Nonepileptic seizures.

The widespread use of video-electroencephalogram monitoring has dramatically increased our recognition of the high prevalence and diversity of nonepileptic seizures. Nonepileptic seizures stand squarely in the interface between psychiatry and neurology, an area that has been both claimed and denied by both sides. Collaborative exploration of this border zone has provided new insights into a disorder that may be as ancient as epilepsy.

Nonepileptic seizures during apparent sleep.

We report 12 patients with nonepileptic seizures (NESs) documented with video-electroencephalographic monitoring during apparent sleep. There were 7 females and 5 males, ages 11 to 67 years (mean, 33 yr). In all patients there was a well-organized posterior alpha rhythm present immediately before onset of clinical changes despite the appearance of sleep. Patients later "confirmed" that these attacks occurred during sleep. Features of these episodes were typical of NESs and none were associated with ictal or postictal electroencephalographic changes other than muscle and movement artifact. Provocative testing with suggestion confirmed the nonepileptic nature of these episodes in 9 patients. Three patients had epileptiform discharges during prolonged interictal recordings and 1 had a documented complex partial seizure. Our findings indicate that reports of seizures during sleep are not uncommon among patients with NESs.

Hyaluronan solution as a cartilage antidesiccant.

During arthrotomy, desiccation of the articular cartilage of synovial joints by the ambient environment of the operating room causes chondrocyte death and may promote degenerative changes within the joint. This study documents the efficacy of a biomaterial, hyaluronan (HY) solution, to maintain chondrocyte viability during desiccation. Rabbit patellar articular cartilage was air-dried for 2 h with and without application of the HY solution. As assessed by 35SO4 uptake and autoradiography, the HY solution helped maintain chondrocyte viability, when compared to air dried specimens (p less than or equal to 0.02). HY solution applied to the articular surfaces of joints undergoing arthrotomy may offset the detrimental effects of desiccation, such as chondrocyte death and degenerative joint changes.
