Regulation of transcription of the human erythropoietin receptor gene by proteins binding to GATA-1 and Sp1 motifs.

Erythropoietin (Epo), the primary regulator of the production of erythroid cells, acts by binding to a cell surface receptor (EpoR) on erythroid progenitors. We used deletion analysis and transfection assays with reporter gene constructs to examine the transcription control elements in the 5' flanking region of the human EpoR gene. In erythroid cells most of the transcription activity was contained in a 150 bp promoter fragment with binding sites for transcription factors AP2, Sp1 and the erythroid-specific GATA-1. The 150 bp hEpoR promoter exhibited high and low activity in erythroid OCIM1 and K562 cells, respectively, reflecting the high and low levels of constitutive hEpoR expression. The GATA-1 and Sp1 binding sites in this promoter lacking a TATA sequence were necessary for a high level of transcription activation. Protein-DNA binding studies suggested that Sp1 and two other CCGCCC binding proteins from erythroid and non-erythroid cells could bind to the Sp1 binding motif. By increasing GATA-1 levels via co-transfection, we were able to transactivate the hEpoR promoter in K562 cells and non-erythroid cells, but not in the highly active OCIM1 cells, although GATA-1 mRNA levels were comparable in OCIM1 and K562. Interestingly, when we mutated the Sp1 site, resulting in a marked decrease in hEpoR promoter activity, we could restore transactivation by increasing GATA-1 levels in OCIM1 cells. These data suggest that while GATA-1 can transactivate the EpoR promoter, the level of hEpoR gene expression does not depend on GATA-1 alone. Rather, hEpoR transcription activity depends on coordination between Sp1 and GATA-1 with other cell-specific factors, including possibly other Sp1-like binding proteins, to provide high level, tissue-specific expression.

Circulating leukocyte counts, activation, and degranulation in Dahl hypertensive rats.

Previous evidence has shown that rats with spontaneous hypertension have on average about twice as many circulating leukocytes in comparison with their normotensive counterparts, the Wistar-Kyoto rats. Since such high levels of leukocytes may increase the risk for vascular complications for hypertensive animals, it is useful to ascertain whether a comparable derangement is present in other forms of hypertension. The present study deals with the properties of the circulating leukocytes in rats exhibiting another form of experimental hypertension; Dahl salt-sensitive (Dahl-S) hypertensive rats were compared with Dahl salt-resistant (Dahl-R) control rats. Measurements were performed to determine the following: circulating hematocrit levels, leukocyte counts, differential counts, number of activated leukocytes (by means of nitro blue tetrazolium [NBT] reduction), leukocyte adhesion in vitro and neutrophil CD-18 expression, alkaline phosphatase activity in individual neutrophils and in the plasma, and myeloperoxidase activity in neutrophils. The experimental cohort consisted of Dahl-S and Dahl-R rats maintained for a 6-week period on a 6% NaCl diet. The results show a highly significant elevation in the number of total leukocytes, neutrophil and monocyte counts, and NBT-positive neutrophils and monocytes in Dahl-S but not Dahl-R rats. There was a significant loss of alkaline phosphatase and myeloperoxidase activity in the neutrophils of the salt-treated Dahl-S rats but not in the neutrophils of the untreated Dahl-S or Dahl-R rats. No significant differences were found in neutrophil adhesion under in vitro test conditions between the two strains maintained on the salt diet.(ABSTRACT TRUNCATED AT 250 WORDS)

[Overexpression of p53 in transitional cell carcinoma of the ovary].

To determine the prevalence of p53 abnormal protein expression in transitional cell carcinoma of the ovary in Peking Union Medical College hospital, and to analyse the correlation between the overexpression of p53 protein and prognostic parameters of transitional cell carcinoma of the ovary, thirty-two transitional cell carcinoma of the ovary and 20 normal ovaries were analysed. Detection of overexpression of p53 protein was carried out by immunohistochemical staining (IHS) using the monoclonal antibody DO-1 on paraffin sections. 41% of 32 samples of transitional cell carcinoma of the ovary showed positive IHS with DO-1, whereas only 5% of 20 normal ovaries showed weakly positive cytoplasmic staining (P < 0.01). p53 overexpression did not correlate with age, CA125 levels, histological grade and lymph node metastasis, but patients with overexpression of p53 had a poor prognosis. Overexpression of p53 is a more common event in transitional cell carcinoma of the ovary. Further studies are warranted to clarify the role of p53 in transitional cell carcinoma of the ovary.

[Overexpression of C-erbB3 in transitional cell carcinoma of the ovary].

