US, CT and MR imaging characteristics of nephroblastomatosis.

OBJECTIVES: To describe the imaging features of nephroblastomatosis with US, CT and MR, to point out characteristics of differentiation between nephrogenic rests (NR) and Wilms' tumour (WT) and to determine the most appropriate imaging modality. MATERIALS AND METHODS: We reviewed the US, CT and MR images of 29 cases of histopathologically confirmed nephroblastomatosis sent to our department for reference evaluation (German nephroblastoma study). The series included 17 kidneys with NR, 6 kidneys with WT and 32 kidneys with both NR and WT. RESULTS: NR presented as multinodular, peripheral, cortical lesions, the diffuse form of distribution being less common. Foci were homogeneous and of low echogenicity, density or signal intensity. The lesions were most clearly depicted with contrast-enhanced CT and T1-weighted (T1-W) MR images. Lesions smaller than 1 cm were rarely identified by US. The most reliable criterion to differentiate NR from WT was their homogeneity. CONCLUSIONS: Contrast-enhanced CT and T1-W MR images are of similar potential and superior to US in the diagnosis of nephroblastomatosis. Due to the significant radiation dose of serial CT, MR imaging should be the method of choice wherever it is available. The cost-effectiveness and availability of US makes it ideal for serial follow-up of known lesions.

[Conventional imaging and computerized tomography in diagnosis of skeletal metastases].

The most effective whole-body screening test for bone metastases is the radionuclide bone scan. Conventional radiography is the best modality for characterizing metastatic lesions. Furthermore, conventional radiographs are useful for depicting impending or early pathologic fractures. The radiographic sensitivity depends on the extent, location and type of bone metastases. Therefore, normal radiographs in patients with either a positive bone scan or persistent, unexplained pain must be suspect. In these cases CT may establish the diagnosis. However, metastatic lesions in the marrow space without architectural changes in the surrounding bone can be detected only by MRI. The use of CT is also helpful in determining the local extent of bone destruction and extension into the surrounding soft tissues when planning surgery or radiotherapy.

[Dialysis-associated amyloidosis. Part 1: Biochemistry, clinical aspects, roentgen morphology].

Dialysis-related amyloidosis is characterized clinically by the carpal tunnel syndrome, pain and swelling of joints. These alterations are due to amyloid deposits in the carpal tunnel, in the synovia, ligaments and bones. It has been shown that beta 2m is the major component of this amyloidosis. Serum and urine concentration of beta 2m are markedly elevated in chronic renal failure due to failure of filtration and lack of metabolism by the renal tubular epithelium. beta 2m-derived amyloid is identified by immunohistochemistry using antibodies derived against beta 2m. Radiology shows cyst-like periarticular bone defects, destructive arthropathy and spondylarthropathy.

[Dialysis-associated amyloidosis. Part 2: Incidence and site of osseous amyloidosis. Dialysis characteristics and clinical findings].

In a conventional radiographic skeletal survey of 90 hemodialyzed patients, 28% (25 patients) showed renal osteopathy; 27 patients (30%) had periarticular bone cysts and 3 patients (3%) presented radiological evidence of destructive spondylarthropathy. The bone cysts were most commonly identified in the carpal bones and around the hips and shoulders; destructive spondylarthropathy was seen in the cervical and lumbar spine. Whereas characteristic radiographic changes of renal osteodystrophy could already be identified in the 1st year of dialysis, periarticular bone cysts occurred at the earliest after 2 years, most frequently after 5 years of dialysis. Destructive spondylarthropathy was seen after more than 10 years of dialysis. A positive correlation between these bone lesions and secondary hyperparathyreoidism, dialysis membranes or renal diseases was not found.

[The radiotherapy of Castleman's lymphoma].

The angiofollicular lymphoid hyperplasia, first described by Castleman, is a benign lymphoma. The authors report a very rare case of Castleman lymphoma in the parotid gland. The tumor was successfully irradiated with 40 Gy fast electrons. Treatment planning and therapy monitoring were done by means of ultrasound and computed tomography.

[Radiologic diagnosis of inflammatory and tumorous disorders of the paranasal sinuses].

In a retrospective study, 158 patients with therapy resistant symptoms of paranasal sinus affections were investigated with conventional tomography, 39 patients in addition underwent computed tomography (CT) examination. From conventional tomography films, inflammatory affections limited to the paranasal sinuses were clearly diagnosed in 99%, and benign tumours in 76%. The dignity of malignant neoplasms was correctly recognized in 100%. CT provided additional information in 10% with regard to bone destruction, in 28% with regard to brain involvement, and in 17% with regard to tumour extension into the orbita. Considering the precise diagnosis and economical factors, conventional tomography is indicated as the primary investigation procedure in the evaluation of inflammatory and benign disease of the paranasal sinuses. CT gives definite additional information in cases of suspected bone destruction and in affections extending beyond the paranasal sinuses because of its more detailed depiction of the paranasal sinus structures.

