[Heat resistance of Cladosporium isolated from laboratory animal facilities].

Heat resistance tests for the saprophyte, Cladosporium, isolated from laboratory animal facilities were carried out. In testing the effects of moderate and high temperature conditions, C. sphaerospermum (C. s) and C. cladosporioides (C. c) were found to grow on media in temperatures less than 32 degrees C, but did not in temperature of 35 degrees C and over. The colony diameter of Cladosporium became smaller as temperature increased. The death time of C. s treated with moist heat was within 12 min at 48 degrees C and that of C. c was within 26 min at 43 degrees C. Both Cladosporium species could not survive for more than 1 min at 55 degrees C. On the other hand, Cladosporium treated with dry heat could not survive more than 69-12 min (C. s) and 39-9.5 min (C. c) at 70-100 degrees C. From these results, it can be seen that Cladosporium was definitely sensitive to heat treatment, and the authors assume that heat is a means of prevention in laboratory animal facilities.

[Clinical significance of thrombin-antithrombin III complex and plasmin-alpha 2 plasmin inhibitor complex in chronic liver diseases].

Thrombin-antithrombin III complex (TAT) and Plasmin-alpha 2 plasmin inhibitor complex (PIC) were examined in fifty two cases of various chronic liver diseases. TAT was significantly elevated in cases of hepatocellular carcinoma (HCC), but PIC did not show significant changes in any chronic liver diseases. Elevations of TAT and PIC were seen in cases of HCC accompanied by tumor enlargement and extensive tumor thrombosis. In cases of HCC undergoing transcatheter arterial embolization (TAE), TAT and PIC increased on the next day after TAE, and tended to recover with time, returning to almost normal at fourth week. Prolongation of prothrombin time, elevation of FDP and positive FM test were noted more often in liver cirrhosis with disseminated intravascular coagulation (DIC) than in severe liver dysfunction without DIC. Of five cases confirmed as DIC, only three cases were diagnosed as DIC by DIC score. On the other hand, TAT and PIC were significantly elevated in DIC cases. Especially, TAT exceeded 30 ng/ml in all DIC cases. TAT was regarded to be useful for the diagnosis of DIC in severe liver dysfunction.

[Progressive glomerular sclerosis and renal failure in rat with 7/8 nephrectomy model].

A model of progressive glomerular sclerosis and chronic renal failure was established in 7/8 nephrectomized rats. Three fourths of left kidney of six-week old Sprague Dawley rats were ligated and removed. One week later right kidney was removed. As controls 3/4 nephrectomized rats operated the same manner as 7/8 nephrectomized rats and sham operated rats were used. Remarkable excretion of the protein into the urine and rise in blood pressure were noted in all 7/8 nephrectomized rats. Eight weeks after 7/8 nephrectomy, serum creatinine rose as high as 2.8 +/- 0.3 mg/dl and BUN 260 +/- 65 mg/dl. In contrast, only minor degree of elevation of blood pressure, excretion of protein into the urine and elevation of serum creatinine were noted in 3/4 nephrectomized rats. Morphologically deposition of rat IgG, C3, fibrinogen and albumin were observed mainly on the capillary wall in 7/8 nephrectomized rats. Minor amount of deposition was observed in 3/4 nephrectomized rats. By light microscopy there were remarkable glomerular and tubular lesions in 7/8 nephrectomized rats. Most of the glomeruli showed hypertrophy, 40% of the glomeruli had cellular or fibrocellular crescent, 34% of glomeruli failed into global sclerosis or hyalinosis, tubular atrophy or enlargement and hyaline cast in the lumen and interstitial fibrosis were also noted. By electron microscopy detachment of epithelial and endothelial cells from the GBM, increase in the mesangial matrix, obliteration of the capillary lumen by fibrin, hyaline material were observed. Only minor degree of increase in mesangial matrix and epithelial degenerative lesions were observed and 3% of glomeruli were sclerosed in 3/4 nephrectomized rats. From these results 7/8 nephrectomized rats can serve as useful model for studying the mechanism of glomerular sclerosis or hyalinosis and effect of various drugs on glomerulonephritis.

