[The effect of antibiotic prophylaxis in vaginal obstetric procedures (author's transl)].

202 cases of vaginal obstetric procedures (forceps and vacuum extraction, breech delivery and manual removal) were reviewed with regard to assessing the value of prophylactic administration of antibiotics. Criteria for the evaluation were fever post partum, duration of stay in hospital and involution of the uterus. The results showed that the prophylactic administration of antibiotics had no significant effect on these parameters, with or without the simultaneous administration of oxytocics. Hence, in view of the risk of allergic reactions, toxicity and the selection of resistant strains the prophylactic administration of antibiotics does not seem to be justified in vaginal obstetric procedures. Scrupulous aseptic precautions during all vaginal or intrauterine procedures are considered essential for the prevention of infection.

[Results of a long-term clomiphene citrate therapy in women with normogonadotropic menstrual disturbanes (author's transl)].

A total of 72 pregnancies was achieved in 59 women with menstrual cycle disturbances (WHO Type II) by administration of clomiphene. 27 of these pregnancies (34.7%) occurred only after more than three courses of cloimphene. A secondary classification of the patients was based on the symptomatic severity of the menstrual disturbance. Clomiphene (100 mg) was administered on the fifth day after the onset of menstruation for five days, whereby in one or two cases up to 19 courses had to be given. The group consisted of women who had rejected HMG-HCG therapy because of the possibility of multiple pregnancy. 59 pregnancies went to full term; 2 tubal pregnancies and 11 cases of miscarriage were recorded. The commencement of pregnancy was evaluated and the time of ovulation and the length of the luteal phase determined in women requiring up to three courses of clomiphene and those requiring over three courses. The investigation demonstrates that clomiphene therapy can be successful even after more than six courses of the drug. Side effects are minimal.

The results of isotope renography and intravenous pyelography in 420 patients with carcinoma of the uterine cervix.

A group of women with carcinoma of the cervix was studied by renography and i.v. pyelography. The incidence of renal and urinary tract disorders detected by these tests increased as did the stage of the disease, being 17.1% with Stage I carcinoma of the cervix, 27.8% with Stage II carcinoma of the cervix, 47% with Stage III carcinoma of the cervix, and 83.3% in Stage IV carcinoma of the cervix. In patients with Stage I and Stage II carcinoma of the cervix, those selected for radiotherapy had twice the incidence of abnormal renograms as those selected for surgery. This difference probably reflected a higher incidence of medical disorders affecting renal and urinary tract function in those considered unfit for surgery. We believe that renograms should be used routinely as a screening test in patients with carcinoma of the cervix.

Female

A simple, versatile, sensitive and volume-independent method for quantitative protein determination which is independent of other external influences.

A method is described which in principle is a quantitative "spot analysis". 0.5-5 microliter of a protein solution in the concentration range between 0.01-10 mg/ml is used for one determination. The sample is taken up by capillary attraction in a 0.5-, 1-, 2- or 5-microliters capillary and transferred to a cellulose acetate strip. The protein is fixed and stained simultaneously by dipping the cellulose acetate strip into a solution of Amido Black or a benzoxanthene derivative (Hoechst 2495) dissolved in in methanol/acetic acid. After elution of the excess of dye (3 x 5 min) the quantitative evaluation can be performed in different ways: 1) The sample is fixed and made transpartent by incubation in dioxane/1-butanol and evaluated densitometrically (Amido Black 10B) or 2) the evaluation is performed in situ by spot fluorometry (Hoechst 2495). 3 The sample can be dissolved together with the acetate layer completely in dioxane, dimethylsulfoxide or N,N-dimethylformamide and evaluated photometrically or 4) fluorometrically. 5) Highest sensitivity is reached if the fluorochrome (Hoechst 2495) bound to the protein is eluted with 15% NH4OH and measured fluorometrically. There is a linear correlation with a correlation coefficient of 0.999 between the fluorescence and a protein amount of 10 ng-20 micrograms. In addition to its simplicity, the method has the advantage of being independent of or well-defined against other external influences, e.g., sodium dodecyl sulfate, mercaptoethanol, Triton X-100, etc. The stainability of a protein with Amido Black is influenced stoichiometrically by sodium dodecyl sulfate (not by mercaptoethanol) whilst the staining with Hoechst 2495 is not at all affected. As there is linear correlation between the area of a spot on an acetate layer and the volume applied in the range between 0.5 and 5 microliters, (only influenced stoichiometrically by the protein concentration in that volume, which in turn is measured by staining with Amido Black), then with a simple iterative calculation on the basis of suitable calibration curves, it is easily possible to determine a protein concentration in mg/ml even in an unknown volume between 0.5 and 5 microliters.

Capillary Action

[The influence of TNS (transcutaneous nerve stimulation) on placental flow (author's transl)].

A report about 12. gravid patients with a diminished uteroplacental flow is given. To increase their placental perfusion a transcutaneus lumbo-sacral electric nerve stimulation was performed. In all 12 cases a distinet increase war measured by the method of radioisotope placenta-perfusion. The mean values showed a high significant difference (p less than 0,001). The reasons for this results and the possibilities of this findings for the therapy of funcitonal placental insufficiency are discussed.

[The influence of ritodrine-hcl (prepar), bunitrolol (stresson) and oxytocin (syntocinon) on Uteroplacental perfusion (author's transl)].

