[Use of intratympanic gentamycin for the treatment of Meniere's disease].

The ablation treatment of Meniere's disease by intratympanic streptomycin applications was first reported by Schuknecht in 1957. Streptomycin and gentamycin are the most frequent aminoglycosides used for the Meniere's disease treatment. Gentamycin is responsible for the damage of vestibular dark cells causing the impairment of endolymph production. This method gives the possibility for the control of the vertigo with a potential hearing preservation. Fifteen patients with unilateral Meniere's disease who had not responded to conventional therapy, were treated by intratympanic gentamycin injections. The hearing status and the caloric test were staged before and after treatment according to the AAO-HNS guidelines. Overall results after minimum 1 year follow-up in this group were as shown below: complete vertigo control--5 patients, substantial vertigo control--8 patients, complete relief of tinnitus--10, relief of aural fullness--12 patients, hearing loss--none. According to presented results, intratympanic injections of gentamycin is the useful alternative to the surgery. This method should be consider in every patient with the unilateral Meniere's disease, who had not responded to the conventional treatment.

[Study of the vestibule in patients with dizziness and vertigo after 60 years of age].

Aging of the vestibular apparatus is shown to consist of processes similar to those occurring in other parts of the nervous system. The recognition of these findings is a first step in the clinical differentiation of presbyastasis from symptoms related to specific disease processes. Authors had reviewed results of otoneurological investigations in the material of 8328 patients and realised that 1034 of them (12.4%) were over-60-year-old. These patients suffered from pathology of peripheral as well as central part of vestibular system.

[Auditory brainstem responses in the diagnosis of retrocochlear hearing loss: selected case reports].

In this study, four cases of patients with retrocochlear hearing loss are presented (acoustic neuroma, cerebellopontine angle tumour, the fourth ventricle region tumour, inflammatory or vascular lesions in the fourth ventricle region). Standard audiometric tests, auditory brainstem response and radiological examinations such as CT scan and MRI were performed in all patients. The abnormal ABR observed in all cases suggested retrocochlear hearing loss. The presence of retrocochlear disorders was confirmed by CT or MRI. The analysis of presented cases showed that the abnormal ABR results should be a sufficient basis for referring the patient to MRI examination.

[The study of the vestibular function in cochlear implant patients].

Fifty-two patients receiving a cochlear implant were evaluated using vestibular function tests. Forty patients received an intracochlear implant. The risk of losing preoperative vestibular function is estimated to be around 50%. The authors suggest to implant with cochlear prosthesis the ear with preoperative decreased or total loss of vestibular function.

[Computer analysis of nystagmus].

An algorithm for the computer analysis of nystagmus is described in this paper. This algorhythms was based on digital differentiator, which monitors the eye position. The computed eye velocity is next used as an input to the fast phase detector. Several procedures, including minimum velocity test, slope sense test, SPV and FPV relationship test are invoked to extract the fast phases from the recorded signal. Based on three-years experience (approx. 3000 examinations) using the ENG-PC System in clinical practice we can conclude, that application of the nystagmus detector was useful in most cases. In our opinion, despite from system imperfection which is observed during the analysis of recording disturbed by artifacts, this system has unquestionable advantage over the traditional technique.

[Glandular ear neoplasms].

A case of ceruminoma was described. The authors also presents actual opinions about glandular tumors of the middle and external ear, their classification and treatment.

[Glioma of the ethmoid].

The authors presented a case of ethmoid glioma which had performed as nasal polyp. They also discussed about problems of classification and treatment this type of neoplasm.

[Indications and counterindications for topical corticosteroidotherapy].

This paper presents indications for topical corticosteroidotherapy in rhinitis and nasal polyps. The differential diagnosis including etiology and pathogenesis of diseases especially aspirin intolerance, are taken into consideration. The main indication for topical corticosteroidotherapy is local nasal eosinophilia. The authors everyday clinical practice help to establish considerations for the use of corticosteroids.
