IL-4 increases Simian immunodeficiency virus replication despite enhanced SIV immune responses in infected rhesus macaques.

It is widely believed that a Th1 type CD4 response is critical for enhancement of CD8 immunity and for controlling HIV-1 infection. Th2 type responses, such as what might be seen in a chronic parasitic infection, would sacrifice cellular immunity and thus benefit the virus at the expense of the host. However, there has been little direct examination of the hypothesis in a primate model system. Accordingly, the simian immunodeficiency virus (SIV) infected rhesus macaque model was used to investigate the impact of immunisation with SIV expressing DNA constructs and co-injection with IL-4 on the SIV specific immunological responses, lymphocyte cell counts, as well as the impact on viral load. IL-4 is a Th2 type cytokine, which enhances antibody production and inhibits a CD4 Th1 phenotype. Rhesus macaques were infected with 10 AID50 of SIVmac239 and treated with 9-[2-(phosphonomethoxy)propyl]adenine (PMPA) 9 weeks post-infection. During PMPA treatment, animals were immunised with plasmids that expressed the SIV proteins, env, rev, gag and pol. In addition, they were immunised with a construct that encoded the gene for IL-4. IL-4 co-immunisation increased the neutralizing antibody titres in this group. Importantly, the viral loads in animals vaccinated with IL-4 expressing plasmid increased during the immunisation regimens despite the higher neutralizing antibody titres. In addition, neutralizing antibodies did not correlate with viral set point prior to PMPA treatment, however, there was a correlation between viral loads and antibody titres following the treatment with PMPA. Antibody titres decreased following the suppression of viral load. Importantly, vaccination in the absence of IL-4 protected CD4 levels without increasing viral load. The data support the hypothesis that inappropriate immune bias toward a Th2 pathway would ultimately enhance disease progression.

Therapeutic immunization of HIV-infected chimpanzees using HIV-1 plasmid antigens and interleukin-12 expressing plasmids.

OBJECTIVE: To assess HIV-1 DNA vaccination and co-immunization with interleukin (IL)-12 and IL-10 as immunotherapy in the HIV-1 infected chimpanzee model system. METHODS: Four chimpanzees that were infected with HIV-1-IIIB for longer than 4 years and remained symptom free were immunized with HIV-1 plasmid vaccines. Two chimpanzees were immunized with DNA plasmids that encoded env/rev, gag/pol along with a plasmid that encoded both chains of human IL-12. A third animal was immunized with HIV-1 DNA vaccine constructs and co-immunized with an IL-10 expressing plasmid. Finally a control animal received the HIV-1 DNA vaccine constructs alone. RESULTS: There was no evidence of systemic toxicity associated with the administration of the DNA vaccines or the cytokine-expressing plasmids. We observed that the IL-12/HIV-1 DNA vaccinated animals had enhanced proliferative responses to multiple HIV-1 antigens at multiple time points. The animal that was co-immunized with HIV-1 and IL-10 did not have any changes in the proliferative responses. Finally, the control chimpanzee demonstrated moderate increases in the proliferative responses to HIV-1 antigens. The animal that received HIV-1 vaccines alone and the animals co-immunized with IL-12 all had declines in viral load over the course of the study, however, the decrease in viral loads were transient in all animals. CONCLUSION: Immunization of HIV-1 infected chimpanzees with DNA based vaccines containing the env, gag and pol genes can transiently boost the env specific proliferative responses. Co-administration of IL-12 expressing plasmids further leads to transient boosting of the proliferative response to the core protein, p24 as well. However, at these doses the impact on viral load is minimal.

Vaccination of seronegative volunteers with a human immunodeficiency virus type 1 env/rev DNA vaccine induces antigen-specific proliferation and lymphocyte production of beta-chemokines.

There is a pressing need to test novel vaccine concepts in an effort to develop an effective vaccine for human immunodeficiency virus (HIV) type 1. A phase I clinical study was done to test the immunogenicity of an HIV env/rev DNA vaccine, which was administered intramuscularly to HIV-1-seronegative persons. Subjects received 3 doses of vaccine at a single concentration (100 or 300 microgram) at 0, 4, 8, and 24 weeks. In at least 1 of multiple assays, the 6 subjects who received the 300-microgram dose had DNA vaccine-induced antigen-specific lymphocyte proliferative responses and antigen-specific production of both interferon-gamma and beta-chemokine. Furthermore, 4 of 5 subjects in the 300 microgram-dose group responded to both the rev and env components of the vaccine. The responses did not persist within inoculated individuals and scored in different individuals at different times in the trial. This study supports that HIV-1 DNA vaccine antigens can stimulate multiple immune responses in vaccine-naive individuals, and it warrants additional studies designed to enhance DNA vaccine immunogenicity.