The administration of betamimetics has proved to be useful for symptom-oriented treatment in patients with reduced utero-placental flow. In order to quantify the betamimetic effect flow measurements were carried out in a total of 12 sheep, yielding the following experimental results: 1) Betamimetics, up to a certain point which correlates with maternal heart rate, raise the uteroplacental flow-rate. 2) The use of cardio-selective betablocker prevents undesired betamimetic side-effects but there is also a loss of the rise in utero-placental flow rate. 3) In sheep pretreated with Oxytoxin the application of betamimetics not only offsets oxytoxin-action but also greatly improves utero-placental blood flow.

[The Le Veen peritoneo-jugular shunt and the testing of its function].

A radioisotope technique for testing the patency of, peritoneo-venous shunts of Le Veen, is reported and is demonstrated by discussing a case history. By now a total number of 16 patients have been tested, after implantation of such a shunt. The demonstrated radioisotope technique, which causes no complaints from the patient, is not only able to inform concerning the functional state of the Le Veen shunts, but also, in case of disturbances, is able to reveal the cause of the underlying malfunctions.

Ascites

[Prophylactic medication to prevent miscarriage in pregnant women with a history of sterility (author's transl)].

A report is given of the outcome of 451 pregnancies occurring among women who had previously attended the Infertility Clinic. General gestagen therapy was given prophylactically in 129 cases before the appearance of any signs of threatened abortion. Massive gestagen therapy was initiated in 41 cases in response to signs of threatened abortion. Gestagen was not given either prophylactically or therapeutically in the remaining 281 pregnancies. The incidence of abortion was 10.1% in women given prophylactic gestagen, whilst the overall rate of abortion in the entire series of 451 women was 13.3%. It transpired on closer scrutiny of the 129 cases given prophylactic gestagen therapy that the abortion rate was 12.0% in the 108 pregnancies in which prophylactic gestagen therapy had been initiated at the time of the first missed period or later, whereas not one single abortion was registered amongst the 21 women who had been given gestagen therapy in the immediate post-ovulatory phase. It can be concluded that commencement of prophylactic gestagen therapy at the earliest possible moment provides optimum conditions for successful implantation of the fertilized ovum, especially in cases with masked insufficiency of the corpus luteum.

Abortion, Threatened

[Localization of the placenta by means of nuclear medical methods].

Accurate localization of the placenta in the cavity of the uterus is indicated prior to transabdominal amniocentesis and in cases where hemorrhages occur during pregnancy. Several nucleo-medical techniques are available for this purpose. Earlier a so-called placentography was performed, a method later replaced by placental scintigraphy. Today the gamma scintillation camera is ideal for localizing the seat of the placenta. In case of a normally embedded placenta, the profile view makes it possible to determine whether the placenta is attached to the anterior or posterior wall. In case of an anterior wall placenta it is possible with a radioactively labelled marker to draw the outline of the placenta on the abdomen of the patient so that the examiner is certain to puncture in an area that does not contain the placenta. Diagnosis of placenta praevia is assured in all cases by use of the gamma scintillation camera. Short-term introduction of an emitter into the posterior fornix vaginae makes it possible to determine the position of the internal orifice of the cervix uteri. If the caudal part of the placenta extends up to this mark, a placenta praevia is likely and all obstetrical precautions indicated have to be taken.

Female

[Conisation during pregnancy (author's transl)].

The paper reviews 15 cases undergoing conisation during pregnancy. In 2 conisation was immediately followed by radical operation for invasive squamous carcinoma. 11 patients had pre-invasive carcinoma by microscopic evidence on serial step sections, while the remaining 2 showed high-grade dysplasia of the cervix. Atypical tissue was successfully removed in toto in 6 of the 8 cases with lesions predominantly localizing at the cervical surface. Of the 5 abnormal epithelial processes which were almost entirely endocervical only 1 was completely removed. Abortion occurred in 4 of the 13 patients undergoing conisation without prophylactic cerclage. Shirodkar's operation was done at the time of conisation in 7 cases, in one after an interval of 10 weeks. This prophylactic measure failed to prevent prematurity in 2 cases, while the remaining 6 gave birth to living and mature infants between the 37th and the 40th week of gestation.

Abortion, Spontaneous

[The influence of the continuous epidural anaesthesia on uteroplacental blood flow (author's transl)].

Continuous lumbar epidural anaesthesia causes a drop in maternal blood pressure by decreasing flow resistance in the peripheral vascular bed. By infusion of 1000 ml NaCl blood pressure can be kept constant. Our work shows, that dilatation of the peripheral vessels has a positive effect in patients with reduced uteroplacental flow, as it results in an augmentation of placental flow. In women with normal placental function such an effect could not be seen. The practical use of these results is illustraded by a case report.

Anesthesia, Epidural

[Pregnancy after conisation for atypical epithelial processes of the uterine cervix (author's transl)].

At the First Department of Obstetrics and Gynecology, University of Vienna Medical School, 148 females aged under 35 years at the time of conisation were examined between 1963 and 1974 to determine their desire for children, their fertility status and the course of pregnancy. After conisation a total of only 42 females (28,4%) wanted children. Of 104 patients with preinvasive carcinoma, a desire for children was reported by no more than 21 (20,2%); among the cases with dysplasia, the number of women desiring children was equally 21 (47,7%). By the end of 1976, after a followup period of 1 to 13 years, 16 of 42 females wanting children failed to conceive, while the remaining 26 gave birth to 20 children. Judging from our admittedly small material, conisation thus apparently has no effect on conception. However, the course of pregnancy was found to be impaired in some cases by the incompetent cervical function. Twelve patients required a Shirodkar operation, which failed to prevent late abortion in 2 cases and premature delivery in 4. Nine women gave birth at or at best 3 weeks before term without preventive methods.
