Disease-adapted relapse therapy for ovarian cancer: results of a prospective study.

Primary therapy of advanced ovarian cancer is standardized, the therapy in relapsed ovarian cancer however is still controversial. In a prospective study the benefit of secondary surgery and/or second-line chemotherapy were evaluated. 139 patients with relapsed ovarian cancer were stratified according to a treatment plan: patients with early relapse (recurrence-free interval 12 months) or primary progression during chemotherapy (n=43) were treated chemotherapeutically with etoposide (p.o. vs. i.v.). Patients with late relapse (recurrence-free interval >12 months, n=96) were referred, if possible, to a secondary debulking operation, followed by a platinum-based chemotherapy. Remission-rate, toxicity and survival time were analyzed. Median survival time in the group was 15 months compared to 30 months in patients with late relapse (p=0.0004). Within the group patients with secondary debulking and chemotherapy (n=59) had a statistically significant survival advantage compared to patients who had only chemotherapy (n=37) (38 vs. 12 months, p<0.0001). The unfavorable group of patients with early relapse should be treated chemotherapeutically, whereas in patients with late relapse a secondary debulking seems to improve prognosis.

[Sonography of fetal growth behavior in maternal diabetes mellitus].

In the Gynaecological Hospital affiliated to the District Hospital of Karl-Marx-Stadt, which is a care centre for pregnant diabetic women, 363 diabetic women of classes White A-F 1779 were subjected to ultrasonic examinations between 1982 and 1988. In this connection, nominal-value graphs were prepared to show the biparietal diameter (BIP), the medium thorax diameter and the head-thorax index in dependence upon the gestational age. These nominal-value graphs give a general idea of the specific fetal growth behaviour in case of diabetes mellitus. They permit to reliably diagnose a fetal hypertrophy or hypotrophy. Moreover, they provide a starting point for a more effective coverage of gestational diabetics and open up new prospects for insulinisation.

[Effect of the delivery procedure on perinatal mortality and neonatal morbidity of underweight infants in the breech position].

The paper deals with the effect of the delivery technique used on perinatal mortality and neonatal morbidity rates. Overall mortality of 236 underweight breech babies (less than 2,500 g) and a number of morbidity parameters of 68 new-born children of 1,000 to 2,000 g birth weight are studied collected over a period of comparable perinatological management. The results show that cesarean section is the most favourable technique for children of 1,000 to 2,000 g birth weight. To do so, it is necessary to exclude non-viable malformations prenatally and to ensure a reliable neonatological intensive care. Another major problem is to prevent premature birth. On the basis of the results obtained and taking into account anamnestic and obstetrical risks, we give preference to an adequate selective section to deliver underweight children from breech presentation, but do not rule out vaginal methods of delivery using good techniques and proceeding as carefully as possible.

[Potentialities and limits of cryosurgery of vulvar cancer--12 years' results of therapy].

From 1971 to 1982 146 patients suffering from vulvar carcinoma have been cryosurgically treated at the Department of Gynecology and Obstetrics of the Medical Academy of Erfurt. There were primary inoperable, progressive, metastatic or recurrent cases after preliminary operative or radiation therapy. The 5-years surviving rate by cryotherapy, rarely combined with other methods of therapy (cytostatic therapy with Bleomycin, x-radiation, operative lymphonodectomy) was 38.5%. Our results show that cryosurgery extends the possibilities of conventional treatment of vulvar carcinoma both in palliative therapy of progressive cancer and in primary treatment, especially in patients, who cannot be charged by other complicated operations. According to every other cancer-therapy cryosurgery should be performed only in a gynaecological centre for cancer treatment.

[Cryotherapy of vulvar precancerous conditions--results of 12 years' treatment].

The recommendations for therapy of the facultative and obligate precancerous diseases of the vulva contain a lot of measurements extending from a local conservative treatment to the vulvectomy designated by a very different share of recurrences. With regard to the sure analgetic and controllable cell destroying effects of the coldness we cryosurgically treated 32 patients suffering from a kraurosis and 3 women with a histological secured Bowen's disease at the Department of Gynecology of the Medical Academy of Erfurt from 1971 to 1982. The present outcome proves this measurement being a very successful method but not always without recurrence. Therefore we consider cryosurgery as a real enrichment in conventional treatment of the precancerous diseases of the vulva but this measure can not be taken for the method of choice.