HIV-1 DNA vaccines and chemokines.

DNA vaccines have a demonstrated ability to induce humoral and cellular immune responses in animal models and humans. The technology, although it dates back to the 1950's, has had an insurgence of interest within the past few years following concurrent research papers. The basic technology is being applied broadly to viral, bacterial and parasitic infections. It has also been demonstrated that genes delivered via plasmid expression vectors result in expression of functional proteins in the inoculated host. Further, injection of plasmids encoding cytokine, chemokine or co-stimulatory molecules, also referred to as immunomodulatory plasmids can lead to the further expansion of this technology to include directed immunology. We have been developing DNA technology specifically with a focus as a vaccine against HIV-1 infection. We report that such vaccines can stimulate immune responses in a variety of relevant animal systems including humoral and cellular responses as well as the production of beta-chemokines. We describe that the beta-chemokines can both modulate the immune response induced by DNA vaccines and be modulated by the DNA vaccines in the murine and chimpanzee models as well as in humans.

[XY gonadal dysgenesis (Swyer syndrome) with gonadoblastoma].

This is a case report of 46 xy gonadal dysgenesis (Swyer-syndrome) with bilateral androgen producing gonadoblastoma in streak gonads in a 15-year-old patient. The presenting features were: hypergonadotrophic hypogonadism, male pseudohermaphroditism and virilisation. A hypoplastic uterus with normal looking Fallopian tubes and bilateral adnexal tumors were detected through laparoscopy. A laparotomy was performed and the streak gonads with bilateral gonadoblastoma were removed. This led to a normalisation of serum testosterone and serum beta-HCG levels and an amelioration of signs of virilisation. Uterus and fallopian tubes were conserved during the operation. A second look laparoscopy 6 months later showed no evidence of recurrent tumor. No mutation were found in the sex-determining gene (SRY) on DNA-screening using SSCP assay.

[Persistent pruritus after hydroxyethyl starch infusions. Retrospective long-term study of 266 cases].

In 266 patients receiving hydroxyethyl starch (HES) for otological indications, we retrospectively analysed the incidence of pruritus and its relationship to clinical and therapeutic parameters. We found that 32% of the patients developed pruritus, which characteristically appeared as pruritic crises. In 55% of pruritic patients onset after the HES medication had already been discontinued was reported. The symptoms persisted on average for 8.8 weeks. Pruritus was generalized, but with a predilection for the trunk and genitalia. Coincidental atopic disease or higher age did not promote pruritus. However, the incidence of pruritus correlated well with the cumulative dosage and also depended on the type and molecular weight of HES given (6% or 10%, mol.wt. 40 or 200 kDa). Light and electron microscopic assessment showed deposits of HES, especially within dermal macrophages and endothelial cells and adjacent to nerve fibres. Pathogenetically a histamine-independent pathway is probably responsible for the induction of pruritus. Hence, classic antihistaminic drugs had no therapeutic effect in our patients.

Does intake of highly demineralized water damage the rat gastrointestinal tract?

Highly demineralized water (hd-water) is used frequently in laboratories and on an industrial scale. However, no systematic data seem available as to whether hd-water has a toxic potential beyond the physical risks of drinking water (drowning, water intoxication). This study investigated the impact of hd-water on function and morphology of the rat's gastrointestinal tract. One group of rats received hd-water together with the usual diet ad libitum for 14 d. A second group was exposed to hd-water after withdrawal of food and water for 24 h. Both experiments had control groups which were treated identically except that hd-water was replaced by tap water. Histology showed no signs of erosion, ulceration or inflammation in the esophagus, stomach or jejunum. Body weights and the uptake of food and water were not significantly different between the hd-water exposures and controls for 14 d. Tissue alkaline phosphatase activity was unaltered, and the mitotic rate in the epithelium of the esophagus and stomach were not different between controls and rats on hd-water. Exposure to hd-water caused no changes in the km or Vmax values for the uptake o of alpha-methyl-D-glucose from the upper jejunum. There findings indicate no impact of hd-water on the function or morphology of the rat gastrointestinal tract. There is no need for additional safety regulations when working with hd-water which go beyond those considered adequate to prevent drowning and water intoxication.

Phytohemagglutinin gene expression during seed development of the runner bean, Phaseolus coccineus.

