Adenocarcinoma of the stomach in Fanconi's anemia.

A case of Fanconi's anemia presenting for the first time at the age of 23 years is described. He died of adenocarcinoma of the stomach 4 months later. As far as the authors are aware, this is the second case of adenocarcinoma of the stomach occurring in Fanconi's anemia. As Fanconi's anemia is known to predispose to malignancy, all patients need detailed evaluation of the gastrointestinal tract by endoscopy as well as radiology for the early detection of gastrointestinal lesions.

Hematopoietic growth factors in drug-induced agranulocytosis.

Drug-induced agranulocytosis (DIA) is a potentially fatal disorder. Hematopoietic growth factors have been used in the treatment of DIA. We report nine cases of DIA treated with granulocyte macrophage - colony stimulating factor (GM-CSF) in a dose of 300 microg/day. All the patients had evidence of systemic infection. Mean time to reach an absolute neutrophil count of 0.5 x 10(9)/L was three days. One patient succumbed to the disease. The cause of death was multiorgan failure. No adverse events were observed with GM-CSF. We conclude that hematopoietic growth factors are useful in shortening the period of neutropenia and reducing morbidity and mortality in these patients.

Ankylosing spondylitis associated with non-Hodgkin's lymphoma.

Malignancies have rarely been reported in patients with ankylosing spondylitis either as a complication of therapy or de novo. Here we report a 37 years male with ankylosing spondylitis who developed non-Hodgkin's lymphoma during the course of his illness.

Chronic myeloid leukemia presenting as gout.

A 53-year-old man presented with gouty arthritis. A physical examination and haematological and biochemical tests showed that he had chronic myeloid leukemia. He was treated with allopurinol, hydroxyurea and analgesics. The arthritis subsided completely within 2 weeks. He continues in haematologic remission (on interferon) with no further recurrence of the gout.

Isolated hypoglossal nerve palsy due to skull base metastasis from breast cancer.

We describe a 44-year-old woman who presented with an isolated unilateral hypoglossal nerve paralysis caused by a skull base metastasis from breast cancer. The patient had a modified radical mastectomy followed by local radiotherapy and adjuvant chemotherapy. Fourteen months later she presented with difficulty in speaking. Physical examination revealed an isolated left hypoglossal nerve paralysis. The MRI scan showed a mass lesion involving the left occipital condyle extending into hypoglossal canal.

Late presentation of congenital factor V deficiency--a case report.

Congenital factor deficiency is a rare coagulation disorder, which is inherited in an autosomal recessive manner. The severity of bleeding symptoms in general is only partially related to the degree of factor V deficiency in plasma. In this report, a boy presenting with hemarthrosis in his late adolescence due to congenital factor V deficiency is reported.

Subdural haematoma with spontaneous resolution--rare manifestation of idiopathic thrombocytopenic purpura.

Spontaneous intracranial haemorrhage presenting as subdural haematoma is an extremely rare presentation in adults due to idiopathic thrombocytopenic purpura. There are only five such case reports available in the literature and four had surgical evacuation of haematoma, while only one had spontaneous resolution. We report the case of a middle aged female who presented with bilateral papilloedema and later diagnosed to have subdural haematoma as a complication of idiopathic thrombocytopenic purpura, which had a spontaneous resolution.

Thrombocytosis--report of a case.

Thrombocytosis is a common clinical condition. Drugs used for thrombocytosis are hydroxy urea, alkylating agents and interferon alpha. We are reporting a case of chronic myeloid leukemia with thrombocytosis who did not respond to hydroxy urea but was controlled with the addition of a new drug anagrelide. Anagrelide is a platelet reducing agent with no serious side effects. It may be the main weapon against thrombocytosis associated with chronic myeloid leukaemia (CML) and essential thrombocytosis in the coming years.

Chronic lymphatic leukemia presenting as hemolytic anaemia.

In CLL autoimmune phenomena manifest most commonly as autoimmune hemolytic anemia followed by thrombocytopenic purpura and pure red cell aplasia. Here an elderly lady presenting with AIHA as the presenting manifestation of CLL is described.

Cold haemagglutinin disease in systemic lupus erythematosus.

A 34-year-old lady presenting with features of cold agglutinin disease during the course of systemic lupus erythematosus is described. Cold antibody titer was very high (1 in 4096) with specificity for 'I' antigen. Even though she had poor prognostic factors like high titer of cold antibodies with low thermal amplitude, she responded well to prednisolone.
