Comparison of a latex agglutination test with five other methods for determining the presence of antibody against cytomegalovirus.

A latex agglutination test for determination of antibody against cytomegalovirus was compared with five other methods: a solid-phase fluorescent immunoassay, an indirect hemagglutination test, two solid-phase enzyme immunoassays, and an indirect fluorescent-antibody method, with sera collected from 210 random blood donors. Of the sera tested, 28% were positive for anti-cytomegalovirus by concordance of four or more methods. The latex agglutination test performed well, with a sensitivity of 100%, a specificity of 99%, and positive and negative predictive values of 97 and 100%, respectively. The methods were also evaluated for the number of sera requiring repeat testing, equivocal results after retesting, ease of performance, turnaround time, and technical demands. The tests which best met the requirements for a screening test were the solid-phase fluorescent immunoassay, the indirect hemagglutination test, and the latex agglutination test. The latex agglutination test is a valuable screening tool for detecting total anti-cytomegalovirus which has high sensitivity, high negative predictive value, and rare equivocal results and also has the added advantages of ease of performance and rapid turnaround time.

[Combined electro-anaesthesia for Caesarean section and gynaecological operations (author's transl)].

Reported in this paper are 98 cases of electro-analgesia in combination with endotracheal nitrous-oxide anaesthesia. A comparison has been made with 65 controls. Excessively stressing anaesthetics may be avoided in such combined electro-anaesthesia. No significant differences were found to exist between the above and conventional approaches, in the context of blood pressure, pulse, and interior temperature. Combined electro-anaesthesia, therefore, is recommended by the authors as a non-invasive method with good applicability to obstetrics and gynaecology.

Facilitated transport of calcium by cells and subcellular membranes of Bacillus subtilis and Escherichia coli.

The level of calcium in growing cells is lower than that in the growth medium. Non-energy-dependent uptake of 45-Ca by log-phase cells of Bacillus subtilis occurs under two conditions: at 0 C or in the presence of m-chlorophenyl carbonylcyanide hydrazone. Similar uptake, but quantitatively less, occurs with Escherichia coli cells under the same conditions. Membrane vesicles prepared from B. subtilis or E. coli accumulate 45-Ca by a process that does not depend on added energy sources and is not inhibited by the respiratory poison cyanide. The properties of calcium transport in all cases is consistent with carrier-mediated, facilitated transport with specificity Ca-2+ greater than Sr-2+ greater than Mn-2+ greater than Mg-2+. Upon transfer of cells from 0 C to 20 C, pre-accumulated 45-Ca is released. Heat-killed cells do not accumulate 45-Ca and calcium is released by cells upon addition of toluene (under conditions that do not cause visible lysis). These results suggest that the facilitated uptake of calcium may be utilizing a transport system that normally is responsible for the energy-dependent excretion of calcium from the cells.

Regulation of manganese accumulation and exchange in Bacillus subtilis W23.

An overnight culture of Bacillus subtilis W23 in low-manganese tryptone broth is unable to sporulate and becomes hyperactive with regard to the manganese active transport system during stationary phase. When manganese is added to cells in spent or fresh medium, the cells immediately accumulate a high proportion of the manganese available in the medium. When the hyperactive cells are diluted into broth containing 10 muM Mn(2+), high intracellular manganese levels are reached, and inhibition of ribonucleic acid and protein synthesis occurs. This inhibition is relieved when the intracellular manganese concentration declines to the nontoxic levels characteristic of cells growing in 10 muM Mn(2+). The release of the accumulated manganese is achieved by a reduction in the uptake rate for manganese while the efflux rate remains essentially constant. Inhibitors of ribonucleic acid and protein synthesis prevent the reduction of the high rate of manganese uptake and, therefore, high net concentrations of manganese are maintained in the presence of these inhibitors. The hyperactive manganese uptake system is temperature dependent and inhibited by cyanide and m-chlorophenyl carbonylcyanide hydrazone.

[Surgical treatment of diabetic osteoarthropathy with severe dislocation].

A case with diabetic osteoarthropathy, destruction of the talus and medial dislocation of the ankle joint is presented. After cleaning up a purulent skin process above the lateral malleolus, caused by the malposition, a calcaneocrural arthrodesis was performed, with a fixateur externe. In the authors' opinion therapy should aim to preserve the limb; this is to be preferred to amputation even in cases with very severe deformities. An arthrodesis with correction of the malposition is recommended as the method of choice.
