4) follicular hyperplasia-toxoplasmic lymphadenitis can be diagnosed with a high degree of sensitivity (100%), specificity (96.6%), and positive likelihood ratio (29). CONCLUSION: Toxoplasma lymphadenitis can be diagnosed with a high degree of confidence using the specific histopathological criteria identified here.

Eosinophils in acute appendicitis: possible significance.

120 consecutive appendicectomies and 20 appendices from medicolegal autopsies were studied. The cases were grouped as. A: Acute appendicitis. B: Acute presentation, not diagnostic of acute appendicitis C: Elective appendicectomies D: Normal appendices from autopsies. Eosinophils and mast cells were counted in the muscularis, in Giemsa stained sections. The mean eosinophil and mast cell counts per mm2 were--A. 215.9; 26.5. B. 66.0; 32.1. C. 6.7; 25.8. D. 4.2; 19.6 respectively. Eosinophil count is significantly higher in A compared to others (p < 0.0001) and there was no range overlap with C and D. B is a heterogenous group with 37.5% having eosinophil counts in the range seen in A. Cases with mural eosinophil showed histological evidence of mast cell degranulation. Eosinophil infiltration of the muscularis is an early event universally seen in acute appendicitis. It is possible that the disease is triggered by Type I Hypersensitivity, and that infection is a later consequence.

Mullerian adenosarcoma of the uterine cervix.

Two cases of mullerian adenosarcoma with heterologous elements which occurred in uterine cervix and had a gross appearance of sarcoma botyroides are reported. Histologically, the tumours were composed of an admixture of benign appearing glands and a sarcomatous stroma, with heterologous elements consisting of skeletal muscle, bone and cartilage. No malignant epithetial components were observed. Indications are that, the prognosis of these tumours are much better than the usual malignant mixed mullerian tumours.

Value of AgNORS in fine needle aspiration cytology of breast lesions.

Silver staining Nucleolar organizer regions (AgNORs) were studied in fine needle aspirates of 48 benign and 36 malignant lesions and correlations with histological types, menstrual status, tumor size and lymph node involvement were looked for. A semiquantitative scoring system (AgNOR Score) reflecting total AgNOR area was applied to each of these parameters and compared with the standard counting method. The objectives were to examine the pattern of distribution and discriminating capacity of AgNORs in fine needle aspirates of different breast lesions and to evaluate the AgNOR scoring system as an alternative to the standard counting method. Mean AgNOR count was significantly higher in malignant (5.4; 95% CI 5.0-5.9) than benign (2.8; 95% CI 2.7-3.0) lesions. For AgNOR scores the corresponding values were: malignant 11. 2; 95% CI 10.2-12.2 and benign 5.3;95% CI 4.9-5.7. For malignant lesions, the counts and scores tend to be more in ductal carcinomas than lobular, more in premenopausal women, in tumors more than 5 cm in diameter and in cases with more than 3 lymph nodes involved. For all parameters the scoring system showed better discriminating capacity. The differences in AgNOR scores were statistically significant for tumor size and lymph node status. Multiple stepwise regression shown tumor size to be best correlated with AgNORs.

How socioeconomic status affects birth and death rates in rural Kerala, India: results of a health study.

Data relating to birth and death were collected from throughout the state of Kerala, India, in a health survey conducted by the Kerala Sastra Sahitya Parishad, a voluntary organization. In this study, the authors analyze birth and death rates as calculated from the sample of 9,940 households (57,665 persons), with respect to other variables such as region, religion, and socioeconomic status. In order to study the effect of socioeconomic factors on birth and death rates, a socioeconomic status rating (SES rating) was developed, taking into account such factors as income, education, housing conditions, and land ownership. Socioeconomic status was found to have a definite influence on birth and death rates, with higher socioeconomic status resulting in lower birth and death rates. This effect was independent of such confounding variables as age structure of the population, religion, and region. The higher risk of mortality among the poorer households can partly be explained by the material deprivation: the higher birth rates could be the result of poorer educational attainments.

Jordans' anomaly.

A case of Jordans' Anomaly of leucocytes is reported in a young boy with congenital ichthyosis and hepatosplenomegaly. Cytoplasmic vacuoles were seen in granulocytes, monocytes and lymphocytes of the patient and his father. Serum triglyceride was found elevated in the child but not in the father. Ultrasonogram of the patient's liver showed features suggestive of fatty change, thus pointing to a possible abnormality of systemic triglyceride storage.

Sickle cell disease in Wayanad, Kerala: gene frequencies and disease characteristics.

BACKGROUND: A large number of patients with sickle cell anaemia are seen at the Medical College, Calicut from among the tribals and Chetti communities of the adjacent Wayanad district. We carried out a population-based study of gene frequencies and disease characteristics to plan an appropriate intervention. METHODS: Clinical examination and haemoglobin electrophoresis were done in 1016 subjects belonging to the tribal and Chetti communities in Wayanad district, by visiting hamlets and schools and evaluating everyone present at the time of the visit. RESULTS: The gene frequency of haemoglobin S ranged from 0.019 in Kattunayakan to 0.196 in Wayanadan Chettis. Wayanadan Chettis, Kurumas and Adiyas showed a high number of homozygotes with the oldest being 48 years. The survival of homozygotes is longer than what is generally recorded in other states. The disease was mild in 52.2% of cases. Painful crises were found in 43.5% and splenomegaly and leg ulcers in 4.3% each. The mean haemoglobin F rate in homozygotes was 25.9%. It was higher in clinically mild cases and in those showing an absence of irreversible sickle cells in the peripheral smear. CONCLUSIONS: The survival of patients with sickle cell anaemia seems to be higher in Kerala as compared to other states. It appears that even small improvements in primary health care available to the population (as in Kerala) are sufficient to achieve this effect. Integration of disease diagnosis and management into the already existing health care delivery system may lead to even better survival and quality of life.

Testicular choriocarcinoma with gastric metastasis presenting as hematemesis.

We report a 28-year-old man who presented with hematemesis due to choricarcinoma of testis metastatic to the stomach. Gastroscopy showed a polypoidal lesion. Testicular wedge biopsy confirmed mixed germ cell tumor, the choriocarcinomatous portion alone getting metastasized to the stomach. He was initiated on chemotherapy with actinomycin-D, etoposide and methotrexate, but died due to multiple metastases to the lung and brain.