[Fine structure analysis of the petrous bone using high-resolution thin-layer computed tomography. 2: Place and success of CT in comparison with conventional x-ray diagnosis of petrous bone fractures].

Thirty patients with suspected petrous fractures following trauma were examined by high resolution CT and by conventional radiography. CT proved the most sensitive method for demonstrating petrous fractures and their complications. CT increased the rate of diagnosis by 17%. All fractures seen on CT could be classified, but accurate classification on the basis of conventional radiographs was only possible in 52% of cases. High resolution CT is superior in demonstrating damage to the labyrinth and the ossicles. Indications for conventional tomography are confined to atypical fractures with a course unsuitable for CT.

Radiological evaluation of temporal bone disease: high-resolution computed tomography versus conventional X-ray diagnosis.

Sixty-two patients with different temporal bone lesions were prospectively examined by high-resolution computed tomography (CT) and conventional plain radiography, including pluridirectional tomography. High-resolution CT enabled a clear diagnosis in 80% of cases, conventional radiology in 63%; 1.6-times more bone information was recorded by high-resolution CT which is clearly superior for imaging cholesteatomas, metastases and inflammatory processes and for evaluating osseous destruction. With regard to pathological soft tissue or effusions filling the tympanic cavities, conventional radiology shows poor sensitivity (0.61). High-resolution CT is the most sensitive method for the imaging and classification of temporal bone fractures, including labyrinthine damage and ossicular chain injuries. Only in cases of atypical fractures with an unfavourable relationship to the CT planes, can carefully directed tomography be more effective. In most cases high-resolution CT replaces conventional radiology and should be the method of choice for comprehensive radiological examination of the temporal bone.

[Angiomyolipomatosis of the kidneys--possibilities and limitations of imaging procedures].

The authors report on a 31-year old female patient with recurrent renal colic since 1981 because of bilateral renal angiomyolipomas. The value of different diagnostic methods of imaging (ultrasonography, IVP, CT, angiography) with regard to the differential diagnosis and their probability of error are discussed. CT follow-up is recommended for all patients not subjected to surgical treatment.

[Radiologic diagnosis of bone metastases].

The diagnostic relevance of the various radiological examination methods - skeleton scintigraphy, conventional x-ray film, computed tomography, sonography and magnetic resonance - was analysed on the basis of an examination of 4765 clinically or scintigraphically suspect skeleton segments in order to arrive at an effective but also economically justifiable diagnostic approach. Conventional skeleton scintigraphy should be employed as the first method of choice, since it has a sensitivity of 93% and is hence very suitable as a searching tool. To differentiate benign and malignant lesions it is absolutely imperative to conduct at the same time a x-ray control of any abnormal accumulation of nuclides, or of parts of the skeleton that display signs or symptoms. Conventional x-ray film diagnosis enabled correct diagnosis of the type of disease in 95% of the cases. In its capacity as a complementary examination method, computed tomography enabled correct diagnosis in 52% of the cases that had remained roentgenologically unclear. A pathological bone scintigram without any x-ray or CT correlate must be considered as being suspect of metastases if trauma is absent. Magnetic resonance proved to be the most sensitive method in identifying malignant infiltrations of the medullary space in two patients suffering from a clearly delineated pain syndrome with normal x-ray and CT findings. Sonography proved superior in detecting extraosseous tumour portions. The value of angiography is the preoperative vascular imaging of metastases of the possibility of performing therapeutic embolisation or intraarterial drug therapy.

[The radiotherapeutic effect on bone metastases in relation to the frequency of metastases, sites of metastases and histology of the primary tumor].

