Isolation of a gene essential for biosynthesis of the lipopeptide antibiotics plipastatin B1 and surfactin in Bacillus subtilis YB8.

Bacillus subtilis YB8 was found to produce the lipopeptide antibiotics surfactin and plipastatin B1. A gene, lpa-8, required for the production of both lipopeptides was cloned from strain YB8. When this gene was inactivated in strain YB8, neither surfactin nor plipastatin B1 was produced. However, the defective strain transformed with an intact lpa-8 gene had restored ability to produce both peptides. Nucleotide sequence analysis of the region essential for the production of the peptides revealed the presence of a large open reading frame. The deduced amino acid sequence of lpa-8 (224 amino acid residues) showed sequence similarity to that of sfp (from surfactin-producing B. subtilis), lpa-14 (from iturin A- and surfactin-producing B. subtilis), psf-1 (from surfactin-producing Bacillus pumilus), gsp (from gramicidin-S-producing Bacillus brevis), and entD (from siderophore-enterobactin-producing Escherichia coli), which are able to complement a defect in the sfp gene and promote production of the lipopeptide antibiotic surfactin. The sequence similarity among these proteins and the product similarity of cyclic peptides suggests that they might be involved in the biosynthesis or secretion of the peptides.

Debridement arthroplasty for advanced primary osteoarthritis of the elbow. Results of a new technique used for 29 elbows.

We report the technique and results of a new method of debridement arthroplasty for advanced primary osteoarthritis of the elbow. Triceps and the periosteum of the olecranon are reflected towards the ulnar side and the joint is opened by dividing the radial collateral ligament. Osteophytes are removed, the olecranon and coronoid fossae are deepened and the fibrosed anterior joint capsule is excised. The degenerative changes are always more advanced on the radial side, with erosion of the capitellum, and it is usually necessary to remodel the head of the radius. In 29 elbows reviewed at a mean of 64 months, the average gain of range of motion was 34 degrees, with good pain relief and improved grip in most patients. Two elbows required reoperation but there were no other serious complications.

Aetiology of Kienböck's disease based on a study of the condition among patients with cerebral palsy.

Examination of 110 patients with cerebral palsy revealed a diagnosis of Kienböck's disease in six wrists in five cases. All had cerebral palsy of the mixed type with athetosis and high muscle tone. It is considered that the increased prevalence of Kienböck's disease in these cases was attributable to repeated minor trauma to the lunate from childhood due to high muscle tone across the wrist joint, together with negative ulnar variance. The high prevalence of Kienböck's disease observed in patients with cerebral palsy strongly suggests that repeated minor trauma together with negative ulnar variance is also the cause of Kienböck's disease in normal individuals.

Further technical considerations of the sleeve microanastomosis.

In sleeve anastomoses, stenoses at the suture site have been the main concern. Mechanical dilatation is one way to prevent the stenosis, as suggested by Lauritzen. In the present study, 50 vessels (femoral and carotid) and 10 veins were used for sleeve anastomoses and the same numbers of vessels were used for conventional anastomoses (as control) to evaluate the effect of mechanical dilatation using resin corrosion cast (Mercox) because the Mercox cast facilitates three-dimensional stereoscopic views. Gradual dilatations around the suture sites were observed in seven carotid arteries, and three of seven resulted into aneurysm formation due to weakening of the inner vascular wall in the sleeve anastomosis. No dilatation or aneurysm was observed in the femoral arteries. Newly proliferating capillaries formed on the endothelial surfaces of the inner vascular walls around the suture sites after 4 weeks in the sleeve anastomoses. Operative time and endothelial trauma were markedly reduced with sleeve anastomoses. The gradual dilatation and aneurysm formation in the carotid arteries show that sleeve anastomoses should be used carefully for high-pressure arteries in clinical practice if mechanical dilatation is performed.

Arthroplasty of the elbow. Twenty years' experience of a new approach.

We report the use of a new approach for elbow arthroplasty in 58 cases over a 20-year period. A wide exposure, obtained by elevating the triceps attachment and dividing the radial collateral ligament, allows the excision of diseased tissue, articular irregularities and osteophytes. Normal anatomy is restored and active mobilisation can be started 10 days after operation. Good or fair results, with over 70 degrees of joint movement, were achieved in 88% of cases.

The duration of denervation and regeneration in nerve grafting. Quantitative histological assessment in the rat.

The common peroneal nerves of 15 male rats were severed and the rats were divided into four groups: immediate grafting (I group); grafting delayed one month (IM group); grafting delayed for three months (3M group) and 5 healthy rats were used as controls (control group). All the rats were sacrificed 5 months after grafting. Three morphological indices of the myelinated axons were evaluated, the mean number per unit area, their mean diameter and the axon packing density. The sites examined were, the proximal stump of the recipient nerve, the grafted nerve and the distal stump of the recipient nerve. When repair was carried out late there was atrophy of the axons within the proximal stump, indicating that delay affected axon regeneration as well as resulting in reduced activity in the distal stump. The "3M" group showed poor regeneration in comparison to the "I" group. However, in the "IM" group, the regeneration rate was similar to that of the "IM" group. Our results indicate that early secondary grafting should be considered when circumstances exist which make primary repair undesirable.

Postoperative results of looped nylon suture used in injuries of the digital flexor tendons.

Primary or delayed suture was performed on 174 patients involving 234 digits with flexor tendon injury on the day of injury to a maximum of 486 days after injury during the past eight years from 1974. In the treatment a 4-0 or 5-0 looped nylon suture was used. Follow-up study was made on 121 patients involving 164 digits in whom at least six months had elapsed since the operation. The evaluation showed the results in 53.0% excellent, 15.3% good, 18.9% fair and 12.8% poor. Discussion is made of a number of factors which have effects upon the results of treatment of flexor tendon injury of the hand.

New surgical procedure for correction of club hand.

Two cases of radial club hand were treated by a new surgical procedure. This procedure consisted of correction of the wrist deviation and pollicization. The metacarpal of the pollicized digit was used for radial osseous support.

Relationship between the number of digital nerves sutured and sensory recovery in replanted fingers.

The relationship between the number of digital nerves sutured and sensory recovery was investigated in 50 fingers of 33 patients, with follow-up of more than five years after replantation of the amputated fingers. Items of investigation were measurements of two-point discrimination (2PD) values and the threshold of touch sense. In cases of repair at the proximal phalanx, good sensory recovery was observed only with bilateral nerve repair. At the middle phalanx, the influence of the location of repair on sensory recovery was greater than that of the number of digital nerves sutured. Sensory recovery was better in those cases with repairs at a more distal location. Modes of sensory recovery were also studied.

Reconstruction of the heel pad by flexor digitorum brevis musculocutaneous flap transfer.

We strongly believe that rotational transfer of a musculocutaneous island flap composed of the flexor digitorum brevis muscle is the best procedure currently available for covering and reconstructing the weight-bearing area of the heel and its posterior area from the histologic and functional points of view. Further, since it requires only one operation, we are confident that it is the ideal method.

Surgical treatment of cleft hand and its associated deformities.

Surgical procedures frequently used in the treatment of the cleft hand and its related deformities are presented. The need to pay attention to the particular stage of the developmental mechanism producing the deformity before undertaking surgery is stressed, for in addition to closure of the central cleft, treatment must include correction of the various symptoms associated with the finger ray fusion.
