[Sonographic detection of embolic masses in the ventricular passage].

Using echocardiography, we succeeded in identifying thromboembolic masses in the heart of 5 patients suffering from massive pulmonary or arterial embolism. In another case we found a thrombus in the right ventricular outflow tract. Premonitorial pulmonary embolism preceded in all cases. One thrombus in arterial embolism was found by tm-echocardiography, five thrombi by sector-echocardiography. The examinations were performed after acute worsening of the clinical condition. In four patients, death resulted from the embolism, whereas two survived, one old patient without thrombolytic or operative therapy.

[Results of sonographic diagnosis of choledochal calculi].

To evaluate sonographic accuracy in the diagnosis of choledochal calculi 44 patients were examined prospectively with a mechanical sector scanner before endoscopic retrograde cholangiography (ERC). Calculi with a diameter of more than 1.5 cm could be detected in all cases. Smaller stones (less than 1 cm) were missed in 50% of cases. To quantitate sensitivity in daily clinical practice, the data of 229 patients, suspected to have common bile duct stones, were analysed retrospectively. The overall accuracy was 76%, 90% when an experienced sonographer performed the examination and 47% if the examiner was less experienced. From these data it is concluded that the sonographic detection of choledochal calculi is influenced by the examination technique, experience of the examiner and diameter of the stones.

[Sonographic detection and clinical studies in the spontaneous passage of bile duct calculi].

Spontaneous transpapillary stone passage was found in 16 of 84 cases with sonographically detected common bile duct stones. Stone passage was confirmed by ultrasound and retrograde endoscopic cholangiography (ERC) as well as by intraoperative cholangiography and also by clinical observation. Sonographic diagnosis was demonstrated by incontestably safe documentation. 15 of 16 patients with transpapillary stone passage had right upper abdominal pain, three patients an attack of acute pancreatitis, whereas in 5 cases hyperamylasaemia was found. Decreasing levels of bilirubin , alkaline phosphatase and gamma-GT are of minor value in diagnosing a stone passage. Despite a new and more successful sonographic technique in detecting common bile duct stones, ERC remains the standard diagnostic procedure.

[Sonographic detection of fistulas in Crohn disease].

The sonographic detection of fistulas in Crohn's disease has been almost impossible up to now and had therefore not been attempted. Two findings of an enterocutaneous and a blind-ending colohepatic fistula are demonstrated. Probably sonographic diagnosis of fistulas is better than has been believed so far, due to the high contrast of tiny gas bubbles; the "compression technique" to bypass meteoristic intestinal loops, which is been increasingly employed, and the improved physical resolution that allows to distinguish between different layers of the intestinal wall, are further contributing factors to the increasing use of sonography in this category of diagnostics.

Rate control with an external SO2 closed loop system.

In 20 volunteers (mean age 35.5 y) and 12 pacemaker patients (mean age 68.7 y), central venous oxygen saturation (SO2) was monitored continuously by means of an optical sensor integrated in an external transvenous pacing lead placed in the right ventricular cavity. From the SO2 signal recorded at rest and during various modalities of exercise, an algorithm for controlling pacing rate of an external pacing system was developed. An open loop system was used in the volunteers, allowing the comparison of the computed pacing rate with the individual intrinsic heart rate. There was an excellent correlation between the two frequencies as far as the dynamic characteristics and the steady state relationship were concerned. In five pacemaker patients who were stimulated via the external lead, a closed loop control of pacing rate was used. In one patient with a DDD pacemaker implanted for third degree AV-block, the rate response of the SO2 driven pacemaker was well in accordance with the rate attained with the implanted atrial triggered system. With both pacing modes, exercise capacity as determined on a symptom limited treadmill test was identical. In four patients (3 AV block III, 1 bradyarrhythmia) an improvement in exercise tolerance up to 65 percent could be demonstrated with the rate responsive pacing mode. In all patients, it could be shown that an autoregulating pacemaker system with SO2 is an open possibility.