In a retrospective study on 239 patients irradiated for osseous metastases in 578 different skeletal areas, the therapy effect was evaluated in dependence on the frequency of metastases, the sites of metastases, and the histology of the primary tumors. Furthermore the duration of improved findings was verified. The primary tumor was a mammary carcinoma in 186 patients, a bronchial carcinoma in 21 patients, a renal cell carcinoma in 20 patients, and a prostatic carcinoma in 12 patients. In patients with bronchial carcinoma the relief of pain by radiotherapy was not as good as in other tumor types. However, a significant correlation between subjective therapy effect and histology of the primary tumor was not demonstrated. Remineralization was found in 55% of all irradiated skeletal areas and an unchanged X-ray picture of bone metastases in 35%. A dependence of the objective therapy effect from the histology of the primary tumor was not statistically demonstrated (recalcification rate in mammary carcinoma 62%, in prostatic carcinoma 57%, in bronchial carcinoma 28%, and in renal cell carcinoma 11%). Significant differences of remineralization were found in solitary bone metastases (68%) and in multiple skeletal metastases (56%). A significant correlation between sites of metastases and objective irradiation effect was proved by the fact that osteolytic destructions of spine and pelvis showed a better remineralization than lesions situated in the extremities. The average duration of the objective, radiologically verified amelioration of findings was 16 months in patients with mammary carcinomas and 12 months in patients with prostatic carcinomas, bronchial carcinomas, and renal cell carcinomas.

[Accelerated irradiation of bone metastases].

In a prospective study, accelerated irradiation was performed over 60 portals in 34 tumor patients with peripheral bone metastases. During a period of three days, three daily fractions of 3 Gy each were applied with intervals of four hours, resulting in a total dose of 27 Gy. Using this fractionation schedule introduced by Ammon, a reduction of metastasis-associated pain was achieved in 90% of cases (54/60). The onset of pain reduction was observed earlier than seen with conventional fractionation schedules (five fractions of 2 Gy each per week, total dose 40 Gy). With regard to objective treatment effects, no significant difference was found between accelerated irradiation and conventional fractionation, the rate of remineralization being 43% (26/60), and the rate of stabilization of formerly progressive bone destructions being 55% (33/60). Accelerated irradiation was tolerated well usually. Marked acute side reactions occurred in only one case with metastatic involvement of joint and reaction of synovia. The major advantage of accelerated irradiation is the shortening of the total treatment period and the consequent reduction of treatment stress in patients with advanced cancer.

[Computed tomography and conventional x-ray diagnosis of tumor bone lesions].

The informative value of conventional x-ray film and computed tomography was analysed on the basis of findings in 214 patients having 9 primary and 189 secondary malignant tumours as well as 16 benign bone tumours. In 44% of the benign bone tumours CT yielded decisive additional information in so far as it became possible to determine the type of tumour (4) or the paraosseous tumour extension (3), respectively. The tumour status of the primary malignant tumours was determined throughout via x-ray film, whereas intraosseous and extraosseous extension of the tumorous process was covered much better by computed tomography. CT did not yield additional information in 28% of the secondary malignant bone tumours, whereas 33% of the osseous destructions--depending on the size and localisation--could be identified only via computed tomography. Decisive additional information was gained via CT in 39% of the patients mainly in respect of intraspinal or endocranial tumour extension.

[Indications for and value of computed tomography and conventional skeletal diagnosis in suspected bone metastases].

In 1003 patients with a total of 2467 clinically or scintigraphically suspect skeletal parts, conventional x-ray examination at the time of first study resulted in 95% of cases (2331 skeletal parts) in a correct diagnosis. Computed tomography permitted an exact diagnosis in 52% of roentgenologically equivocal findings (136 skeletal parts). In 40% of these patients even by computed tomography metastasis was only suspected, in 8% there were unspecific findings, while by follow-up bone metastasis was proven. In 64.8% of the whole patient collective there were metastatic destructions and in 32.6% of patients benign lesions were found. Superiority of CT compared to conventional x-ray diagnosis resulted from exact demonstration of the intra- and extraosseous extent of lesions and the possibility of density measurements. It depended mainly upon the localisation of the pathologic process.

[Linitis plastica of the stomach in breast cancer].

Metastasis of the stomach wall in mammary cancer as first sign of disease spread 9 years after removal of the primary carcinoma is reported. The diagnosis was proven by surgical biopsy with demonstration of positive hormone receptors. The radiological and CT signs of infiltration of the stomach wall without mucosal destruction is a rare but typical finding in tubular invasive breast cancer.

[Uptake behavior of bone metastases of hypernephroma in the 99m Tc-MDP bone scintigram. A comparison with x-ray findings].

Ninety-one radiologically confirmed osteolytic metastases in 30 patients with hypernephromas were studied with regard to their uptake of 99mTc-MDP and this was compared with the radiological findings. In 16% of the radiologically proven metastases, there was no correlation with their isotope uptake. Compared with other bone metastases whose isotope uptake has been studied and described in the literature, there appears to be a higher proportion of hypernephroma secondaries that do not show uptake of 99mTc-MDP.