[Usefulness of partial splenic embolization (PSE) in hepatocellular carcinomas showing a risk of gastrointestinal bleeding after transcatheter arterial embolization (TAE)].

We have examined 103 patients with a hepatocellular carcinoma (HCC) who were treated by transcatheter arterial embolization (TAE) with a partial splenic embolization (PSE) (n = 5) or without a PSE (n = 98). It was found that gastrointestinal (GI) bleeding after TAE occurred in 10 (10.2%) out of the 98 TAE patients and within 10 days after the TAE. In these GI bleeding after TAE patients, the platelet counts were significantly low and marked splenomegaly was seen. It also was found that there was a risk of GI bleeding after TAE in cases showing thrombocytopenia (less than 50,000/microliters) and marked splenomegaly with a splenic index of more than 50. In 5 patients with a high risk of GI bleeding, for whom a PSE was performed, the platelet counts were markedly increased even four weeks after the TAE. The survival rate of TAE patients given a PSE was significantly higher than in TAE patients manifesting a risk of GI bleeding. From these results, it would seem that TAE patients given a PSE is the better therapy for HCC patients manifesting a risk of GI bleeding.

[A case of metastatic liver cell carcinoma in the abdominal wall due to direct skin invasion from protruding liver tumor].

A case of a hepatocellular carcinoma with a metastasis to abdominal wall is reported. The patient was a 52-year-old man who, in December, 1982, had been found to have a 16 x 13 cm of tumor in the right lobe of the liver. He received transcatheter arterial embolization (TAE) several times and a hepatic segmentectomy twice. He survived more than six years until his death on January, 17, 1989. A subcutaneous tumor that had appeared in May, 1988 had gradually increased in size and was found to have reached 6 x 5.5 x 2.5 cm on autopsy. The tumor had connected directly with the liver tumor.

[A case of hepatocellular carcinoma (HCC) with bleeding due to duodenal perforation by the tumor].

On June 11, 1986, a 70-year-old man was introduced to our hospital because of an elevated AFP and hepatomegaly. He was diagnosed as having an HCC in the left medial segment and a transcatheter arterial embolization (TAE) was able to reduce his AFP level. In December, 1986, repeated tarry stool was noted, and he was readmitted to hospital on January, 28, 1987, because of severe anemia. An ordinary X-ray revealed an abnormal gas shadow in the right upper abdomen. A subsequent endoscopic examination showed a tumoral mass protruding into the duodenal lumen through a duodenal perforation. After death an autopsy revealed that the perforation was due to the expansive growth of the tumoral mass to the duodenum.

Localization of alpha 1-adrenoceptor in the rat kidney: in vitro autoradiography using [3H]-bunazosin.

We studied the precise localization of alpha 1-adrenoceptor in the rat kidney using in vitro macro- and micro-autoradiography (ARG) of [3H]-bunazosin. The slide-mounted nonfixed frozen sections (10 micron) of kidney were incubated with [3H]-bunazosin (5 nM) at 24 degrees C for 45 min, and then applied to macro- and micro-ARG. The macro-ARG revealed that the bunazosin binding sites were present mainly in the rat renal cortex. These bindings were evaluated to be specific by means of computerized image analysis system (RAS 1,000) of ARG. Micro-autoradiogram showed that the tubules in the renal cortex had higher specific grain density than the glomeruli. These findings suggested that alpha 1-adrenoceptor in the rat kidney located mainly on the cortical tubules.

[A marked Hegglin syndrome in pheochromocytoma].

