Epidemiological and clinical study of epilepsy in Benghazi, Libya.

An epidemiologic study of adult patients (aged greater than 15 years) with epilepsy was made over a 2-year period, through the policlinics, EEG laboratory, and university hospitals in Benghazi, northeastern Libya. Based on a total of 568 patients (310 male and 258 female), the prevalence rate on 31 December 1984 was 2.3/1,000 in the population above the age of 15 years (1.9, if age standardized). The age-specific prevalence per 1,000 population decreased from 3.6 in the 15-24 year age group to 1 in the greater than 60-year age group. For 70.8% of the patients the onset of epilepsy occurred in the first 2 decades of life, and for 7.6% it commenced after the age of 40. The duration of epilepsy when first seen in the study period was less than 1 year in 14.6%, and greater than 10 years in 25.7%. The percentages of primary generalized, partial seizures secondarily generalized, simple partial, and partial complex seizures were 55.6, 18.8, 10.4, and 14.6%, respectively. Antecedents that could be considered potential causes of epilepsy were found in 17.5% of cases. Fifty-five patients had status epilepticus, 24 of whom had symptomatic epilepsy and eight recurrent status. EEG revealed generalized epileptic activity in 199 cases and focal seizure activity in 118. Computerized tomography (CT) scan, done in 219 patients, disclosed abnormal results in 50. Among those with abnormal CT scans, 84% had partial seizures. Monotherapy with carbamazepine or phenytoin was used in the majority of cases. The influence of social, cultural, and religious factors in the diagnosis and management of epilepsy as seen in Benghazi are discussed.

Melanotic differentiation in dysembryoplastic neuroepithelial tumor.

Dysembryoplastic neuroepithelial tumor (DNT) is an uncommon intracranial tumor which is usually encountered in patients with medically intractable complex partial seizures of temporal lobe origin. Melanotic differentiation in a DNT has not been reported previously. Herewith we report a case of DNT with unique melanotic differentiation in a 23-year-old female who presented with medically intractable epilepsy. She underwent craniotomy and tumor decompression with a good outcome. We present the histopathological features of DNT with melanotic differentiation and discuss its prognostic significance.

Cardiac dysautonomia in leprosy.

Responses to various tests of cardiovascular reflexes were studied in 15 patients with leprosy. Heart rate was continuously monitored by electrocardiogram and blood pressure was recorded simultaneously. Three patients showed evidence of cardiac dysautonomia, showing involvement of both the sympathetic and parasympathetic systems. These three patients belonged to the bacilliferous group.

Ischaemic stroke in the young adult: clinical features, risk factors and outcome.

BACKGROUND: Stroke is an important cause of morbidity and mortality in young adults, especially in developing countries. This study aimed to evaluate the clinical features, risk factors and outcome of ischaemic stroke in young adults aged 15-45 years. METHODS: A retrospective review was done of the medical records of 177 patients seen in a tertiary referral centre in Thiruvananthapuram, Kerala between January 1988 and March 1994. RESULTS: There were 135 males and 42 females with a mean (SD) age of 34.7 (8) years at onset of stroke. An aetiological categorization of stroke was obtained in 111 (63%) patients and it was uncertain or unknown in the rest. Athero-thrombotic stroke and cardio-embolic stroke occurred in 24% and 17%, respectively. Hypertension, smoking, hyperlipidaemia and athero-thrombotic stroke were significantly more prevalent in the 31-45 year age group compared to the 15-30 year age group. There was only one patient with postpartum intracranial sinovenous thrombosis. At a mean follow up of 7 months (range 1-62 months), 75% of the patients were independent or only mildly disabled. The case-fatality rate was 1%. CONCLUSION: Ischaemic stroke in the young adult is more frequent in males. The aetiopathogenesis can be determined in the majority and the athero-thrombotic process predominates. The mortality is negligible and the functional outcome is good in most patients.
