[Analysis of skin changes in outpatients bitten by a tick during a 1-year period].

Ninety-two patients with tick bites (Ixodes ricinus) visited Outpatient clinic for dermatovenerology of Military Medical Academy during a period od one year. Tick bites were generally localized on lower extremities and were seasonal in character. Most patients were 50-59 years old. Typical early skin manifestations in the form of Erythema migrans were diagnosed in 19.56%, and atypical forms of erythema in 8.69%. Late manifestations of illness were found on only two patients.

[Xanthomatosis in patients with hyperlipoproteinemia].

The clinical findings in 63 patients with xanthomas were analysed. Among them 5 had xanthelasmas and normal lipids. The largest group (37) consisted of females with xanthelasmas and heterozygotic form of hyperlipoproteinemia (HLP) type II. In this group HDL cholesterol values (1,5 mmol/l) were normal and ischemic heart disease (IHD) was rare. However, in 14 males HDL values (1,1 mml/l) were low while IHD was common. Cholesterol deposits in the folds of the palm (xanthochtomia striata palmaris), tuberous xanthoma and peripheral artery changes were characteristical findings for all three patients with HLP type III. In patients with HLP type IV and eruptive xanthomas, obesity was common finding (4/4) and disturbed glycoregulation (2/4) and triglyceride values were very high (X = 61,1 mmol/l).

[Electron microscopy of blood vessels in alopecia areata].

The use of electron microscope in the study of the diseased skin of four patients with Alopecia areata has detected changes in the endothelial cells with narrowed capillary lumen. The authors have brought these findings in connection with the decreased blood circulation in lesions of patients with Alopecia areata which necessiates the use of vasodilatator agents in their therapy. Etiopathogenesis of the disease has been discussed.

[Treatment of porphyria cutanea tarda using plasma exchange].

Results in the treatment of 5 patients with porphyria cutanea tarda by plasma exchange are presented. The procedure was applied every second or third day, 2-4 procedures in total. Saline solution, 5% or 12% albumin were used as plasma substitutes. The treatment lead to rapid fall in urine porphyrin level and improved clinical picture. The treatment was continued ambulatorily by 60 mg of vitamin E tbl. daily and Essentiale tbl. 3 X 1, which lead to further progressive fall in porphyrin level and improved clinical picture. In one patient citrate intoxication occurred which was prevented by Calcium Sandosten ampl. The importance of this method in the treatment of porphyria cutanea tarda and indications for this treatment are discussed.