The expression of phytohemagglutinin (PHA) genes was studied in various tissues during the development of the seeds of Phaseolus coccineus cv. Hammond's Dwarf Scarlet by means of northern hybridization and reverse transcription polymerase chain reaction (RT-PCR). The expression is highly development-dependent, starting in the late stage (cotyledons begin to fill the endosperm cavity, i.e. 17 to 24 days after pollination), and tissue-specific. The highest levels of PHA-mRNA are found in the cotyledons (at a very late stage), much lower levels in the embryo axis. Very low levels could be detected in earlier stages of the endosperm, the integument, the funiculus, and probably also in the embryo suspensor. Some PHA mRNA was found in the cotyledons of dry seeds, indicating the presence of undegraded transcripts in mature seeds.

[Measurement accuracy of oscillatory and whole body plethysmography determination of airway resistance. Study of a mechanical model].

The degree of accuracy of the plethysmographic and oscillatory method in determining respiratory resistance has been examined on a mechanical lung model. At this model different levels of the resistance could be reproducibly adjusted and exactly determined with sensitive measuring instruments. The plethysmographic method allows a precise estimation of the resistance. It was found that the absolute variation of the plethysmographically measured values is not greater than 5%. The Ros pointer scale of the Siregnost FD 5 yields systematically incorrect curve diagrams. In the lower range of the resistance the measured values are to high while the measured results of the resistance become progressively to low with an increasing resistance. The reason is the Ros pointer scale which does not show the real component of the impedance at a phase angle of 0 degree. The values of the real component of the respiratory impedance (Rreal) which yields the Siemens standard set show a great coincidence with the lung model resistance (R(aw)). The coincidence could be even improved by use of electronic data processing. With a computer program developed by us it is possible for the first time to indicate and registrate consecutively individual and average values of the real component (Rrealcomp) and the reactance of the respiratory impedance as well as the phase angle between the alternating pressure delta p and the oscillating flow (V). Thereby the accuracy of measurement is improved and the long winded analysis with the "phase diagram" is not necessary anymore. Further experimental and clinical investigation have to show whether the oscillatory method in the way described above will offer new possibilities for the assessment of the pulmonary function. The phase angle and its course during the respiration cycle is in this connection of special importance as a possible new parameter.

High cell density fermentation of recombinant Escherichia coli expressing human interferon alpha 1.

A defined medium was developed which, by means of a specific fed-batch mode, allows growth of the recombinant Escherichia coli strain TG1 (pBB210) up to a cell density of 60 g dry weight/l. Apart from glucose and aqueous ammonia fed as carbon and nitrogen sources, it was necessary to supply other nutrients or O2-enriched air. Aqueous ammonia also served for pH control. The pO2 level was kept at 20% saturation via closed-loop controls operating the two output variables of stirrer speed and glucose feeding rate. This fed-batch method prevented significant accumulation of acetate and other metabolic by-products. The recombinant E. coli expressed interferon alpha 1 more efficiently at a lower specific growth rate (muPr approximately 0.15 h-1) than at the maximum specific growth rate (mu max = 0.45 h-1). Therefore, fermentation in the batch phase at mu max was only allowed to continue up to a medium cell density. In the succeeding fed-batch phase, the specific growth rate was reduced to muPr by increasing the stirrer speed according to an empirically developed time scale.

[Accuracy of measurement and comparability of anterior rhinomanometry and whole body plethysmography in determination of nasal resistance. Studies with a mechanical model].

The degree of accuracy of anterior rhinomanometry and the plethysmographic method in determining nasal resistance was examined in a mechanical model. The plethysmographic method allows a precise estimation of nasal resistance. It was found that the absolute variation of the plethysmographically measured values is not greater than 5%. Anterior rhinomanometry as described by Bachmann (2) allows a valid estimation of the pressure-flow relation. Here, too, inaccuracy remains within a range of 5% if the values are recorded with an X-Y-plotter. With the system developed by EVG, a microprocessor-controlled analysis is also possible. The flow rate is measured at pressure rates of 75, 150, and 300 Pa. Inconstancies in respiratory flow cause deviations from the correct pressure-flow graph. Should these deviations accidentally coincide with the predefined points of evaluation these erroneously achieved results will be assumed to be representative for the whole graph. This analytical fault has meanwhile been corrected by interpolating the points at the predefined pressure rates. Due to the differences in analytic techniques the methods of determining nasal resistance are not comparable in practice. The authors therefore recommend that, in addition to the usual results of analysis, average value of nasal resistance be determined for each measuring procedure. Today, this can be done without any problem by using microprocessors.

