[Psychological aspects of erectile dysfunction].

It is widely accepted opinion that male sexuality comprises more than an erected penis. This fact should be considered in the management of erectile dysfunction. Therefore, the valuation of sexual disturbances have to be essentially included in the general examination programme of erectile impotence. Several causes are known to contribute to an erectile dysfunction. The anamnesis can be taken as an important tool for classification of the erectile dysfunction. The following questions are helpful in the clinical practice: erectile dysfunction during sexual intercourse only but also during masturbation; spontaneous erections; dependence of the erectile impotence on special situations, partners and events; questions for somatic and psychosocial risks. Sexual therapy represents a complex intervention will be aim of improvement of the functional disturbances in consideration of the general risks for sexual impairments.

Erectile Dysfunction↗

[Correlation between chronic urticaria and depression/somatization disorder].

In a multicenter study, 100 patients with chronic urticaria were examined with a standardized personality test (Giessen test), a standardized symptom questionnaire (Giessener Beschwerdebogen) and a specially developed questionnaire concerning symptoms, history and behaviour during symptomatic periods. Almost one third of the patients showed elevated scores both for depression and for symptoms that are often associated with depression. It therefore seems worth-while examining such persons more specifically and possibly treating them by psychosomatic methods. The present results do not allow the classification of chronic urticaria as a somatization disorder. Since one-third of the patients had symptoms of depression, combined dermatological and psychosomatic approach may make it possible to offer them an appropriately targeted treatment.

[Effect of hypnosis and autogenic training on acral circulation and coping with the illness in patients with progressive scleroderma].

In 12 patients with systemic sclerosis the influence of hypnosis and autogenic training on the acral blood circulation and the coping with the disease was investigated in a pilot study. In the first step significant increases in the skin temperature of the finger (mean +/- SD: 3.9 +/- 1.2 degrees C) could be found after relaxation hypnosis. In the second step six patients (study group) gained experience with autogenic training. The other six patients served as control group. In the study group, the skin temperature of the fingers (short-term effect) was significantly higher than in the control group (1.9 +/- 1.0 degrees C). Long-term effects of the autogenic training (mean acral rewarning time, duration and course of the Raynaud attacks, acral lesions of the hands, psychosomatic status of complaints, type of relation to the disease as precondition for coping with the disease) were not found within the relatively short follow-up period of 4 months. Two patients, however, reported that they could shorten the duration of Raynaud attacks by autogenic training. In our patients heterogenicity and an increased score of multiple psychosomatic complaints were registered at the outset. As far as the type of relation to the disease is concerned, the patients could be assessed as almost adapted. Hypnosis and autogenic training can be recommended as complementary therapy in systemic sclerosis.

[Substance P and vasoactive intestinal peptide in patients with progressive scleroderma. Determination of plasma level before and after autogenic training].

In 12 patients suffering from systemic sclerosis (SSc) the influence of autogenic training on the plasma level of the neuropeptides substance P and vasoactive intestinal peptide (VIP) was studied. Compared with healthy controls the SSc patients exhibited significantly elevated levels of substance P (mean +/- SD: 7.1 +/- 3.2 pmol/l vs 1.6 +/- 1.6 pmol/l). Apart from variations the VIP plasma concentration did not significantly differ from that in healthy controls (mean +/- SD 10.7 +/- 7.1 pmol/l versus 12.0 +/- 5.3 pmol/l). Autogenic training did not bring about any significant changes in the plasma levels of neuropeptides.

[Psychosomatic dermatology].

An appreciative cooperation among dermatology, clinical psychology and psychology and psychiatry is necessary and useful. By selected facts interactions between central and autonomous nervous system on one side and the immune system on the other side were described. As far as the neutrocutaneous interrelations are concerned the cutaneous stimulation appears to be an important factor for the physical and physiologic development. In addition, the skin communicates to the environment and other people. Therefore, it plays a role in the social integration. Psychiatric diseases can affect the skin (e.g. delusions of parasitosis in schizophrenia). On the other hand primary skin diseases such as psoriasis, atopic dermatitis, acune vulgaris, chronic idiopathic urticaria and alopecia areata may induce psychologic features. The characteristics of the personalities of dermatological patients are discussed. The psychodiagnostics (personal interview, questionaires) is followed by psychotherapeutic procedures such as personal consultations, treatment in groups, hypnosis and autogenous training. Our experiences in the psychodiagnostics and treatment are briefly reported. Finally, psychotropic drugs-antipsychotic, anti-depressant, antianxiety, and hypnotic agents - may be useful as an adjunct in the management of dermatologic disorders, if applied under precautious indications.

[Development of partner relations and of the child after successful therapeutic donor insemination].

The psychological development of couples and children was examined after successful therapeutic insemination by donor (TID). 222 couples were investigated by sending questionnaires to the families. 122 (55%) couples fulfilled our requests. The answers suggested that there was no deviation from the behaviour of the control groups concerning attitudes towards pregnancy and delivery, actual interrelation between wife and husband, degree of neurosis of the parents and children. The conclusion could be drawn that there was no need for more restriction on the TID provided that a good psychological diagnostic regarding the special TID-problems was performed before the insemination treatment.

[Partnership status in preparation for therapeutic donor insemination].

An investigation is presented which inquiries the psychological problems of sterile couples after diagnosing of male infertility. The interpretation of conversation topics accomplished by 460 couples suggests that coping with infertility by the couple, the character of the desire for a child, the level of stress tolerance, the ability to cooperation and the agreement to compromise determine the psychological situation of a couple infertile by male. These facts should be taken into consideration in regard of the therapeutic insemination by donor.
