Lack of spontaneous activity of cutaneous spinal dorsal horn neurons in awake, drug-free, spinally transected cats.

Extracellular single-unit activity was recorded from neurons with cutaneous input in the dorsal horn of the spinal cord (L4-L6) of awake, drug-free cats before (for several weeks) and after (day 1 through day 7) cord transection (T12). The spontaneous activity of the neurons was minimal or nonexistent in both recording conditions. The lack of spontaneous activity following spinal cord transection contrasts sharply with activity recorded in acute spinal-cord transected preparations in which high rates of spontaneous activity have been reported. This discrepancy may reflect an important difference between the chronic, awake, drug-free and acute preparations.

Partial splenic embolization for hypersplenism concomitant with or after arterial embolization of hepatocellular carcinoma in 30 patients.

PURPOSE: To study the value of partial splenic embolization (PSE) for the treatment of hypersplenism in patients undergoing embolization of hepatocellular carcinoma (HCC). METHODS: Transcatheter hepatic arterial embolization (THAE) combined with PSE was performed in 30 patients with HCC complicating liver cirrhosis, portal hypertension, and hypersplenism. Gelfoam sponge was used as the embolic material for PSE and limited to 100-150 pieces. RESULTS: More than 50% of splenic parenchyma was infarcted in 27 patients. Leukopenia and thrombocytopenia were corrected by PSE in 25 of 27 patients with hypersplenism. In 26 patients with esophageal varices, including 5 patients with bleeding, no rebleeding occurred during a 6-17 month follow-up. Hypersplenism was not corrected in 2 of 3 patients whose infarcted splenic parenchyma was less than 50%. No splenic abscesses or other severe complications were observed. Of the 30 patients treated, 19 are still alive after 1 year. CONCLUSIONS: THAE combined with PSE is a safe and effective measure for patients with HCC.

Noise exposure among construction electricians.

Data-logging noise dosimetry was used to assess the exposure levels of electricians working for a major electrical subcontractor in Washington State at five sites using four types of construction methods. Subjects documented activities and work environment information throughout their work shift, resulting in an activity/exposure record for each of the 174 full-shift samples collected over the 4-month duration of the study. Over 24% of the TWA samples exceeded 85 dBA; 5.2% exceeded the federal Occupational Safety and Health Administration permissible exposure limit of 90 dBA. The National Institute for Occupational Safety and Health exposure metric, which specifies a 3-dB ER, was also utilized; using this metric, 67.8% of the samples exceeded 85 dBA and 27% exceeded 90 dBA. Subjects were directly observed for a subset of 4469 min during which more detailed activity and environmental information was recorded. Linear and logistic regression models using this subset were used to identify the determinants of average exposure, and exposure exceedences, respectively. These models demonstrated the importance of multiple variable modeling in interpreting exposure assessments, and the feasibility and utility of modeling exposure exceedences using logistic regression. The results further showed that presumably quiet trades such as electrician are at risk of exposure to potentially harmful noise exposures, and that other workers' activities and the general environment contribute substantially to that risk. These results indicate that noise control strategies will have to address the construction work environment as an integrated system.