[Genital tuberculosis of women (author's transl)].

Genital tuberculosis of women, one of the extrapulmonary forms of tuberculosis, has remained to be in existence, though morbidity and mortality of the disease are on a declining trend. Affected are women of all age groups. A general account is given of gynaecological and obstetric aspects relating to the clinical pattern of genital tuberculosis, with reference being made to the author's own findings recorded from 155 patients in ten years, on the basis of bacteriologically or histologically secured diagnosis. These results have reaffirmed the need for complex diagnosis, therapy, and aftercare for female patients with genital tuberculosis. It is a challenge which, together with the demand for thorough prophylaxis, cannot be met unless meaningful interdisciplinary cooperation is ensured.

[Course of pregnancy following cryotherapeutic treatment of the cervix uteri].

Course of pregnancy of 26 women is reported in this paper. All of them had undergone cryosurgical treatment during or prior to one of their pragnancies. The results obtained from such surgical treatment are compared to those recorded from traditional operations on the uterine cervix. Scarless healing of the portio and full retention of its elasticity are points made to claim good functional results of cryotherapy. Suggestions are made for use of cryosurgery during pregnancy.

[Problems of urogenital abnormalities--uterus bicornis and bicollis, right double ureter with hydroureter, urogenital cyst formation].

Reported in this paper is a rare urogenital malformation in a patient, 17 years of age. Extensive presurgical diagnosis of cele and cysts formation in the minor pelvis, with the right parts of vagina and ureter of a doubled urine-excreting system being involved, was confirmed by surgical therapy in two sessions. This hydronephrotic ureter, with its normal ostium located in the urinary bladder, was found to communicate with the right cervix of a double-bicorn uterus. Excess pressure in the cavities caused fluid to empty splashingly from the right vagina into the left which was in normal condition. The effects of such combination of malformations on individual life are discussed.

[Accuracy and general relevance of preoperative lymphography in gynaecology (author's transl)].

Lymphographic findings obtained from 304 patients were compared with results obtained from histological testing of extirpated lymph nodes, with the view to making an assessment of accuracy of presurgical lymphography. Agreement was recordable in 82.3 per cent of all cases, while 3.3 per cent of the findings were falsely negative and 4.9 per cent falsely positive. The relevance of preoperative lymphography to diagnosis and planning of therapy for various gynaecological carcinomas is discussed.

[New technique for use of low temperatures in uterine cavity (author's transl)].

Cases of repetitive or extremely secondary anaemia in the wake of therapy-resistant genital bleeding so far had been tackled by radium treatment, which led to radiological castration. Such approach was based on hysterosalpingography and established absence of malignancy, and it used to be applied also to younger patients with reduced or lost operability. The author have devised and tested a simple cryo-intrauterine applicator by which lasting or strong recurrent uterine bleeding can be readily controlled. Complete removal of the endometrium is now possible without any impairment of ovarian function. Foundations and methods are described in detail, with clinical conclusions being drawn.

[Role and results of placental scintigraphy in modern obstetrics (author's transl)].

Placental scintigraphy was performed on 367 pregnant women over seven years. Indications were prepartum bleeding in 200 patients, Rh incompatibility in 111, and other types of high-risk pregnancy in 56. Nuclide113m In Cl3 was primarily used, with a scintillation camera being used for scintigraph recording. The sensitivity accounted for 98 per cent. The placenta was found to stick to the fundus or other parts of the uterus in 280 cases or in deep positions in 37 cases. It was partially existent in 27 cases and totally existent in 16 instances. Placenta praevia had been the cause of prepartum bleeding in 17 per cent of the patients checked, in that context. Abnormal foetal presentations were recordable more often from those cases, but rates of malformation were not increased. Clinical retention time and mode of delivery were found to depend on both obstetric results and position of the placenta. - Dependable findings with regard to localisation prior to the 28th week of pregnancy were scintigraphically obtainable only from 82 per cent of all cases, which figure went up to the order of 91.2 per cent in response to scintigraphy as of the 35th week of pregnancy. - The scintigraphic findings recorded from 209 patients were rechecked and verified in 193 cases, bringing the accuracy to 92.4 per cent.
