[About changes in the structure of surgically treated goiter].

On the base of our own goiter material treated surgically between 1974-1996, we compared incidence of various types of goiter and thyroid carcinoma in years 1974-1985 and 1986-1996. We stated statistically significant decrease of operated Basedow goiter and significant incidence increase of thyroid cancer. In given years frequency of incidence of differentiated and anaplastic thyroid carcinoma have been changed.

Cancer in hyperthyroidism.

The aim of the study was to determine the incidence of thyroid cancer in toxic goitre. Based on the material from 14,500 goitre surgeries, including 3,466 patients with toxic goitre, incidence of thyroid cancer, in relation to age and sex of the patients were determined. In 14,500 goitre surgeries, 12,887 women and 1,613 men (F:M 7.99:1), 639 (4.4%) cases of thyroid cancer were found. Among 3,466 patients with toxic goitre, thyroid cancer was found in 21 cases (0.6%)-20 females and 1 male, including 18 nodular toxic goitre and 3 Graves disease cases. The difference between the incidence of thyroid cancer in nontoxic and toxic goitre is statistically significant.

[Thyroid neoplasms in children and adolescents--personal experience].

Between 1974-1992 we operated 12,344 patients at the Clinic of Surgery of the Institute of Endocrinology of the Lódź Medical Academy for various forms of goiter. Of these, 295 patients were between the ages of 9-18 years, including 268 (90.8%) girls and 27 (9.2%) boys. Malignant thyroid neoplasm was diagnosed in 19 of these children (6.4%). From among the 268 girls, malignant tumors were discovered in 15 (5.6%), while among the 27 boys, in 4 (14.8%). Ten of these neoplasms were carcinoma papillare, 9 carcinoma folliculare. All of the children are alive without signs of metastases or recurrence. None developed hypoparathyroidism, but one child presented unilateral paresis of the vocal chords.

[Strumectomy in children and adolescents--personal studies with 12,000 operations].

At the Surgery Clinic of the Endocrinology Institute in Lódź we operated 12 344 patients between 1974 and 1992 because of various types of goitre. Among these there were 295 children and adolescents, 2.4%- aged between 9-18 years: 268 girls (90.8%) and 27 boys (9.2%). The goitre was 9.9 times more frequent in girls than in boys. In adults the ratio was 8:1.

Gastrin and somatostatin levels in patients with gastric cancer.

Gastrin and somatostatin-like immunoreactivity (SLI) levels were studied by means of radioimmunoassay in peripheral venous blood of healthy volunteers and patients suffering from gastric adenocarcinoma or duodenal and gastric ulcers. Gastrin and SLI levels were also evaluated in patients in blood drawn from gastric veins during surgery. The elevations of gastrin and SLI levels were found in patients with gastric cancer as compared with healthy people and patients suffering from ulcers. The impairment of the negative feedback between gastrin and somatostatin secretions in patients with gastric cancer was suggested.

Cyclic AMP in non-toxic nodular goiter.

Cyclic AMP concentrations in a non-toxic nodular goiter were measured by means of the radiocompetitive protein-binding assay. It has been shown that cold thyroid nodules contain a higher concentration of cAMP than the macroscopically normal, extra-nodular thyroid tissue.
