Familial risk factors for a second primary malignancy in endometrial or ovarian carcinoma.

OBJECTIVE: To elucidate the risk factor for a second primary carcinogenesis in endometrial or ovarian carcinoma patients. METHODS: Endometrial and ovarian carcinoma patients treated from 1975 through 1990 were analyzed clinicopathologically. RESULTS: The incidence of patients with a second primary malignancy was 10.5% (28/267) in endometrial carcinoma cases, and 9.0% (13/144) in ovarian carcinoma cases. Endometrial carcinoma patients whose parents, siblings, and/or children had developed malignant diseases were at risk for a second primary malignancy. This familial factor is also believed to be a risk factor for ovarian carcinoma patients, but the results lacked statistical significance. There also was no statistical significance with respect to smoking and/or alcohol consumption. More than one-half of the second primary carcinomas were breast, colon, or stomach carcinomas. CONCLUSIONS: Cancer patients with this familial high-risk factor should be carefully and regularly followed up by monitoring at every anatomic site, especially the breast, stomach, and colon, in order that the development of a second primary carcinoma can be detected as early as possible, and not be overlooked in examinations.

Cell dynamics of undifferentiated carcinomas of the ovary. Immunohistochemical estimation of their growth fraction and apoptotic status.

The proliferative and apoptotic status of undifferentiated carcinomas of the ovary was estimated immunohistochemically by antibody for Ki-67 proliferative antigen and Le(Y) apoptotic antigen. Well-differentiated types of ovarian carcinoma were examined for comparison. In 8 cases of undifferentiated carcinomas, the proliferative activity in terms of the growth fraction expressed as percentage of positive nuclei for Ki-67 antigen ranged from 28.2 to 81.2% (mean 60%). In well-differentiated carcinomas, which included 8 serous, 8 mucinous, 4 endometrioid and 4 clear cell adenocarcinomas, the growth fraction ranged from 8.2 to 33.4% (mean 20%). Thus, statistically significant differences were found in the proliferative activity between undifferentiated and well-differentiated ovarian carcinomas. On the other hand, apoptotic status as estimated by Le(Y) staining varied considerably among the cases in both undifferentiated and differentiated carcinoma groups. Nevertheless, in general, undifferentiated carcinomas seemed to be more prone to apoptosis than well-differentiated carcinomas. It is concluded that in ovarian undifferentiated carcinomas, more active cell proliferation takes place with a concomitant cell loss compared to their differentiated counterparts.

Basement membrane status in undifferentiated carcinomas of the ovary. Immunohistochemical distribution of type IV collagen and laminin.

Basement membrane status in well-differentiated and undifferentiated carcinomas of the ovary was examined and compared immunohistochemically by using antibodies which react with type IV collagen and laminin. In well-differentiated carcinomas, which included 8 serous, 8 mucinous, 4 endometrioid and 4 clear cell adenocarcinomas, the staining showed a regular and linear pattern in the interface between epithelium and stroma. However, in 8 cases of undifferentiated carcinoma, the staining for two antibodies was irregular, being either completely negative or focally positive in the portion surrounding the carcinoma cell nests. In some cases, intracytoplasmic staining was observed. Staining patterns for two antibodies were somewhat different within undifferentiated carcinoma cases. It is concluded that in ovarian undifferentiated carcinomas, the architecture of basement membrane is substantially altered compared to their differentiated counterparts.

Expression of the proto-oncogene bcl-2 in uterine cervical squamous cell carcinoma: its relationship to clinical outcome.

We studied bcl-2 expression in patients with uterine cervical squamous cell carcinoma and correlated this phenomenon with survival. Immunohistochemical analysis with a monoclonal antibody specific for bcl-2 was used to detect the protein in tumor samples from 259 patients undergoing surgery for squamous cell carcinoma of the uterine cervix. Of the total, 67% (174) of the tumors were bcl-2 negative, and 33% (85) were positive. No significant difference in survival at five years was noted between patients with negative (78%) and positive (82%) tumors. However, when bcl-2 positive tumors were divided into partially stained (62 of 85, 73%), and diffusely stained (23 of 85, 27%) groups, the patients with partial staining had a better prognosis than those with diffused or negative (p < 0.01), staining (p < 0.05) (5 year survivals, respectively; 92%, 61%, and 78%). Though detection of bcl-2 positivity may itself not have clinical value for uterine cervical squamous cell carcinoma, the staining characters may add to predicting prognosis.

