An evaluation of the extent of rehabilitation of 512 Danish dialysis and transplant patients, based on a questionnarie, has been carried out. All transplanted patients and dialysis patients and dialysis patients with only slightly limited working capacity before treatment experienced an improvement, whereas disabled dialysis patients did not experience major changes. Sixty per cent of the patients receive disablement pension and the allotment of pension early in the course of treatment is suspected to be one of the main reasons for the low degree of vocational rehabilitation.
In a double-blind study 236 nulligravidae were randomized to insertion of standard models of two commonly used intrauterine devices, Multiload and Gravigard, or shorter versions of these devices. Before insertion the total axial uterine length and the length of the endometrial cavity were determined. After 12 months the total continuation rate was 57.2%. The clinical performance of Multiload models was not statistically different from that of the Gravigards, and the short IUD-types did not do better than the longer ones. Among the longer models a significant higher continuation rate was observed with the Multiload. The frequency of side effects was not increased when the total uterine cavity length was less than 7 cm. As the incidence of side effects was higher than normally reported after insertion of IUD's in parous women it is concluded that insertion of IUD's in nulligravidae should be performed only when other methods of contraception are unacceptable. The ratio between the lengyh of the uterine cavity and the length of the IUD seems to be of no significance for the clinical events following insertion of IUD's in nulligravidae.
An evaluation of the extent of rehabilitation of 159 haemodialysis patients and 36 transplant patients from 6 of the 9 Danish nephrological centers was carried out. Half of the patients experienced their working capacity to be severely reduced prior to treatment and the percentage did not change during treatment. At the time of interview 88 percent of the patients received full disablement support and only ten per cent had retained their previous vocational position.
OBJECTIVES: To review our studies on the clinical and metabolic impact of contraceptive methods in women with insulin dependent diabetes mellitus (IDDM) and women with previous gestational diabetes mellitus (GDM) in order to provide suggestions for the contraceptive counselling of these women. METHODS: The clinical events following first insertions of copper IUDs were studied in 103 women with IDDM and in 119 non-diabetic women. Moreover we studied the effects on glycometabolic control and lipid metabolism in women with well-controlled IDDM using low-dose oral contraceptives (OCs) containing ethinylestradiol combined with norethisterone (n = 10), levonorgestrel (n = 9) or gestodene (n = 11). Hemostatic and endothelial function was also studied in the women using the gestodene-containing preparation. Finally, we studied the impact of oral contraceptives on the insulin sensitivity in women with previous GDM. RESULTS: The continuation rates and indications for medical removal of the IUDs were similar in the diabetic and the non-diabetic women. There was no increased pregnancy rate or increased frequency of pelvic inflammatory disease in the diabetic women. The glycemic control was not changed by the OCs and none of the treatment regimens were associated with changes in plasma lipids linked to increased risk of atherosclerosis. Indications of increased fibrin formation, which seemed to be compensated by increased fibrinolytic activity, were noted with the gestodene-containing preparation. None of the women developed microalbuminuria during the study. Compared to normal women, we found reduced insulin sensitivity in the women with previous GDM using the pill. CONCLUSION: Intrauterine devices and barrier methods can be used by diabetic women with the same reservations as in the general population. Low-dose OCs do not influence the glycemic control and have no adverse impact on plasma lipids. The balance between fibrin formation and resolution was maintained during intake of the gestodene-containing pill. Our findings suggest that combined oral contraceptives can be used in women with uncomplicated IDDM and in women with previous GDM if clinical and metabolic monitoring can be ensured.
