[Abnormal uterine bleedings in premenopausal women--diagnosis, treatment and prevention].

PURPOSE: To reveal the reasons for the haemorrhages such as pregnancy, iatrogenic factors, systemic diseases and evident genital tract pathology among the cases with abnormal uterine bleedings (AUB) in premenopausal women by the way of elimination as well as to establish the histomorphological status of the endometrium in the cases with dysfunctional uterine bleedings (DUB) aiming at defining the necessity and the type of treatment. MATERIAL AND METHODS: The study covered 224 females with AUB. The following diagnostic methods were made use of: general and special history of the disease, inspection with valves and bimanual vaginal examination, vaginal ultrasonography, microbiological examination including diagnosis by using polymerase chain reaction (PCR), biopsy as well as abrasio probatoria separata. RESULTS: A total of 96 patients (42,86 per cent of the cases) presented with AUB of true organic origin. In 9 of them (4,02 per cent) the histomorphological examinations proved either a carcinoma, or atypical morphological findings. DUB that were pathogenetically related with disturbed rhythm of ovarian steroidogenesis were diagnosed in the rest 128 patients (57, 14 per cent). These females were administered a monohormonal gestagen treatment as a prevention measure against eventual relapses as well. CONCLUSION: A diagnostic distinguishing between the organic AUB and DUB in the premenopausal women with AUB is needed in order to sufficiently motivate both therapy and prevention.

[Menoflavon and lipid profile in menopausal women].

The purpose of the study was to assess the effect of Menoflavon on plasma lipoprotein levels in menopausal women. Twenty-eight female patients were examined and treated. After a 3-month treatment, the authors established a reduction of the total plasma cholesterol levels by 7.15%. This reduction was more considerable (by 11.45%) in the females presenting with higher risk values (> 6.20 mmol/L). After a 6-month treatment, however, the subsequent cholesterol reduction was insignificant and the plasma levels were stabilized. Menoflavon therapy diminished plasma LDL-cholesterol levels by 8.71% after a 3-months long course and by 9.5% after a 5-months long one in comparison with the initial levels. Menoflavon did not exert any effect on the plasma levels of HDL-cholesterol and triglycerides as well.

[Stepwise uterine devascularization in postpartum hemorrhages].

The authors reported the results from a retrospective investigation of 10121 deliveries for a three-year period (1981-1983) as well as from a prospective study of 6239 child-births during a three-year period after a 20-year-long time lag, i.e., in 2000-2002. The cases with early post-partum hemorrhages (EPH) and with accomplished laparotomy were examined. The causes for EPH, the obstetric therapeutic measures and manipulations prior to the laparotomy as well as the surgical interventions after opening the abdomen were considered. The authors emphasized that 20 years ago three uteri only could be "saved" by means of ligation of the uterine vessels. A hysterectomy was carried out in 21 cases with massive EPH after the laparotomy. After two decades the situation changed dramatically. The hysterectomies amounted to only 22.22% of the cases (n = 4) while in the rest females (n = 14) the copying of the massive hemorrhages was performed by means either of stepwise ligation of the uterine vessels, or of ligation of the hypogastric ones in 3 cases with additional vaginal lacerations. It was outlined that mother's mortality rate was zero during these two periods. The conclusion has been drawn that the ligation of the uterine vessels represents an alternative to the hysterectomy when copying the EPH and preserves the child-bearing functions of some females because of the subsequent vascular recanalization.

[Role of Gardnerella vaginalis in the development of sexually transmitted diseases--diagnostic criteria].

The authors analyze the results from the clinical and microbiological investigations of Gardnerella vaginalis in two female groups: 28 patients with clinical manifestations of bacterial vaginosis (BV) and a control group of 25 females without any complaints. Along with the data from the anamnesis and the gynecological examination, the following parameters have been examined: a native preparation for "clue cells", pH of vaginal content, ratio between polymorphonuclear leukocytes and epithelial cells, specimens for the diagnosis by polymerase-chain reaction (PCR) as well as cultural examination by using HBT agar (human blood two-layer Tween agar). The authors discuss five diagnostic criteria and emphasize that the diagnosis of BV requires the presence of at least four of them. Every criterion possesses a high frequency (between 67.86 and 100 per cent) among the female patients with clinical evidence of BV.

[Intranatal asphyxia, prematurity and congenital infection--their role for brain damage in very low birth weight newborns].

The authors examined 273 newborns weighing less than 1500 g. Their birth weight was very low (1001-1500 g) in 179 but extremely low (< 1000 g) in 94 newborns. Neurological damage was proved in 101 cases (36.99%). The probable reasons for intra- (peri-) ventricular and cerebral hemorrhages in these newborns were the following: respiratory distress syndrome (in 11 cases or 10.89%), mother-fetal infection (in 16 cases or 15.84%), and combination of asphyxia and infection (in 10 cases or 9.9%). The severe degree of prematurity (and immaturity) remained the only causative factor in the rest 64 premature newborns (63.37% of the cases). The results from the distribution of the neurological lesions according to the gestational age were also considered. Usage of monofactorial regression models detected statistically significant differences between asphyxia, infection and brain damage in the newborns of different gestational age.

[Eikenella corrodens in the etiology of tubo-ovarian abscesses in patients with intrauterine devices].

The authors analyzed the microbiological findings in 39 female patients with tubo-ovarian abscesses and intrauterine devices (IUD). An anaerobic microflora was isolated in 66.67% of the aspirates after puncture of cavum Douglasi (CD) as well as in 41.67% of the abdominal exudates or abscesses during laparotomy. Eikenella corrodens was isolated from CD in three patients while in two of them the same bacterium was intraoperatively isolated from the abdominal exudate, too. All the three patients had IUD and regular sexual intercourses, orogenital inclusive, with a lot of partners. Eikenella corrodens is an opportunistic pathogenic agent. It causes infection in case of disturbed local and general immunity under the conditions of traumatic mucosal lesions. It is resistant to numerous antibiotics.
