Coeliac axis-oriented approach in surgery for advanced gastric cancer.

To achieve the systemic curative lymph node dissection in gastrectomy for advanced gastric cancer, coeliac axis-oriented approach was proposed. Mobilization of the spleen as well as the tail and body of the pancreas from the left retroperitoneum and Kocher's maneuver lifting up the hepatoduodenal ligament and head of pancreas from the right retroperitoneum permits the observation of the coeliac axis under direct vision and enables the para-aortic node dissection with ease. This approach was applied to 131 cases with advanced gastric cancer with neither mortality nor notable complication.

Emulsification tendency of silicone-phenylsilicone copolymer.

PURPOSE: We compared the emulsification tendency of silicone-phenylsilicone copolymer (DPC; 5%-phenylated, specific gravity 0.984) with that of silicone oil (SO; specific gravity 0.966) and fluorosilicone oil (FSO; specific gravity 1.256), all of which are used clinically as intraocular tamponades. METHODS: We investigated the tendencies of emulsification in SO, FSO, and DPC. Each was placed in a separate glass container with equal amounts of albumin solution (1 mg/mL) or 1 gamma-globulin solution (1 mg/mL) and shaken. We also investigated the toxicity of DPC in the rabbit eye. Following vitrectomy, we injected DPC into the vitreous cavity and assessed the retinal damage histologically. RESULTS: The SO and DPC, because their specific gravities, are closer to water, tended to become less emulsified than did FSO. We found that DPC did not cause any severe histological damage in the rabbit retina. CONCLUSION: Highly phenylated DPC is slightly heavier than water and may be used instead of FSO to treat inferior retinal detachment.

Treatment of full-thickness macular holes with autologous serum.

A total of 28 patients (29 eyes) with stage 2-4 idiopathic full-thickness macular holes were treated with the use of autologous serum. Autologous serum (20-30 microL) was placed over each macular hole followed by injection of 16% perfluoropropane gas. Postoperatively, 28 eyes (97%) had flattening of the macular hole, and the hole could not be detected in 27 eyes (93%). Twenty-two eyes (76%) showed visual acuity improvement by at least two lines or more. Preoperative factors such as good visual acuity, earlier stage, and younger age were correlated with postoperative good visual acuity. These results suggest that autologous serum is beneficial in the treatment of full-thickness macular holes.

Diffuse submucosal cysts of the stomach associated with gastric cancer: contribution of Helicobacter pylori infections.

Diffuse submucosal cysts (DSCs) in the stomach are often associated with gastric cancer and a high occurrence of multiple gastric cancers. We studied the clinicopathological features of four early gastric cancer patients with DSCs in the submucosal layer of the stomach. All patients had early gastric cancers with gastritis and erosion in the gastric mucosa, and were positive for Helicobacter pylori (H. pylori). Based on a review of the reported cases, we found that a very high proportion (>94%) of DSCs are associated with infection by H. pylori. Although DSCs have previously been considered to be paracancerous lesions of gastric cancer, we speculate that DSCs might be post-inflammatory changes following infection by H. pylori, which may result in the high incidence of gastric cancer development.

Development of an interleukin-2 slow delivery system.

The authors developed a slow delivery system for interleukin-2 (IL-2) using highly purified bovine collagen to overcome the rapid clearance and side effects of IL-2; 1 X 10(6) microns of recombinant human IL-2 was successfully infused into needle-shaped "IL-2 mini-pellets". Pharmacokinetic study in C57BL/6 mice revealed that a single subcutaneous injection of an IL-2 mini-pellet could prolong IL-2 retention and decrease maximal concentration in the serum. Elimination half-life was 360 min for subcutaneously injected IL-2 mini-pellets, while it was 8 and 15 min, respectively, for intravenous and subcutaneous injections of aqueous IL-2. No side effects were observed throughout the experiment. This slow delivery system using collagen as a carrier proved to be effective and may be applicable to other kinds of drugs.