A 46-year-old man was hospitalized with paroxysmal headache and chest discomfort. His blood pressure varied, occasionally being up to 300/160 mmHg. Cardiac examination revealed a decrescendo type of diastolic murmur (aortic regurgitation) and S4 gallop. Both blood and urine catecholamine levels were extremely high. Electrocardiogram and echocardiogram showed severe left ventricular hypertrophy pattern presenting hypertrophic cardiomyopathic changes. The phonocardiogram showed marked Hegglin syndrome (QT; 450 msec and QII; 310 msec). Right adrenal tumor, pheochromocytoma was found on the abdominal CT scanning and 131I-metaiodobenzylguanidine (MIBG). In the post-operative period, Hegglin syndrome was completely abolished (QT; 360 msec and QII; 345 msec,), and also electrocardiographic left ventricular hypertrophy pattern improved immediately after surgical removal of the pheochromocytoma.

An infection of cow with Mortierella wolfii.

A Japanese black cow from a farm in Gunma prefecture showed pyrexia, anorexia and depression. An autopsy revealed hepatic lesions consisting of numerous approximately spherical pale lesions scattered about on the surface and cut parts. The inflammatory lesions were observed in the right lobe of the lung. A part of the trachea was whitish and thickened. Histologically, necrotic foci were observed throughout most of the liver with fungal proliferation about the blood vessels. The lesions of the lung were seriously exudative and the fungus was also present in some of the bronchiole and on bloodvessel walls. Mortierella wolfii was isolated from the liver. The hyphae in the liver were identical in appearance with those seen in the tissues of rabbits experimentally inoculated with M. wolfii. This report is the first case of M. wolfii infection in Japan.

Properties of [3H]bunazosin binding in rat kidney.

Properties of alpha 1-adrenoceptor binding in the renal cortex of rats were studied with [3H]bunazosin as the radiolabeled ligand. Both temperature and incubation time influenced bunazosin receptor binding. Subcellular distribution of [3H]-bunazosin binding revealed that the membrane fraction (30,000 x g pellet) had the largest proportion of total binding sites. Pretreatment of membrane preparation at 56 degrees C for 30 minutes greatly decreased the specific binding of [3H]-bunazosin, though there was no significant effect of the pretreatment at 24 degrees C and 37 degrees C for 30 minutes. Among various cations, some divalent ions such as Cu++ and Zn++ greatly decreased bunazosin binding, whereas monovalent ions had no effect on specific binding. Results of Scatchard analysis suggested that the rat renal cortex membrane has single binding sites with an apparent dissociation constant of 0.38 +/- 0.06 nmol/L, though the rat medulla membrane has no potent binding activity with bunazosin. The displacement study revealed that various adrenergic agents inhibit [3H]bunazosin binding in dose-dependent fashion; the rank order of potencies was bunazosin prazosin > phentolamine >> dl-norepinephrine > clonidine, yohimbine >> pindolol, propranolol. These findings reveal that bunazosin has specific receptor binding in the rat renal cortex, indicating that alpha 1-adrenoceptors exist in the rat renal cortex.

Keratomycosis due to Alternaria alternata corneal transplant infection.

A 53-year-old woman was found to have an ulcer on her successfully transplanted corneal graft. Many fungal elements were observed in the smear of the ulcerated tissue, and Alternaria alternata was cultured. The ulcer was treated with pimaricin and thimerosol topically, and 5-fluorocytocin (5FC) generally and healed to scar after two months. The ulcer did not invade the host cornea, but remained in the donner cornea in all clinical course. The MIC of five drugs on the isolated strain was following; thimerosal 0.0063, pimaricin 2.0, amphotericin B 3.2, aystatin 6.3 and 5FC 100.0 micrograms/ml each.

Human dermatophytosis caused by Trichophyton equinum.

A case of dermatophytosis caused by Trichophyton equinum is reported. A 25-year-old man employed at a breeding center of a horse racing course was infected on the left arm in August, 1981. The lesion had a vesicle or a small pustule accompanied by severe itching. The fungal elements of the scale were identified by microscopic observation. Griseofulvin administration was found to be very effective for treating this infection. In a mycological examination, T. equinum was isolated mainly on cycloheximide-chloramphenicol Sabouraud's dextrose agar and chloramphenicol potato dextrose agar. Equine dermatophytosis was quite prevalent at this race course, so that this area as well as equipment used for the maintenance and care of the horse was very likely to be the source of the patient's infection.