Immunohistochemical characterization of undifferentiated carcinomas of the ovary.

Immunohistochemical characteristics of undifferentiated carcinomas of the ovary were examined using formalin-fixed, paraffin-embedded tissues with an avidin-biotin staining approach. Eight cases were collected from the pathology files of our Institute from a total of 214 recorded malignant ovarian tumors. For immunostaining, antibodies reacting with epithelial membrane antigen (EMA), pankeratin, vimentin, CA 125, CA 19-9, carcinoembryonic antigen (CEA), alpha-fetoprotein (AFP), alpha-1-antitrypsin (AT), epidermal growth factor receptor (EGFR), c-erbB-2, bcl-2 and p53 proteins were used. All the cases examined were positive for EMA and pankeratin, specific markers for epithelial tumors, negative for the non-epithelial tumor marker, vimentin, and also positive for EGFR. Interestingly, only one case was positive for CA 125, despite it being one of the commonest reported indicators of ovarian cancer. CA 19-9 was positive in 7 cases, CEA in 5, AFP in 2, AT in 6 and c-erbB-2 protein in 4. Two cases were positive for p53 protein, and in 1 of these positive staining for bcl-2 was also observed. These results indicate that the epithelial nature is well preserved in undifferentiated ovarian carcinomas, although consistently positive reactions were not observed within the cases for some antigens. They further clearly show that a negative signal for CA 125 can not be considered to exclude the possibility of a primary ovarian tumor.

Intramural adenocarcinoma of the uterus, arisen from adenomyosis uteri, showing unique histologic appearances. Report of two cases.

Two cases of uterine adenocarcinoma which grew mainly in myometrium and showed unique histologic appearances are reported. Both cases were considered to have arisen from adenomyosis uteri because transitional figures were observed between carcinoma cells and adenmyotic glands in one case, and carcinomatous glands were scattered in myometrium and were associated with endometrial stroma which mimicked benign adenomyosis in another. The former case was poorly differentiated adenocarcinoma and the latter was well-differentiated adenocarcinoma. Interestingly, in the latter case, the carcinoma cells were transformed into papillary adenocarcinoma such as has been observed in the thyroid gland, and formed well-demarcated nodular mass. These findings indicate that adenocarcinoma which arise from adenomyosis uteri could show various histologic appearances, in addition to usual endometrioid adenocarcinoma.

Diagnostic problems relating to uterine cervical polyps with malignancy.

In order to clarify the diagnostic problems relating to, and the proper treatment of, uterine cervical polyps, 951 polyps were analyzed clinicopathologically. All polyps were examined histopathologically and divided into 2 groups: benign polyps (Group-1), and polyps with neoplastic or malignant lesions (Group-2). Polyp size and cervical cytology are not always reliable for predicting the correct diagnosis of neoplastic or malignant lesions. Colposcopic diagnostic accuracy was 56.3%, but increased to 75.0% when combined with cytology. Neoplastic or malignant lesions were found at both the cervical polyp and the cervix in 21 out of 24 Group-2 patients (87.5%). These results suggest that in order not to miss rare neoplastic or malignant lesions in polyps and/or cervices, histopathological examinations of all polyps, aided by cervical cytology and colposcopic observation, are necessary.

Clinicostatistical study of low potential malignancy ovarian cystadenoma (borderline cases).

The number of ovarian cystadenoma of low potential malignancy (abbreviation: LPM) was 31 (5.7%) among common epithelial tumors [benign cystadenoma = 400 (74.0%), carcinoma = 110 (20.3%)]. Their mean ages, bilaterlity, frequency of ascites and nulligravidity were situated intermediately between those of benign cystadenoma and carcinoma. Abdominal tumor, distention, pain and metromenorrhagia were the main symptoms, although gastrointestinal complaints were rare in LPM cystadenoma. With LPM cystadenoma, 19 (61.3%) patients were at stage I, 7 (22.6%) at stage II and 5 (16.1%) at stage III, but with carcinoma, 44 (40.0%) were at stage III-IV [33 (30.0%) at stage I, and 31 (28.2%) at stage II]. The 5-year actuarial survival rate was 90% for LPM cystadenoma, but 43% for carcinoma. These results suggest that the clinical and biological features as well as the histological definition of LPM cystadenoma are situated intermediately between those of benign cystadenoma and carcinoma.