OBJECTIVE: To determine the prevalence of C-erbB3 abnormal protein expression in transitional cell carcinoma of the ovary in our hospital and to analyse the correlation between the overexpression of C-erbB3 protein and prognostic parameters of transitional cell carcinoma of the ovary. METHODS: Thirty-two transitional cell carcinoma of the ovary and 20 normal ovaries were studied for overexpression of C-erbB3 protein by immunohistochemical staining (IHS), using the monoclonal antibody RJT2 on paraffin sections. RESULTS: Forty seven percent of 32 samples of transitional cell carcinoma of the ovary showed positive IHS with RJT2, and 39% of 20 normal ovaries showed positive cytoplasmic and membrane staining (P > 0.05). C-erbB3 overexpression did not correlate with age, CA125 levels, histological grade and lymph node metastases, but patients with overexpression of C-erbB3 had a poorer prognosis. CONCLUSION: Overexpression of C-erbB3 is a common event in transitional cell carcinoma of ovary. Further studies are warranted to clarify the role of C-erbB3 in transitional cell carcinoma of the ovary.

Estrogen and progesterone receptors in normal cervix and primary cervical carcinoma.

Estrogen and progesterone receptors (ER, PR) were measured in 21 specimens of cervical cancer and in 17 specimens of normal cervix by monoclonal enzyme immunoassay (EIA). In normal cervix, 88.2% of specimens were ER-positive ( > 15 fmol/mg protein), 74.5% were PR-positive ( > 15 fmol/mg protein) and 74.5% were both ER- and PR-positive. In cervical cancer, 66.7% were ER-positive, 42.9% were PR-positive and 38.1% were both ER- and PR- positive. There was no significant difference in ER status between the normal cervix and cervical cancer (P > 0.05), but PR status and levels in normal cervix were significantly higher than those in cervical carcinoma (P < 0.05). ER levels in squamous cell carcinoma was not correlated to the tumor stage, histologic grade and menopausal status. PR levels in premenopausal patients with squamous cell carcinoma were significantly higher than those in postmenopausal patients (P < 0.01). Adenocarcinoma of the cervix contained significantly more ER and PR than squamous cell carcinoma (P < 0.01, P < 0.05). In addition, serum E2 level was also assayed in 21 patients with cervical cancer. There was significant difference in E2 levels between the premenopausal and postmenopausal patients (P < 0.01). Patients were stratified according to E2 levels, a significant difference in PR levels and in the ratio of PR/ER was noted (P < 0.05, P < 0.01).

[Selective arterial embolization in the management of hemoperitoneum caused by trophoblastic disease].

In the field of gynecologic oncology, surgical intervention has been the traditional management of hemoperitoneum. Recently selective arterial embolization has been reported to effectively control gastrointestinal bleeding caused by peptic ulcer and postoperative abdominal or retroperitoneal bleeding associated with gynecologic malignancy. This study reports the use of selective arterial embolization in the management of hemoperitoneum caused by trophoblastic disease. Analysis of 4 cases provides valuable information which enhances the safety and efficiency of this procedure. Results suggest that selective arterial embolization should be considered in the management of hemoperitoneum caused by trophoblastic disease.

Ovarian sex cord tumor with annular tubules: a report of six cases.

Six patients suffering from ovarian sex cord tumor with annular tubules (SCTAT) were reported in this article with special reference to the clinical features, histological characteristics, sex hormone profile, and management of disease. SCTAT was documented to be a estrogen-progesterone-secreting tumor based on the observations of glandular atrophy and decidual change of stroma in the endometrium and assays of steroid hormone. Menometrorrhagia followed by persistent amenorrhea and pelvic mass were presented as important clinical features. This tumor was considered as a tumor with low-grade malignancy, and retroperitoneal lymphatic metastasis was thought to be an important pathway of spread. Unilateral salpingo-oophorectomy together with ipsilateral pelvic and para-aortic lymphadenectomy were suggested as an effective treatment for SCTAT. Radiotherapy can be used for local recurrence and distant metastases.

[Double primary carcinoma of the uterine corpus and ovary].

This article presents 19 patients with double primary carcinoma of the uterine corpus and ovary diagnosed and treated at Queen Mary Hospital, University of Hong Kong in last ten years. 14 patients had typical endometrial adenocarcinoma and endometrioid carcinoma of the ovary (group A), 5 had non-endometrioid carcinoma in the uterine corpus and/or ovary (group B). There is no significant difference in survival rate in group A and B (78.6% and 80%, respectively, mean follow-up 42 months), but the total survival rate in our series is 78%, which is equal to stage I ovarian cancer. The tumor grade and positive abdominal washing were proven to be statistically significant indicators of the poor prognosis (P < 0.05). The result reveals that the prognosis of double primary carcinoma of the uterine corpus and ovary is better and it is necessary to distinguish double primary carcinoma of the uterine corpus and ovary from stage II ovarian cancer and stage III endometrial carcinoma.

The bacteriology of cholelithiasis--China versus Japan.

