The neurologic workup by the primary care physician.

The family physician can and should manage many neurologic problems. A satisfactory neurologic examination also requires a thorough general assessment of the patient's health. The history usually points up the likely diagnosis, while the examination pin-points the lesion. The workup should include routine studies and, when indicated, more specialized diagnostic procedures.

Humans↗

Craniofacial dysostosis with syringomyelia and associated anomalies.

A 16-year old boy had craniofacial dysostosis, hydrocephalus, and syringomyelia. Other anomalies included platybasia, a Klippel-Feil anomaly, webbed toes, and a cutaneous hemangioma. Evaluation included cerebral angiography, ventriculography, and myelography.

Abnormalities, Multiple↗

Phosphorylase deficiency associated with isometric exercise intolerance.

In the patient reported here, most of the clinical and biochemical correlates of McArdle's disease were present; however, symptoms developed after isometric rather than after isotonic exercise. Histochemical reactions and assay of muscle homogenates confirmed the absence of phosphorylase; however, venous serum lactate increased in the arm after isometric and isotonic ischemic exercise, although the increase was significantly less than in a normal control. Although the symptoms differ significantly from those in McArdle's disease, this disorder could be a variant.
