Gastric stump carcinoma following Billroth II resection for peptic ulcer disease. Comparison with cancer in non-operated stomach.

40 gastric stump carcinomas have been investigated. The results are compared with 72 gastric carcinomas without preceding Billroth II resection. The aim of this study was to contribute to our knowledge about the pathogenesis of gastric stump carcinoma. The mean age of patients with gastric stump carcinoma and with carcinoma without preceding Billroth II resection is identical. The age distribution, too, is similar in both groups. Furthermore the time of the Billroth II resection for benign peptic ulcer shows no correlation with the appearance of the gastric stump carcinoma, which suggests that there is no constant time lag between Billroth II resection and stump carcinoma. The histological investigation of the tumor-free gastric mucosa shows similar alterations in the two groups compared. Intestinal metaplasia and cystic dilatation seems to be more common in the gastric mucosa after Billroth II resection as compared with the non-tumorous mucosa of patients without preceding Billroth II resection. But the grade of nuclear atypia in non-tumorous mucosa is identical in both groups. Interestingly enough, the frequency of the main types of gastric cancer, intestinal carcinoma and diffusely infiltrating carcinoma is very similar. Thus, both the epidemiological and histological data seem to indicate that the pathogenesis of gastric carcinoma is very similar to that of gastric carcinoma without preceding gastric surgery. The Billroth II resection does not seem to be a main factor in pathogenesis, but may have co-factorial significance.

[Treatment of reflux esophagitis with sucralfat].

In a multi-centre placebo-controlled double-blind study, the therapeutic effectiveness of sucralfate (aluminium sucrose sulphate, four times 1 g daily) was tested on 66 out-patients with endoscopically confirmed reflux oesophagitis. After 12 weeks of treatment complete healing occurred in 72% of the sucralfate group, and in a further 14% there was improvement. In the placebo group the healing rate was 40%, with improvement in a further 20%. Serious side-effects did not occur. Sucralfate treatment was especially effective in patients with signs of high-grade inflammation (stage III of Savary and Miller) unsuccessfully treated with other drugs. Contrary to previously available drugs, sucralfate is effective also in lesions caused by alkaline reflux and is thus a useful addition to the hitherto unsatisfactory means of treatment.

[Controlled results of standardized X-ray examination technique (author's transl)].

2000 patients were examined in our hospital within a period of 1 1/2 years using a standardized technique of x-ray examination of the stomach. Of these examinations, the gastroscopic, histological and partly also surgical results are now available from 241 patients. Hypotonic double-contrast technique, a highly dense, low-viscosity barium preparation and standardized documentation by means of 12 to 14 images taken with a 100 mm camera, have proved successful. The article reports on the accuracy of the x-ray examination based on these 241 results.

[Endoscopic papillotomy in Austria].

In 5 gastroenterologic centres in Austria 383 EPT were performed at a total of 10031 ERCP until July 1980. In 350 cases this procedure was successful (91.3%). The most frequent indication was a choledocholithiasis in 300 cases (85.7%). Complete disappearance of gall stones was achieved in 94%. Complications happened in 14 patients (4%), in 4 patients (12%) surgical procedures were necessary consequently. The death rate was 0.6% (2 patients). These favorable results and the low rates of complications and letality of this procedure encourage for the more frequent use of this method for the treatment of choledocholithiasis also in Austria in capable and experienced hands. In view of reducing complications the limitation of the EPT to few specialised centres is demanded.

Ampulla of Vater

Carbenoxolone patients with gastric ulcers. A double-blind trial.

Carbenoxolone was compared with placebo in a randomised double-blind study involving 40 patients with gastric ulcer (20 each in the treated and placebo groups). The evaluation was based on eight variables. The endoscopic findings on completion of 4 or 6 weeks of treatment, revealed greater improvement in response to carbenoxolone than to placebo. The difference was statistically significant at the previously fixed level of significance of p less than 0.05. The plasma potassium level fell significantly in the treated group but the actual reduction of 7% was slight. No statistically significant differences were measured in any of the other variables, i.e. plasma sodium, SGOT, SGPT, systolic and diastolic blood pressure, bodyweight, or oedema formation.