Clinicostatistical study of ovarian tumors of germ cell origin.

Ovarian tumors of germ cell origin consisted of 440 dermoid cysts, 5 struma ovariis, 3 immature teratomas (G1, G2), 2 carcinoids, 8 dysgerminoma, 3 yolk sac tumors, one choriocarcinoma, 4 dermoid cysts with malignant transformation, and one immature teratoma (G3) of a total of 1,387 ovarian tumors. The patients' mean ages ranged between 17.3 years to 31.9 years, but the mean ages of those with carcinoid and dermoid cyst with malignant transformation were 46.0 and 53.0 years, respectively. In dermoid cyst bilaterality was 17.3% and occurrence during pregnancy or puerperium was 19.6%. In benign tumors, torsion of pedicle and association with pregnancy or puerperium were frequent unlike common epithelial carcinoma, but ascites was rare. Abdominal tumor, pain and metromenorrphagia were the main symptoms, but abdominal distension was scarce with dermoid cysts. The rate of malignant transformation of dermoid cysts was 0.9% overall but 4.3% in patients over 40 years. The prognosis for borderline germ cell tumors was good.

Long-term mass screening in uterine cervical carcinoma eradication strategy.

Results of the Tochigi Cancer Detection Center screening of a total of 252,453 women for uterine cervical neoplasm, conducted over fifteen years, are reported. Among the women, 16,454 (6.5%) were referred for precision reexamination and 14,129 (85.9%) responded. Eighty-nine patients with unmistakably invasive carcinomas at International Federation of Gynecology and Obstetrics (FIGO) Stage Ib or more, 81 with microinvasive carcinomas at Stage Ia and 192 with carcinomas in situ were found, all of them receiving appropriate treatment. The cervical neoplasm detection rate decreased from 26.2 per 10,000 to 6.9 per 10,000 during the 15 years (average rate, 0.15%) with a relatively increasing trend of early stage carcinoma (microinvasive and in situ carcinomas) (62.9-84.0%) over frank carcinoma (37.1-16.0%). During the same period, the ratio of the uterine carcinoma mortality rate to that of all carcinomas decreased from 5.0 to 2.7%. The mean ages (years) of patients with frankly invasive (53.4), microinvasive (48.2) and in situ (45.6) carcinomas, however, remained constant over the 15 years. Five-year actuarial survival rates were 86.6% for frank, 98.8% for microinvasive and 98.8% for in situ carcinomas. Among rechecked women, all frank carcinoma patients were rescreened after more than one-and-a-half years of their last check. These results suggest that long-term mass screening and yearly checks would be of value both for detecting uterine carcinoma earlier and for reducing its mortality rate.

Detection of human papillomavirus types 6/11, 16 and 18 in exfoliated cells from the uterine cervices of Japanese women with and without lesions.

Human papillomavirus (HPV) infection of the uterine cervices of Japanese women with and without lesions was identified by the filter in situ hybridization method. Exfoliated cervical cells from 23 cervical papillary condylomas, 70 cervical intraepithelial neoplasia (CIN) grade I/II, 26 CIN III, 31 invasive cervical cancers and 666 cervices without evidence of disease (including 53 pregnant women) were tested for the presence of HPV types 6/11, 16 and 18. The positive rates for the detection of HPV types 6/11, 16 and 18 DNA were 47.8%, 26.1% and 8.7% in cervical condylomas, 5.7%, 15.7% and 8.6% in CIN I/II, 0, 34.6% and 0 in CIN III, 3.2%, 38.7% and 9.7% in invasive cervical cancers and 0.9%, 1.8% and 0.6% in the control cervices. These data suggest that, in Japan, HPV6/11, HPV16 and HPV18 infections are also prevalent in cervical cells with normal phenotype, and the type of HPV infection of the uterine cervix is related to the histological diagnosis.

Bacterial overgrowth in the jejunum of ICR mice and Wistar rats orally administered with a single lethal dose of fusarenon-X, a trichothecene mycotoxin.

