Sudden hearing loss associated with interferon.

With the increasing long-term use of interferon, several new adverse effects have been recognised. We have prospectively assessed auditory function in 49 patients receiving interferon, after we saw a case of sudden sensorineural hearing loss during interferon therapy. Auditory disability (tinnitus, hearing loss, or both) occurred in 22 patients (45%) during treatment with audiometry-documented sensorineural hearing loss in 18 (37%). The auditory disability often developed in the late stage of treatment and resolved in all patients within 7-14 days after discontinuation of interferon.

Structural studies on the chondroitinase ABC-resistant sulfated tetrasaccharides isolated from various chondroitin sulfate isomers.

Various commercially available chondroitin sulfates, including an A isomer from whale cartilage, C and D isomers from shark cartilage, and an E isomer from squid cartilage, were exhaustively digested with a commercial highly purified Proteus vulgaris chondroitinase ABC. Gel chromatography of all digests yielded a disaccharide and an oligosaccharide fraction which was resistant to the enzyme digestion and which accounts for 20-31 mol% of the produced total oligosaccharides. Variably sulfated tetrasaccharides were isolated from the oligosaccharide fraction of each chondroitin sulfate isomer by HPLC, then characterized chemically and enzymatically. One disulfated and three trisulfated components were also characterized by 500-MHz one- and two-dimensional 1H NMR spectroscopy. The structures of one tetrasulfated, four trisulfated, and five disulfated tetrasaccharides with the common core structure, alpha-L-delta 4,5HexpA-(1-->3)-beta-D-GalpNAc-(1-->4)-beta-D-GlcpA-(1-->3) -D-GalpNAc, were determined. All isolated tetrasaccharides were resistant to the highly purified enzyme, but susceptible to the conventional, commercial chondroitinase ABC. The former was also inactive towards alpha-L-delta 4,5HexpA-(1-->3)-beta-D-GalpNAc-(1-->4)-beta-D-GlcpA-(1-->3) -D-GalpNAc isolated from chondroitin, beta-D-GlcpA-(1-->3)-beta-D-GlcpNAc-(1-->4)-beta-D-GlcpA-(1- ->3)-D-GlcpNAc from hyaluronan, and alpha-L-delta 4,5HexpA-(1-->3)-beta-D-GalpNAc4SO3(-)-(1-->4)-alpha-L-Id opA-(1-->3)-D- GalpNAc4SO3- from dermatan sulfate. These results indicate that, unlike the conventional enzyme, highly purified chondroitinase ABC cannot degrade tetrasaccharides irrespective of their sulfation profiles. The enzymatic action is size-dependent.

Importance of cervical muscles in galvanic body sway test.

Retro-auricular galvanic stimulation in Romberg's posture elicits body sway which consists of an initial response (IR) and a deviation response (DR). Generally, anodal stimulation of the right side elicits IR to the left and DR to the right. We measured the deviation response (VDR) in various conditions in normal subjects. There was no significant difference between mean VDR in the standing posture weighted on the left foot (0.49 +/- 0.17 cm) and on the right foot (0.39 +/- 0.20 cm). Stimulation in the squatting posture also elicited IR to the left and DR to the right. Stimulation of the right side produced DR backward and to the right in the condition of the head facing to the right, and forward and to the right in the condition of the head facing to the left. The same body sway response as the after galvanic stimulation was also induced by non-galvanic maneuvers like inclining the head or moving the finger. Anodal stimulation in the sitting posture elicited slight but apparent head inclination to the stimulated side. These results suggest that cervical muscles play an important role in the galvanic body sway test (GBST), viz. IR and DR seemed to be secondarily produced by changes in the muscular tension of the neck.

A study of right unilateral spatial neglect in left hemispheric lesions: the difference between right-handed and non-right-handed post-stroke patients.

We report 20 cases of right unilateral spatial neglect caused by lesions in the left cerebral hemisphere. Differences in neuropsychological symptoms and lesions sites are discussed in connection with handedness. Of the right-handed patients, 6 had severe aphasia, 4 had Gerstmann's syndrome, and 1 had pure agraphia, but unilateral spatial neglect in these cases disappeared after a number of months. Six of the non-right-handed patients had moderate-to-severe aphasia, while the other 3 cases had no aphasia at all. Eight of the 9 cases in this group continued to have right unilateral spatial neglect for more than 6 months. Lesion site as determined by CT differed as to hemisphere, but all fell into the common area previously mentioned in connection with such disorders: i.e., the temporal, parietal and occipital lobes.

["Left unilateral agraphia with right hemiparesis" after occlusion of the left middle cerebral artery].