[Groove pancreatitis--its pathological anatomy and sonographic findings].

Groove pancreatitis is a special type of segmental pancreatitis. Its morphological aspects with participation of the peripheral dorsal and cranial parts of the pancreatic head, the pancreatic soft tissue, and the duodenal wall with frequent narrowing of the distal common bile duct and suprapapillar stenosis of the duodenum can be detected by means of sonography. If this type of seqmental pancreatitis is known, the suspicion of a carcinoma of the pancreatic head can be removed.

Solvent abuse associated pulmonary abnormalities.

Organic solvent inhalation leading to intoxication has become an important medical problem. Pulmonary function studies were performed on 42 young (ages 11-31) solvent inhalers and on 20 controls (ages 10-26). Ventilatory studies were normal in the majority, but residual volumes of inhalers were significantly higher (136.26% +/- 32.64% S.C.) than in controls (104% +/- 27.78% S.C.)--p less than 0.001. Lung tissue obtained at autopsy, from three known solvent inhalers was found to contain microscopic abnormalities similar to those seen in experimental panlobular emphysema and it is assumed these changes are consistent with increased residual volumes.

Dorsal root entry zone lesions for the control of deafferentation pain: experiences in ten patients.

Experiences with radiofrequency lesions of the dorsal root entry zone in 10 patients are reported. All of these patients suffered from central (deafferentation) pain. The early postoperative results were poor in the 2 patients with traumatic paraplegia and good in all 8 patients with pain in the cervical segments. Two patients treated with cervical DREZ lesions died. In 2 of the remaining 6 patients with cervical lesions, pain recurred. Four had a good result up to 30 months after operation.

Solvent abuse associated cortical atrophy.

Eleven of 42 toluene inhalers were evaluated with computed tomography scans because of neurologic abnormalities. Six of the 11 were found to have cerebral cortical atrophy. In addition, two of the six had cerebellar atrophy. All six had been exposed to toluene for at least 10 years. This study suggests that atrophy of the central nervous system may occur in chronic toluene abusers.

[Sonographic detection of free air in the abdominal cavity].

Diagnosis via x-ray film had so far been the only way to prove the existence of free air in the abdominal cavity and hence the presence of gastrointestinal perforation. However, it is possible to establish the existence of free air in the abdomen sonographically provided a suitable examination method is employed. A characteristic feature is represented by pronounced pre-hepatic echoes with multiple echoes or sound shadow phenomenon. Experimentally it was possible to identify 1 ml air in the abdomen of an ascites patient both by x-ray examination and by sonography with equal certainty. The clinical usefulness of the ultrasound method was scrutinized. Of 10 patients with gastrointestinal perforations, 9 were recognized by sonography, including roentgen-negative cases; in one case only, sonography yielded a false negative result. Sonographic differential diagnosis is more versatile and differentiation can be practised by the highly experienced investigator only. The proof of the existence of free air in the abdomen represents a considerable extension of the scope of sonographic emergency-case diagnosis.

Policy choices available in genetic counseling for people at-risk for Huntington's disease.

Staff on 94 wards in 78 Veterans Administration Medical Centers caring for Huntington's disease patients were interviewed by telephone to determine their policy, practice, and attitude regarding counseling HD families. Considerable variability among wards within a given hospital and among different hospital was reported. Presentation of genetic information was found to be significantly associated by statistical tests with existence of some kind of ward counseling policy, treatment on a neurology ward, active seeking-out of relatives, construction of pedigree charts, and specific staff designated to provide such counseling. Genetic discussions with families of HD patients and at-risk kindreds raise some sensitive ethical questions, notably the vigorous denial of disease in these families, the high suicide rate of those at-risk, and consideration of patients' rights regarding privacy of medical data. A consistent genetic information-giving policy, at least among wards of each hospital and possibly among all hospitals within a large hospital system, is recommended as a logical and desirable goal.