A single oral dose of fusarenon-X (F-X), a trichothecene mycotoxin, resulted in abnormal microflora in the jejunum in ICR mice and Wistar rats with some differences in dose response between the species. In the acute phase, enterobacteria, streptococci, Clostridium perfringens and bacteroides showed remarkably increased counts in the jejunum of mice and rats dosed with F-X while lactobacilli showed a decrease in count. F-X brought an invasion of Pseudomonas aeruginosa into the livers, lungs, kidneys and spleens of ICR mice. Changes in the jejunal microflora appeared after 7 h in ICR mice and after 24 h in Wistar rats after a single oral dose of F-X of 7.5 and 4.0 mg/kg b.w., respectively, and the microflora returned to its normal state at 72 h in mice and 96 h in rats. The changes of intestinal microflora were followed by alterations in the growth curves of both animal species. The pH in the glandular stomach was also greatly enhanced before changes in the jejunal microflora. Acute F-X intoxication may be an involved manifestation of essential cytotoxicity of F-X mycotoxin alone and secondary bacterial overgrowth in the bowel.

Reconstruction of sigmoid vagina and conduit in total pelvic exenteration for recurrent cervical carcinoma.

Although total pelvic exenteration is performed for central recurrent or persistent uterine cervical carcinoma, the patient is damaged both physiologically and psychologically by its massive surgical procedure and complications as well as the loss of the vagina. Vaginal reconstruction at the time of surgery brings much advantageous satisfaction to the patient's future life and a strong bond with her husband. Furthermore, a new reconstructed vagina occuping the pelvic cavity prevents intestinal herniation and is available for bimanual examination in the follow-up to surgery. A case of recurrent cervical carcinoma is presented, with reconstruction of the vagina and conduit using the sigmoid colon in total pelvic exenteration.

Experience with the treatment of metastatic ovarian carcinoma.

Metastases from non-genital sites comprise about one in six of our malignant ovarian tumors. The mean age of our 24 patients with such metastases was 43.1 years and 81% had bilateral ovarian metastases. Out of the 24 patients we studied, 21 had a gastric primary. The overall survival rate was 41.7% in 6 months, 25.0% in 1 year and 12.5% in 2 years. Patients with no extraovarian metastases had a survival rate of 75.0% at 6 months, 62.5% at 1 year and 25.0% at 2 years after operation.

Is postoperative radiotherapy or maintenance chemotherapy necessary for carcinoma of the uterine cervix?

The effectiveness of postoperative radiotherapy and adjuvant chemotherapy was examined for patients with uterine cervical squamous cell carcinoma. Whole pelvis irradiation is unnecessary for patients with less than 2/3 of depth of cervical local involvement without pelvic regional lymph node metastases. Adjuvant chemotherapy with tegafur [1-(2-tetrahydrofuryl)-5-fluorouracil] (600 mg/day) has failed to improve the survival of patients with positive lymph node metastases.

Pregnancies following conservative treatment of malignant ovarian tumors.

Sixteen patients with malignant ovarian tumors were treated conservatively; 1 had a serous cystadenoma of low potential malignancy; 6 had a mucinous cystadenoma of low potential malignancy; 2 had a pure dysgerminoma; 2 had a mature solid teratoma; 3 had a mucinous cystadenocarcinoma; and 2 had an endodermal sinus tumor. Postoperative chemotherapy was given in 12 cases and 1 patient with a pure dysgerminoma had radiotherapy. Eighteen pregnancies occurred in 13 patients. One full-term and one 8 month premature infant died in utero, and 2 infants had malformations; the other 14 infants are all well. The patient with moderately differentiated mucinous cystadenocarcinoma who delivered a premature infant and 2 patients with endodermal sinus tumors died of recurrence.

Effects of dietary lactose and purified diet on intestinal microflora of rats.

In the rats fed on lactose-containing purified diet (PDL), bifidobacteria remarkably increased in number in both the jejunum and cecum as compared with those fed on control diet (PD) or conventional diet (NMF). Besides, lactobacilli, streptococci, staphylococci, and Escherichia coli increased in number in the jejuna and/or ceca of rats fed on PDL as compared with those fed on PD. The incidence and numbers of Lactobacillus species were remarkably different between the rats fed on PDL and those on any of the other diets. Indole-producing Bacteroides spp. were rarely isolated from the PDL-fed rats. The cecum was markedly enlarged in the PDL rats and reduced in the PD rats as compared with that in the NMF animals.
