A case-control study of breast cancer among Japanese women: with special reference to family history and reproductive and dietary factors.

To study the effects of family history and reproductive, anthropometric, and dietary factors on the risk of breast cancer among low risk populations, we conducted a hospital-based case-control study involving 908 patients with breast cancer and their matched controls, in Japan. A positive family history of breast cancer significantly increased the risk of breast cancer (odds ratio = 1.52, 95% confidence interval: 1.14-2.03). The risk further increased with increasing number of family members affected. Obesity, single marital status, fewer births, a late childbirth, and less consumption of green-yellow vegetables and dairy products were also associated with an increased risk of breast cancer. These associations were independent in multivariate analyses. There was no increase in risk associated with consumption of high fat foods. When analyzed by menopausal status, the association with family history of breast cancer, especially in the first degree of relatives, was more evident for premenopausal breast cancer. The associations with obesity and lower consumption of dairy products were more pronounced for postmenopausal breast cancer, while those with lower parity and single marital status were stronger for premenopausal breast cancer.

[Two cases of Ewing's sarcoma originating from the adult rib].

Two cases of Ewing's sarcoma originating from the adult rib were reported. Case 1: A 23-year-old male was admitted for further examination of a right anterior chest wall tumor. The tumor was resected and the pathological examination revealed Ewing's sarcoma. Adriamycin, vincristine, cyclophosphamide and actinomycin-D were administered. After 1 year the patient died due to local relapse. Case 2: A 28-year-old male was found to have a left extrapleural tumor on chest roentgenogram. He responded to chemotherapy consisting of CDDP, etoposide, ifosfamide and radiotherapy at a dose of 38.4 Gy before surgery. On the surgical specimen no tumor cells were observed and his postoperative course has been good. According to our experience, exact preoperative diagnosis and adjuvant therapy are necessary to improve the prognosis of Ewing's sarcoma patient.

A harvesting method of monocytes/macrophages derived from bone marrow and their characteristics--special reference to the effect of 1,25-dihydroxyvitamin D3 on monocytes/macrophages.

We have investigated the relationship between monocytes/macrophages and bone metabolism with the in vitro study. In the present study monocytes/macrophages derived from rat bone marrow were harvested and their phenotype was examined. Collected cells exhibited many characteristics of monocytes/macrophages at various points. In addition, 1,25-dihydroxyvitamin D3(D3), one of the osteotropic hormones was found to alter the activities of acid phosphatase (AcPase) and lysozyme in the media and also stimulate alkaline phosphatase (AlPase) positive cells to increase in number.

Rapidly destructive coxarthrosis--a report of a patient with "strip-mining" and "interior crumbling" of bone architecture.

The pathomechanism of rapidly destructive coxarthrosis has not been clarified. Interesting findings were obtained in specimen from a 64-year-old male patient, i.e., the break-down (crumbling) of the internal structure of osteonal bone and superficial strip-mining of the femoral head. The detritic fragments and hydroxyapatite crystals were mainly deposited in the pocket of the joint capsule and some of them were embedded beneath the synovial membrane. We conjecture that apatite-associated destructive arthritis and/or detritic synovitis might accelerate the progression of rapidly destructive coxarthrosis.

[A phase II study of carboplatin in small cell lung cancer].

A phase II clinical trial of carboplatin for small cell lung cancer was conducted in 20 institutions of the National Chest Hospital lung cancer cooperative study group. Carboplatin was administered by three dosing schedule of 300 mg/m2, 400 mg/m2 and 450 mg/m2. Out of 30 patients registered in this trial, 29 patients were evaluable for response and toxicity. Seven patients achieved PR with the response rate of 24.1%. The response rates for 300 mg/m2, for 400 mg/m2 and for 450 mg/m2 were 25.0%, 8.3% and 44.4%, respectively. Thrombocytopenia (less than 7 x 10(4)/mm3) and leukopenia (less than 3,000/mm3) were observed at 25.9% and 17.2% of cases, respectively. Nausea/vomiting was also observed at an incidence of 55.2% with mild degree. No renal and ototoxic damage was observed.

[Multiple primary cancers observed after surgery of lung cancer, and their countermeasures].

Multiple primary cancers were found after surgery in 80 (2.5%) of 3,154 patients with lung cancer. Patients with squamous cell carcinoma most frequently suffered from multiple primary cancers. The period up to discovery of the second primary cancers was within two years in 78.8% of the patients. Second primary cancers were most frequently observed in the lung, followed by the stomach, colon and rectum. Second primary cancers were found in the lung in 26 patients, 14 of these (53.8%) having squamous cell carcinoma. Multiple primary cancers were noted in 2.8% of 1,210 lung cancer patients who underwent stage I curative operation and 2.4% of the remaining 1,944 cases. As to prognosis, 14 patients survived for more than three years after the second primary cancers were found.

Bone resorption under the influence of parathyroid hormone in vitro.

Parathyroid-induced bone resorption in vitro was observed by phase contrast microscope, 16 mm time-lapse cinemicrograph, light- and electron microscope. These findings led us to the following conclusions. That is, the macrophages seem to be capable of resorbing the bone tissue, and perhaps they may degrade and digest the collagen fibers, while the osteoclasts may resorb the bone minerals. The osteoclasts are destined to extinction after about two to three weeks of life span.
