Bacterial persistence in leprosy.

Bacterial persistence is the capability of microorganisms to survive in the host despite adequate antimicrobial treatment. This is a general phenomenon and has been observed during infection with a number of organisms. The subject is reviewed in relation to the treatment of leprosy. The presence of persisters in the large bacterial populations that exist in patients with lepromatous leprosy is not surprising. It is unlikely that in the near future a new drug or combination of drugs will be found that is capable of eradicating persisting M. leprae. It may be useful, nevertheless, to reconsider whether the sporadic demonstration of viable, drug-susceptible organisms after antimicrobial treatment of variable duration and efficacy warrants a policy of life long chemotherapy in lepromatous leprosy.

Present knowledge of immunization against tuberculosis.

This is a selective review, which, after recalling some immunological aspects, analyses the present knowledge on the protective efficacy of BCG vaccination, the vaccination reactions and complications that may be observed, and current methods of vaccine production and control. As regards the application of BCG vaccination, particular attention is given to dosage and vaccination techniques, direct and simultaneous vaccination, and revaccination. Finally, the evaluation of BCG vaccination programmes is briefly discussed.

Gelatin and Tween 80 as stabilizing agents for tuberculin dilutions.

The addition of Tween 80 to tuberculin dilutions as a stabilizer is known to increase the size of moderate and strong reactions but to reduce that of weak reactions. In order to find out whether this effect on reaction size is connected with the stabilizing action, a study was carried out to compare the reactions to tuberculin plus Tween 80 with those to tuberculin plus gelatin, an almost equally effective stabilizer. In this study, 2519 young adults were given simultaneously, by random selection, one of four experimental tests (1 TU of RT 23 with gelatin, 2 TU of RT 23 with gelatin, 2 TU of RT 23 with Tween 80 and 2 TU of RT 23 without stabilizer) in one arm and a reference test (2 TU of RT 23 with Tween 80) in the other arm.The results showed very clearly that gelatin, unlike Tween 80, increased the size of the weak, moderate and strong reactions uniformly, and it is therefore concluded that the depressive effect of Tween 80 on weak reactions is not directly connected with the stabilizing action, but is an additional effect.
