Systemic NO synthase inhibition blunts the chronotropic, but not the inotropic response to isoprenaline in man.

There is evidence that nitric oxide (NO) is involved in the chronotropic, the inotropic, and the vasodilator response to beta-adrenoceptor agonists. In the present study we hypothesized that inhibition of NO synthase may modulate the systemic vascular and cardiac effects of isoprenaline, a beta-adrenoceptor agonist, in healthy subjects. Subjects received stepwise increasing doses of isoprenaline (0.1-0.8 microg/min) in the absence or presence of systemic NO-synthase inhibition using two intravenous doses of N-monomethyl-L-arginine (L-NMMA; dosage 1, 3.0 mg/kg over 5 min, followed by 30 microg/kg/min over 75 min; dosage 2, 6.0 mg/kg over 5 min, followed by 60 microg/kg/min over 75 min) or peripheral vasoconstriction using exogenous endothelin-1 (ET-1; 5.0 ng/kg/min for 80 min). The chronotropic (RR interval) and the inotropic (QS2c) responses were assessed by noninvasive measurement of systolic time intervals. L-NMMA alone did not influence QS2c, but did increase the RR interval (P < 0.001) and the mean arterial blood pressure (P = 0.003). L-NMMA did not attenuate the blood pressure and the QS2c responses to isoprenaline, but significantly and dose-dependently blunted the heart rate response to beta-adrenoceptor stimulation (P = 0.029). ET-1 decreased the RR interval (P < 0.001) and increased the mean arterial blood pressure (P = 0.028). Our results indicate that beta-adrenoceptor mediated effects on the heart rate are much more susceptible to NOS inhibition than inotropic responses. This indicates that NO has an important role in heart rate control during beta-adrenoceptor stimulation.

Effects of atropine and propranolol on retinal vessel diameters during isometric exercise.

PURPOSE: There is controversy regarding the nervous control of retinal blood vessels in humans. Most in vitro studies indicate that the intraocular part of the central retinal artery lacks autonomic innervation. We investigated the response of retinal vessels to isometric exercise during blockade of beta-receptors (propranolol) or muscarinic receptors (atropine). METHODS: Twelve healthy subjects performed squatting for 6 min during infusion of either propranolol atropine or placebo. Blood pressure and pulse rate were measured non-invasively. Retinal vessel diameters were measured continuously using the Zeiss Retinal Vessel Analyser. RESULTS: Squatting induced a significant increase in blood pressure and pulse rate, which was paralleled by a decrease in retinal vein and artery diameters. Atropine did not change the retinal vessel response to isometric exercise. Propranolol significantly blunted the exercise-induced vasoconstriction in retinal arteries. CONCLUSION: This result likely indicates propranolol-evoked vasoconstriction in the extraocular parts of the central retinal artery during isometric exercise.

[The evaluation of the quality of life among professional soldiers with diagnosed alcohol addiction syndrome].

Presented paper is the first estimation probe in Poland of the "quality of life" of alcohol addicted people. The group of 70 professional soldiers that had been undergo treatment in Military Hospital the Section of Psychiatric Clinic in Cracow in years 1996-1998 has been investigated as alcohol addicted syndrome. For the research it has been used: the questionnaire with sociodemographis and epidemiological data, Polish version questionnaire SF-36 for "quality of life" research and Michigan Alcoholism Screening Test (MAST)--the questionnaire for estimation alcohol problems. The main aim of investigation was to recognise the influence of alcohol addicted syndrome that impact on "quality of life" estimation, range changes and their dynamics that depend on alcohol addicted syndrome degree, in age, military degree. The exploration aim was some sociodemographis and epidemiological data analysis. Results confirm the lower "quality of life" within estimated group. All ingredients Q1 have decreased, and the highest differences related such ingredients as: "physical limits in role perform", "social function", "emotion limits in role perform" so that "social activity sphere" creation ingredients. The estimation hypothesis related to QL decrease together with addiction degree (numbers of MAST) and decrease of "quality of life" together with age within estimated group have been not confirmed. The analysis of chosen sociodemographis variables has shown the significant variation, relative to QL and: "sexual live", "alone drinking" and "conflict". The differences between peoples who are alcohol addicted and non habitual drinking alcohol peoples has been confirmed within range; "frequency of alcohol drinking", "quantity of single drinking alcohol", "frequency to be drunk", "reasons of drinking", "behaviour after drinking", "suffering after drinking", "continuos drinking" or "alcohol tolerance".

[RSV infections in children including managed cases in premature babies].

The authors assessed RSV infection issues within the Department of Neonatology during the year 2000/2001. The epidemiology, pathology, pathogenesis and the clinical course were discussed. The treatment and prevention patterns have been also presented. There were 5 cases of RSV infection of premature neonates within the Department of Neonatology. One of the mothers was probably the source of infection. The course of infection was described as severe in two infants (diagnosed respiratory insufficiency) and moderate in two other cases (respiratory disorders improved with n-CPAP). The symptoms of disease have been mild only in one case. Prematurity, age between 2-6 months, lung, and heart pathology and immunodeficiency must be considered as negative prognostic factors. Development of vaccination against RSV infection seems to be crucial solution for preterm neonates.
