Prognostic significance of lymph node histology in patients with squamous cell carcinoma of the larynx, pharynx, or oral cavity.

In a double-blind retrospective analysis, sections of lymph nodes regional to head and neck squamous cell carcinomas were microscopically examined to assess morphologically the immunologic pattern of response. Patients whose nodes showed evidence of immunologic stimulation had five-year survival rates significantly higher than those whose nodes showed no evidence of immunologic stimulation. None of the patients whose nodes showed the lymphocyte depletion pattern survived five years. The stage or histologic grade of the tumors did not influence these correlations. Metastases occurred much more frequently in patients whose nodes showed immunologic activity than in those whose did not. The data support the concept that immunologic capacities are important host defense mechanisms against malignancy. Histologic assessment of immunologic activity in regional nodes seems to be an important parameter for predicting survival.

Thyroid function after radiation and surgery for head and neck cancer.

Preoperative radiation followed by surgical excision is accepted therapy for head and neck cancer. The effects of these modalities on thyroid function were evaluated in a prospective study. Sixty-one patients were given cobalt 60 radiation. Results showed that 41 patients (67%) were euthyroid, 12 patients (20%) were clinically and chemically hypothyroid, and 8 patients (13%) had transient loss of thyroid reserve. Permanent hypothyroidism occurred frequently in patients with hemithyroidectomy, but rarely in those treated with radiation alone. Onset of hypothyroidism was zero to six months, with transient loss of thyroid reserve occurring up to 18 months. This constitutes the initial report of an ongoing systematic study of thyroid function in such patients. The high incidence of hypothyroidism indicates a need for careful periodic evaluation.

[Thin layer chromatographic separation and enzymatic detection of carbamate pesticides in plant extracts].

The authors developed a thin-layer chromatographic technique for the separation of carbamate pesticides from plant material and an enzymatic method for their determination. The cellulose plates were impregnated with polar substances (ethylene glycol, triethylene glycol and beta,beta'-oxydipropionitrile). n-Heptane/chloroform and iso-octane/dioxane served as solvents. The slight enzyme-inhibiting effect and the colouration of the plant extracts render the detection of the pesticides difficult. Nevertheless, the separation and the enzymatic detection of Methomyl, Carbaryl, Baygon and Aldicarb could be achieved. The Rf values of the carbamates are not influenced by the kind (apples, pears, onions) and the quantity (50--200 mg) of the plant material.

Necrotizing sialometaplasia of the nasopharynx.

Necrotizing sialometaplasia is an uncommon salivary gland disease originally described by Abrams et al in 1973. The disease may occur wherever salivary gland tissue is found. Theories on the etiology of this disorder have been advanced, but never definitely determined. Treatment consists of adequate biopsy and observation until healing occurs at six to eight weeks. While the disease is considered benign, its similarity to more aggressive neoplasms can be both disturbing and misleading to the patient and the clinician. Such a case of necrotizing sialometaplasia of the nasopharynx is presented to demonstrate the clinical and histological similarity of this disease to carcinoma. In this case, the patient first presented with a neck mass which could easily have been mistaken for a regional metastasis. The current literature is reviewed.