Effect of chelating agents on biliary excretion of arsenic in perfused livers of guinea pigs pretreated with As2O3.

The effect of the dithiols British Anti-Lewisite (Bal), dimercapto-propanesulfonic acid (DMPS), dimercaptosuccinic acid (DMSA) and a new metal binding agent 2,3-bis-(acetylthio)-propanesulfonamide (BAPSA) on the biliary excretion of arsenic in perfused livers of guinea pigs pretreated with As2O3 was investigated. Guinea pigs received As2O3, 2.5 mg/kg sc twice daily for 5 consecutive days. Sixteen hours after the last dose the livers were perfused (35 ml/min) with Krebs-Henseleit buffer with glucose for 80 min. After 50 min of perfusion 0.1 mmol/L or 0.7 mmol/L BAL, DMSA, DMPS, or BAPSA were added to the perfusate and arsenic elimination in the bile and effusate was measured. The total arsenic excretion in control livers between the 50th and 80th min was 6.1% of the total arsenic liver content. After antidote addition (0.1 mmol/L) the excretion increased to 7.9% (DMSA), 9.2% (BAL), 23.9% (BAPSA), and 27.1% (DMPS), respectively. After 0.7 mmol/L of antidote the excretion of arsenic was found to be 19.3% (DMSA), 19.9% (DMSA), 24.0% (BAL), and 43.3% (BAPSA), respectively. The increase resulted mainly from increased biliary excretion. In these experiments BAPSA was significantly more effective in the overall elimination of arsenic than DMSA, DMPS, and BAL. The treatment with chelating agents may cause a substantial shift to fecal elimination by the increase in biliary excretion (BAL less than DMSA less than DMPS less than BAPSA). From the therapeutic view the shift to fecal elimination may have the advantage that the amount of the toxicant which passes the kidney is reduced and thereby also the portion which might be harmful for the organ.

[Use of glass ionomer cement in surgery of the incus-stapedial joint. An initial report of experiences].

Chronic inflammations of the middle ear are characterised by perforation of the tympanic membrane as well as hearing loss, due to disintegration in the incudostapedial joint. Usually the perforation is easily restored by means of available modern procedures; on the other hand, failure of the reconstruction of the ossicular chain, especially of the incudostapedial joint, is not uncommon. Hence, the improvement of hearing capacity is transitory, since later, due to scars, graft lateralisation results in a dislocation of the primarily reconstructed ossicular chain. The patient's hearing capacity is then as poor as it has been before surgery. The aim of this study is to demonstrate the ways of satisfactory reconstruction of the incudostapedial joint by means of glass-ionomer cement: 1. Scar tissue connecting the ossicles in slight defects of the incudostapedial joint can be stabilised by glass-ionomer cement leading to a reasonable sound transmission. 2. If the incus has to be removed in case of a more serious defect, the columella can be properly fixed to the stapes by means of glass-ionomer cement without the risk of scar-induced lateralisation. 3. The fixation of the stapes prosthesis' wire loop to the incus can be effectively achieved by glass-ionomer cement.

[Simultaneous determination of oxygen partial pressure in the scala tympani, electrocochleography and blood pressure values in the guinea pig].

Nineteen guinea pigs were exposed to impulse noise from gunfire (G3 of the Federal German Army, 156 dB peak SPL), 6+6 shots or 12+6 shots, with a 3-s pulse interval. For simultaneous measurements of pO2, cochlea microphonics (CM) and compound action potentials of the auditory nerve (CAP), we used the thin 0.5 microns microcoaxial needle electrode described by Baumgaertl and Luebbers, which was placed through the roundwindow membrane into the scala tympani to a depth of 1000 microns. After exposure to the first 6 or 12 gunshots, the pO2 increased by about 20% of the original values in 12 guinea pigs (63%). In the following 30 min of recovery time the pO2 decreased, stabilized or showed a further decline. There were only 3 animals with a pO2 loss of 70% of the original values. Most animals showed a decline of 25% at the end of the recovery period. In all animals after 6 additional shots, the pO2 only decreased by another 5% of the original values. Amplitudes of CM and CAP were reduced by about 40% of the original values after 6 or 12 shots and by another 20%-24% (CM) and 5%-15% (CAP) after 6 additional shots. The intra-arterial blood pressure in the common carotid artery remained constant. The results are discussed with respect to the well-known morphological damage, subsequent ion imbalance and hypoxia within the cortilymph after exposure to gunfire. These changes are reflected in the loss of CMs and CAPs.