This paper presented a case of a right-handed male who showed a right hemiplegia without aphasia and apraxia. He lost the ability to write with the left hand. A 56-year-old right-handed man, who had a daughter of left-handedness, was sent to our hospital with a homonymous hemianopsia, facial weakness, spastic hemiparesis and sensory disturbance in the right side. CT scan revealed an infarction in the territory of the left middle cerebral artery. On a month after the onset, he was alert and oriented. His speech was normal and verbal comprehension was intact. Although he neglected the right side of the page, he could read and comprehend it correctly. In contrast with his normal abilities to speak, comprehend, and read, difficulties in writing were prominent. Spontaneous writing with the left hand was extremely poor, and he even had difficulty writing his own name. His dictation was also poor, but his writing improved with copying letters. Agraphia had seen even after USN was recovered. Analysis of this case suggested the presence of the dominance for speech, comprehension, and praxis in the intact right hemisphere, and writing center in the damaged left hemisphere.

Positional nystagmus due to alteration of the specific gravity in the labyrinth.

Horizontal positional nystagmus having characteristics similar to ice water caloric nystagmus was induced by injecting heavy water into the rabbit middle ear cavity or facial nerve through the stylomastoid foramen. Furthermore, with general administration of glycerol to normal subjects, horizontal positional nystagmus as with general administration of heavy water to experimental animals was observed. It is possible that alteration of the specific gravity arising from infiltration of heavy water or glycerol into the labyrinth accounts for the manifestation of positional nystagmus. These studies on positional nystagmus may be one of the means to elucidate the mechanism of caloric nystagmus and to examine the function of the human blood-labyrinth-barrier.

[Experimental positional nystagmus induced by argon laser irradiation to the oval window].

Experimental positional nystagmus (EPN) similar to benign paroxysmal positional nystagmus (BPPN) was induced in 8 of 20 rabbits by argon laser irradiation to the footplate. The power of the argon beam used was 2.0 watts and irradiation time was 1 or 2 sec. EPN and BPPN showed the following similarities maintained (1) When the head was maintained in the affected ear down position and the head positioned from the sitting to the supine position nystagmus directed to the affected ear or vertical nystagmus corresponding to rotatory nystagmus in man were provoked. (2) Delayed onset for provocation of nystagmus. (3) Limited duration. (4) Fatigability. (5) The direction of nystagmus was reversed when the head returned to the initial position (in two rabbits). Histological examination of the temporal bones showed separation of otoconia from the otolithic membrane of the saccule and bleeding in the saccule in both conditions. No lesions in the utricle and semicircular canals were evident. It was assumed that ablation of otoconia of the saccule causes this experimental positional nystagmus.

Effect of increasing dietary energy on hepatic lipogenesis in growing chicks. I. Increasing energy by carbohydrate supplementation.

The effect of the increasing the energy content of the diet by supplementation with increasing amounts of carbohydrate on hepatic lipogenesis and the activities of associated enzymes of liver was examined in force-fed growing chicks. Hepatic lipogenesis and the activities of nicotinamide adenine dinucleotide phosphate (NADP-MDH)-malate dehydrogenase (EC 1.1.1.40) and citrate cleavage enzyme (EC 4.1.3.8) in liver were significantly increased as dietary metabolizable energy (ME) increased by supplementation of carbohydrate. Triglyceride content in liver significantly increased as dietary ME increased by supplementation of carbohydrate. The results demonstrated a positive correlation between hepatic fatty acid synthesis and the activities of NADP-malate dehydrogenase and citrate cleavage enzyme in liver.

Effect of increasing dietary energy on hepatic lipogenesis in growing chicks. II. Increasing energy by fat or protein supplementation.

The effect of increasing the energy content of the diet through supplementation of various levels of fat or protein on hepatic lipogenesis and activities of associated enzymes of liver was examined in force-fed growing chicks. Hepatic lipogenesis was significantly decreased as the dietary metabolizable energy level was increased through supplementation of fat or protein. The activity of nicotinamide adenine dinucleotide phosphate-malate dehydrogenase (NADP-MDH) (EC 1.1.1.40) in liver was higher (P less than .01) in chicks fed diets containing the lowest energy level. The activity of citrate cleavage enzyme (EC 4.1.3.8) in liver was significantly depressed as the dietary metabolizable energy level increased through supplementation of fat, whereas increasing the dietary metabolizable energy level through protein supplementation resulted in a significant increase in citrate cleavage enzyme activity in liver. Nonesterified fatty acid concentration in serum was significantly increased as the dietary metabolizable energy level was increased through supplementation of fat.
