Iodine-131 metaiodobenzylguanidine intra- and extravesicular accumulation in the rat heart.

In order to establish the appropriate time for [123I]MIBG human myocardial imaging to assess the adrenergic nerve activity, the time courses of metaiodobenzylguanidine (MIBG) intra- and extravesicular accumulation in the rat heart were estimated by using [131I]MIBG and reserpine. In the heart, the intravesicular accumulation was relatively constant, while the extravesicular accumulation decreased rapidly from 5 min to 6 hr. The intravesicular percentage of the total cardiac tissue concentration reached a plateau value of 50% at 4 hr after i.v. injection of [131I]MIBG. In the spleen, similar time courses were observed as those in the heart, both of these organs being richly innervated by adrenergic nerves. Along with the time activity difference previously observed in the human hearts, these results suggest that at 4 hr post i.v. injection, [123I]MIBG myocardial imaging will best express the neuronal accumulation of the tracer and may be useful for the assessment of adrenergic function in various pathological conditions of the human heart.

A solid-phase enzyme immunoassay for anti-insulin antibody in diabetes mellitus patients.

A solid-phase enzyme immunoassay for the measurement of anti-insulin antibodies in the sera of patients with diabetes mellitus was developed. Porcine insulin conjugated with bovine serum albumin was used for coating microtiter plates. This assay was as sensitive as the conventional radioimmunoassay. Anti-insulin antibody titers measured by the enzyme immunoassay and conventional radioimmunoassay correlated well and the correlation coefficient was 0.920, which was statistically significant (P less than 0.001). The enzyme immunoassay detected anti-insulin antibody in 23 out of 35 sera of patients with insulin-dependent diabetes mellitus. The present enzyme immunoassay does not require radioactive materials, is less expensive and is concluded to be practically useful.

Rapid clearance of iodine-131 MIBG from the heart and liver of patients with adrenergic dysfunction and pheochromocytoma.

Iodine-131 MIBG, a radiolabeled adrenergic neuron-blocking agent, decreased rapidly from the heart and liver of patients with adrenergic dysfunction (n = 3) and pheochromocytoma (n = 2) when compared with eight controls. The 4-hr activity expressed as percentages (mean +/- s.d.) of the 20-min counts were as follows: 80 +/- 3.0% in the controls compared with 60 +/- 7.6% in the patients over the heart (p less than 0.01) and 79 +/- 3.2% in the controls compared with 51 +/- 17% in the patients over the liver (p less than 0.02). However, there was no significant difference in the rate of [131I]MIBG decrease in these organs between controls and patients in the intervals subsequent to 4 hr (p greater than 0.05). These findings suggest that adrenergic neuronal uptake of [131I]MIBG in these organs is smaller in the patients than in the controls. Measurements of time-activity relationships of radioiodinated MIBG may be useful for assessment of adrenergic function of these organs and thus of generalized disorders of adrenergic innervation.

Application of ACTH stimulation to adrenal imaging with radioiodocholesterol.

ACTH-stimulation adrenal imaging (ACTH-I) was performed in 14 patients after baseline imaging (B-I) was performed. In six patients with no adrenal diseases in whom the findings of B-I were equivocal, morphologic normality of the adrenals was confirmed by ACTH-I because of increased adrenal uptake of radioiodocholesterol. In three patients with cortisol-producing tumors, visualization of contralateral glands by ACTH-I provided indirect evidence for autonomous cortisol secretion of the tumors. In three patients with pheochromocytoma or cyst, ACTH-I increased adjacent cortical radioactivity to more clearly delineate the lesions. In two patients with primary adrenocortical insufficiency, exogenous ACTH had no effect on adrenal uptake of the tracer. Correlation was observed between response of the adrenal net counts and urinary excretion of 17-OHCS and 17-KS. ACTH-I is useful when B-I does not provide sufficient diagnostic information or further information is needed due to low or absent radioiodocholesterol uptake by the adrenal gland(s).

Adult human autoimmune thymus transplantation into nude mice - functioning of human thymus epithelium in nude mice.

Thymuses from patients with autoimmune disease were transplanted into nude mice. The thymus transplants survived for over 40 days and evidence of restoration of thymic function was observed. The de facto loss of human thymocytes from the implanted tissue resulted in an in vitro culture of pure thymus epithelium. The results suggest that adult human autoimmune thymus has the functional ability to restore immunocompetence for a thymusless mouse, and that human "thymic hormone" is a substance which does not have species specificity. This new approach to the functional activity of thymus epithelium may also be useful for a study in the pathogenesis of autoimmune disease.

In vitro culture of thymic epithelial cells from various autoimmune diseases: its thymosin-like effect on human peripheral lymphocytes.

We succeeded in the culture of the thymic epithelium which obtained from the patients with various autoimmune diseases aged from 20 to 65 years whose thymuses should have already been involuted with decreased thymic function. The mean percentage of active rosette-forming cells significantly increased after the incubation with thymic monolayer culture medium. These observations suggest that thymic epithelia in autoimmune diseases produce thymosin-like factor(s) in culture medium.

Thymus abnormalities in ulcerative colitis- comparative study with other autoimmune diseases.

Since the thymus is thought to play an important role in the pathogenesis of ulcerative colitis, thymus abnormalities were studied on 18 cases of ulcerative colitis by using pneumomediastinography and histopathological examination. We have compared the thymic abnormality of ulcerative colitis with that of 104 cases of variety of autoimmune diseases. The denser and larger thymic shadow was seen in ulcerative colitis and this finding was same as in other autoimmune diseases. But the thymic size in ulcerative colitis was the largest in all examined autoimmune diseases. The incidence of thymic lymphoid follicle formation in ulcerative colitis was significantly higher than that of accidental death and this high incidence of lymphoid follicle formation is similar to other autoimmune diseases. We suggested that ulcerative colitis might be caused by the abnormal thymus dependent system.