Cholelithiasis differs considerably from area to area in the world. Calcium bilirubinate stones or brown pigment stones are said to predominate in the Orient, however, this situation may differ within the Orient. In order to compare cases in China and Japan, 102 consecutive cases of cholelithiasis operated on in Shenyang, China were analyzed for the composition of gallstones and bacterial species isolated from bile in relation to the location and composition of gallstones. In Shenyang, calcium bilirubinate stones predominated, occurring in 49.0 per cent of the cholelithiasis cases. This was much higher than in Japan, which had 17.5 per cent. The incidence of bacteria was also very high, ranging from 20 to 96 per cent, with an average of 66.7 per cent depending on the kind of gallstone present. Bacterial species possessing beta-glucuronidase activity were present in nearly all the cases of calcium bilirubinate stones (92.0 per cent). The incidence of bacteria with beta-glucuronidase activity especially of E. coli was much higher than in Japan (50.8 per cent versus 21.8 per cent) in concordance with the higher incidence of calcium bilirubinate stones in China (49.0 versus 17.5 per cent).

Leukocyte counts and activation in spontaneously hypertensive and normotensive rats.

The etiology for the progressive organ injury in hypertension is largely speculative. Recent studies have shown that leukocytes play a key role in several cardiovascular diseases. As an initial step toward investigating the role of leukocytes in hypertension, we measured leukocyte counts and spontaneous activation of granulocytes of freshly drawn unseparated blood samples in spontaneously hypertensive rats and in their normotensive counterpart, Wistar-Kyoto rats. The animals were derived from one breeder in the United States and from two breeders in Europe. Total leukocyte counts in young, mature, and old hypertensive rats were 50-100% above the controls. The number of granulocytes in mature and old spontaneously hypertensive rats in more than 100% elevated compared with control rats. In young hypertensive rats the mean granulocyte count was only slightly elevated. The number of spontaneously activated granulocytes, as detected by the nitroblue tetrazolium reduction, increases with age in both species; in mature spontaneously hypertensive rats, it is more than 300% above the values in the controls. Furthermore, in mature hypertensive rats the number of monocytes, activated monocytes, and the lymphocyte count are also significantly elevated over the values in the normotensive controls. It is proposed that these elevated leukocyte counts may constitute an enhanced risk for organ injury in the spontaneously hypertensive rat.

[Merits, demerits and methods of preoperative biliary drainage for malignant obstructive jaundice].

One hundred twenty-four patients with malignant obstructive jaundice undergoing Whipple's procedure were divided into two groups. Group I, 72 patients with an average preoperative serum bilirubin level of 13.3 mg/dl underwent pancreaticoduodenectomy after the level dropped to 5.88 mg/dl by preoperative external biliary drainage (21 patients with PTCD and 51 with cholecystostomy). Group II, 52 patients with a mean bilirubin level of 7.18 mg/dl received Whipple' procedure without preoperative drainage. There was no significant difference in mortality and morbidity between the two groups. We believed that biliary drainage is essential to patients with severe jaundice, otherwise the mortality and morbidity would be much higher AS to method of biliary drainage, we recommend cholecystostomy in hands not skillful with PTCD.

[Ovarian sex cord stromal tumors with annular tubules: a report of 6 cases].

Six patients with ovarian sex cord stromal tumor with annular tubules (SCTAT) were diagnosed and treated from 1968 through 1988, accounting for 6% of a total of 99 cases of ovarian sex cord stromal tumors admitted during the same period. Of these, 3 were operated on for the first time and 3 were recurrent cases referred from other hospitals. The menstrual disturbance was characterized by menometrorrhagia followed by sustained amenorrhea. Glandular atrophy and decidual change of stromal were observed in 3 of the 4 endometrial samples available for histologic examination. Serum hormone assays carried out in 2 cases revealed elevated levels of both estradiol and progesterone. The primary tumor was unilateral in 5 cases and bilateral in one the recurrent tumors were exclusively retroperitoneal and ipsilateral to the primary ovarian tumors. In these cases, lymphatic metastasis was also observed in the ipsilateral pelvic lymph nodes. Complete and sustained remission has been achieved in all 3 patients receiving primary surgery by unilateral salpingo-oophorectomy. After removal of the metastatic tumors in 3 recurrent cases, 2 have survived with residual diseases for 2 and 9 years respectively and one died 4 years after the secondary surgery.

Prevalence of the 'combination syndrome' among denture patients.

The prevalence of symptoms associated with "combination syndrome" were documented in 150 maxillary complete denture wearers. The five alveolar ridge changes that are most consequential to denture wearing and most difficult to correct surgically were found in 7% of the population studied. However, these changes were found in 24% of patients who have natural mandibular anterior teeth opposing complete maxillary dentures. This rate did not differ significantly between patients who do and do not wear a mandibular removable partial denture.
