[Individual rehabilitation planning in the vocational rehabilitation of mentally ill young people].

For many young people, their aspiration for an environmentally compatible, and societally meaningful, field of activity is very marked and reflects the values they hold. Vocational rehabilitation should be oriented accordingly, and take up the motivational potential implied. The more accurate the knowledge of previous vocational experience, motivations, attitudes toward work, and of the subjective difficulties in coping with work-related requirements, the more detailed can vocational rehabilitation planning be devised. The potential is increased by a concept that is guided by insights from the psychology of work and emphasizes individual rehabilitation planning as well as follow-along developmental counselling. Assessment of 52 individual service instances for mentally ill youths and young adults, of Berlin-Kreuzberg, revealed an appropriate placement quota of 67%. The developmental course taken in the individual cases shows that satisfactory long-term outcomes can be achieved through careful entry diagnostics, individual rehabilitation planning, ongoing developmental counselling, selective educational intervention, industrial try-outs, as well as stabilizing aftercare.

Eikenella osteomyelitis of the mandible associated with anemia of chronic disease.

An unusual case of mixed bacterial osteomyelitis has been reported. The prodominant organism, E corrodens, normally is an inhabitant of the aerodigestive tract; its pathogenicity should not be overlooked in the diagnosis and treatment of oral infections. In addition, descriptions of anemia of chronic disease and of the spontaneous healing of a large defect of the bone have been presented.

Adult

Defective chemotaxis in uremia.

Of uremic patients, 37% demonstrated the presence of CFI in their sera. The serum factor acts directly on leukotatic factors, C5f, C3f, and BF to render them irreversibly inactive. Inhibitory activity was found in five of 16 patients: 37% to 61% for C5f, 25% to 36% for C3f, and 28% to 51% for BF. There is no correlation of this inhibitory activity with BUN or creatinine levels. The presence of this chemotactic inhibitor may be responsible for the increased incidence of infection in uremia.

Aminocaproic Acid

Vestibular extension with xenogenous fascial grafts.

Fascia obtained from the abdominal wall of a rat was transplanted to the oral cavity of five dogs as a graft material for vestibular extension. In all five dogs the grafted fascia remained in place as the surgical defects were covered with epithelium, which transpired within a period of 4 weeks. After 6 months, the vestibular depth was increased significantly and the vestibule was smooth and regular.

Intraoral-mucosal-xenogenous fascial grafting.

Fascia obtained from the abdominal wall of a rat was transplanted into mucosal defects created in six dogs. In all of the dogs, the fascia was found to adhere firmly to the underlying tissue and surrounding epithelium. Biopsy specimens were taken at weekly intervals for 4 weeks. The defects healed without any apparent contracture and were covered by new epithelium. It appears that xenogenous fascia is a potentially useful graft material for intraoral soft tissues.

Animals

Multiple differences in red cell antigens and isoimmunization.

Data available for 175 families with maternal isoimmunization to red blood cell antigens, exclusive of A, B, and Rho, were analyzed for interaction of unrelated antigens. In 153 families the mother developed antibodies for single antigens, distributed among seven blood group systems; rare, high frequency antigens; and unclassified antigens. The immunizing fetal red blood cells ABO-incompatible with the mother in less than 5 per cent of instances, a finding attesting to the impact of ABO incompatibility on immunogenicity of most red blood cell antigens. No evidence was found for interference with immunization to one particular red blood cell antigen by simultaneous differences in unrelated antigens. Maternal immunization to more than one red blood cell antigen, encountered in 22 families, was associated in 13 instances with preceding blood transfusion to the mother and/or pregnancies in which fetal red blood cells contained only one of the immunizing antigens. In six of the remaining nine instances of multiple isoimmunization, Kidd antibodies were involved. Preceding immunization to a variety of red blood cell antigens was noted to interfere with immunization to potent immunogens: Rho (nine instances of complete, and one instance of partial, suppression), hr' (eight instances of suppression), and K (two instances of suppression). A mechanism, designated as "diversion of antigen", was proposed in order to explain the phenomena observed in human isoimmunization as well as in animal models in which preceding immunization to one red blood cell antigen affects immune responses to red blood cells carrying the immunizing and additional antigens.
