[A case of pancreatic cancer surviving for 7 years 11 months following intra-arterial and portal infusion chemotherapy].

A case of unresectable pancreatic cancer in a 76-year-old female was reported, in which a tumor had liver involvement. She was treated with infusion chemotherapy of mitomycin C (MMC) from gastroduodenal artery and portal vein after palliative operation. A drug was administered via an extracorporeal infusion catheter. MMC was given totally 47 mg from artery and 27 mg from portal vein for 9 months. Then, the serum CA 19-9 level gradually decreased, the metastatic liver tumor disappeared and the pancreatic tumor became smaller. After that, the tumor did not grow. The patient survived for 7 years 11 months until she died from other causes. Thus, it seems that infusion chemotherapy might be tried in some cases of pancreatic cancer.

[Two cases of spontaneous pneumomediastinum in healthy young men].

We report two cases of spontaneous pneumomediastinum. Both were young males, and had no causal diseases or apparent precipitating episodes. Both recovered rapidly with bed rest only. Symptoms, signs, radiological diagnosis, and pathogenesis of pneumomediastinum are discussed.

[Endoscopic treatment of early gastric cancer in patients undergoing chronic hemodialysis].

Endoscopic treatment of early gastric cancer (type Ila in all patients) was performed in four patients undergoing chronic hemodialysis. Surgical treatment was contraindicated because of various risk factors in these patients. Endoscopic treatment was performed by concomitant use of YAG laser irradiation, local ethanol injection and mucosal resection. After endoscopic treatment, stomach biopsy revealed no cancer in four patients. In one patient who is alive now, no sign of cancer recurrence has been noted for about two years after endoscopic treatment. This patient has undergone hemodialysis on outpatient department. The remaining three patients died from diseases other than gastric cancer. When one of them was autopsied, no gastric cancer demonstrated. Taking the prognosis related to life in to consideration, endoscopic treatment should be used positively when surgical treatment of early gastric cancer cannot be performed in patients undergoing chronic hemodialysis.
