Retrospective inquiry investigations of 263 women with epilepsy at the age above 15 years demonstrated that the reproductive functions in these women were twice lower than in the general population of this region. The number of unmarried was 7% above that in the population, 25% of married epileptic women had no children, and most married women controlled the birth rate resorting frequently to artificial abortions. The index of perinatal mortality of the newborns, dead fetuses and early deaths) was over three times higher than in the population: 88.2 and 25.6 respctively. The risk of spontaneous abortion was 11.3%, the risk of stillbirth 3.9%, the risk of early neonatal death 4.6%. The risk of spontaneous abortion and early neonatal death was higher in the group of women taking anticonvulsants during pregnancy.
In a young man at the age of 29 and 35 years without risk factors for atherosclerosis and, consequently, cerebrovascular disease two severe strokes of atypical clinical pattern developed leading to considerable impairment of psychomotor fitness. Diagnostic tests and study of family members demonstrated familial hyperlipidaemia type IV.
A rare case of Hodgkin's lymphoma metastasis to the spinal cord is reported. The first signs of the metastasis appeared in the 6th year of cytostatic treatment. Diagnostic difficulties hampered causative treatment.
In a man aged 40 years frequent attacks of basilar migraine with consciousness disturbances and signs of central nervous system defects developed due to circulatory failure in the posterior cerebral arteries and cerebellar arteries. The frequency and intensity of migraine attacks decreased only after treatment with flunarizine.
A case of Isaacs-Mertens syndrome is reported in which the predominating clinical symptom was muscular rigidity. Characteristic bioelectric activity was recorded in resting muscles. Good response was observed after diphenylhydantoin.
In 83 patients aged 17-68 years somatosensory evoked potentials by median nerve stimulation, and visual and auditory evoked potentials were studied 5-28 days after craniocerebral trauma. Brain concussion was diagnoses in 43 cases on the basis of neurological examination, CT and duration of unconsciousness. In the remaining 40 cases brain contusion was diagnosed. In SSEP the latency was calculated of waves N9, N13, P16, N20, P22, N35 and P40: in the visual evoked potentials the latency of the P100 component, and in auditory evoked potential the latency of waves I, III and V, and interpeak latency I-III, III-V and I-V SSEP changes were found in 39% of cases of brain concussion and 52.9% of brain contusion cases. The abnormalities in both groups involved mainly the component of latency and deviation P100 of visual evoked potential P40 and N35. Prolongation of the latency of P100 of the visual evoked potential was recorded in 20% of patients with brain concussion and 16.7% with brain contusion. Auditory evoked potentials were abnormal in 10.3% of brain concussion and 26.5% of brain contusion cases. In 64 cases all three types of evoked potentials were studied and pathological changes in at least one of these types were found in 56.4% of brain concussion and 72% of brain contusion cases. The results show that as least in a part of cases diagnosed as brain concussion according to generally accepted criteria, central nervous system injury is present.
In 42 patients aged 18-68 years somatosensory, visual and brainstem auditory evoked potentials were studied in the first month after trauma and after 6-8 months after craniocerebral trauma for evaluation of their diagnostic importance in less severe craniocerebral trauma. On the basis of the results of neurological examination, CT of the head, and duration of unconsciousness in 22 cases brain concussion and in 20 cases brain contusion was diagnosed. In patients after brain concussion the frequency of early and late abnormalities after head trauma was as follows: for SSEP 36.4% and 27.3% respectively, for visual evoked potentials 21.1% and 5.3%, for auditory potentials 9.5% and 4.8%. In cases of brain contusion abnormalities were found in the 1st month and 6-8 months after trauma in SSEP in 60% and 20% respectively, in visual EP in 11.1% and 22.2%, and in auditory EP in 30% and 40%. In the group of concussion the greatest and most persistent changes developed in the later components od SSEP. Among certain patients with brain contusion deterioration of the results of visual and auditory evoked potentials were noted late after trauma.
58 men treated for epilepsy, average age 38.5, and their 100 children aged on average 11.2 were subjected to psychological tests adapted to patients age. Mental effectiveness test (IQ) were based on WHO criteria for Wechsler Scale. Among the epileptic patients 77.6% presented normal IQ tests results, whereas 5.6% were mentally retarded. In the children group, 85% achieved normal IQ tests results. The proportion of mentally retarded children (5%) was higher than in general population. The results below the normal range of IQ were significantly more frequently observed in children whose fathers themselves achieved low IQ (p less than 0.05) or had severe form of epilepsy (p less than 0.025) or in children from families with poor socioeconomic status (p less than 0.025).
A case is described of Becker type muscular dystrophy in a man aged 52 years, in whom complete atrioventricular block was found with Morgagni-Adams-Stokes attacks. The pathogenesis of the syndrome and the necessity of implantation of a pacemaker are discussed.
EEG tracings were analysed in 100 children of 58 fathers with epilepsy. A group of children aged up to 18 years was isolated and in 40 of them serial EEG investigations were carried out during 4-5 years. In this group abnormal tracings were found in 51% of cases, with seizure activity in 31%. In five children (5%) epilepsy was diagnosed. Seizure activity was significantly more frequent in girls than in boys (42.8% vs 19.6%). Seizure activity occurred with similar frequency in children born before or after manifestation of the disease in the father (30.1% and 31.9%). The observation of evolution of seizure activity during 4-5 years suggests that the greatest penetration is at the age of 4-7 years (52.6%) and then it decreases gradually. Non-seizure pathological activity with features of bioelectric immaturity was observed most frequently in the youngest children aged 1-3 years, especially in boys. The evolution of these changes was bidirectional: in most cases the tracings became normal within 4-5 years, in a lower number of cases seizure activity appeared.
The evaluation of health of the offsprings of male epileptics, rarely undertaken as yet, has shown that epileptic seizures occurred in them ten times as frequently as in the general population and threefold as frequently abnormal EEG tracings with burst activity were observed. The finding of retarded mental development in 6% in this group, that is two times as frequently as in the general population, and the presence of minor anomalies in 23% of these children, as well as a slightly greater prevalence of major anomalies (3%) than in the population are the reasons that their health may be regarded as worse than in the general population. Moreover, it was found that the health of the children born after the onset of epilepsy in the father was worse than that of children born before this onset. Another observation was that male epileptics had a statistically significantly greater risk of having offsprings with generally poor health, mental retardation and presence of congenital anomalies, especially in cases with an unfavourable course of the disease, whose offsprings were mentally dull and in the family the socioeconomic conditions were poor. However, relation to the results of investigations of the offsprings of female epileptics often published in the literature, the health state of the offsprings of male epileptics was better.
Forty children of epileptic fathers were followed-up for 5 years. The mean age of the children was 10.2 years. Two girls (5%) had mild epilepsy, one child (2.5%) had a major anomaly, and 12 had minor anomalies (30%). Permanent mental impairment was found in 2 children (5%) with infantile cerebral palsy, and another 2 (5%) had borderline mental retardation. During this five-year observation of mental development this development was normal in the remaining children, and in 15% of them the IQ exceeded 116. Emotional disturbances were present in one-fourth of this group and they were connected with poor socioeconomic conditions and family breakdown. Abnormal EEG tracings were found in half the children, and in 40% of them seizure activity was present. The observed regression of seizure activity was related to age. In 60% of the children the general health state (somatic development, frequent infections) was not satisfactory.
